Extensive genetic diversity among clinical isolates of Streptococcus pyogenes serotype M5.

The genetic diversity of clinical isolates of Streptococcus pyogenes serotype M5 has been characterized. Strain genotypes were defined by macrorestriction profile, 16S ribotype, emm gene subtype, insertion element IS1239 profile, and exotoxin gene determinant. By these criteria, clinical isolates of M5 constituted a multiplicity of strain clusters rather than a homogeneous population as found for certain serotypes. Distance matrices and an unrooted tree were constructed from macrorestriction data with three rarely cutting endonucleases, determined by PFGE. A single IS1239 profile was common to

Uroporphyria in mice: thresholds for hepatic CYP1A2 and iron.

In mice treated with 5-aminolevulinic acid (ALA) and polyhalogenated aromatic compounds, the levels of both hepatic cytochrome P450 (CYP)1A2 and iron-which can be quite different among inbred strains-are critical in causing experimental uroporphyria. Here we investigate the development of uroporphyria as a function of CYP1A2 and iron levels in the liver of mice having a common C57BL/6 genetic background. We compared Cyp1a2(-/-) knockout mice, Cyp1a2(+/-) heterozygotes, Cyp1a2(+/+) wild type, and Cyp1a2(+/+) mice pretreated with a low dose of 3,3',4,4',5-pentachlorobiphenyl (PCB126) (4 microg/k

Impact of different paraspinal muscle mass on the prognosis of ACDF-a finite element analysis.

INTRODUCTION: Cervical spondylotic myelopathy (CSM) is a common degenerative disease of the cervical spine, for which anterior cervical discectomy and fusion (ACDF) serves as an effective surgical treatment. Recent studies have suggested that the quality of the paraspinal muscles, particularly the multifidus muscle, is closely related to postoperative outcomes; However, biomechanical evidence remains limited. The aim of this study is to investigate the biomechanical impact of varying paraspinal muscle mass on the cervical spine following ACDF. METHODS: A finite element model of the cervical s

A qualitative analysis of formerly incarcerated women's perspectives on a prison-based perinatal support program.

BACKGROUND: People who are pregnant while incarcerated are a high-risk population with complex and often unmet health care needs. Although perinatal support programs are becoming more common in carceral settings, limited research has examined individuals' interest in and experiences with these programs. This study explored formerly incarcerated perinatal individuals' knowledge about, participation in, and perceptions of currently existing and potential future components of a perinatal support program in a Mid-southern women's state prison. METHODS: Semi-structured interviews were conducted wi

Comparative study regarding the stability of a proximal ulnar stump with or without distal oblique bundle reconstruction during the Sauvé‒Kapandji procedure: a finite-element analysis.

BACKGROUND: The most common postoperative complication of the Sauvé‒Kapandji (S-K) procedure is proximal ulnar stump instability. The distal oblique bundle (DOB) is a stable tissue used to stabilize the distal radioulnar joint. This study created finite-element models of the distal oblique bundle (DOB) to determine its effect on the proximal ulnar stump instability encountered during the Sauvé‒Kapandji procedure. PURPOSE: We hypothesized that a proximal ulnar stump with distal oblique bundle reconstruction would provide greater stability than a proximal ulnar stump without distal oblique bund

Microbial community dynamics and coexistence in a sulfide-driven phototrophic bloom.

BACKGROUND: Lagoons are common along coastlines worldwide and are important for biogeochemical element cycling, coastal biodiversity, coastal erosion protection and blue carbon sequestration. These ecosystems are frequently disturbed by weather, tides, and human activities. Here, we investigated a shallow lagoon in New England. The brackish ecosystem releases hydrogen sulfide particularly upon physical disturbance, causing blooms of anoxygenic sulfur-oxidizing phototrophs. To study the habitat, microbial community structure, assembly and function we carried out in situ experiments investigatin

Noise, air pollutants and traffic: continuous measurement and correlation at a high-traffic location in New York City.

BACKGROUND: Epidemiological studies have linked both noise and air pollution to common adverse health outcomes such as increased blood pressure and myocardial infarction. In urban settings, noise and air pollution share important sources, notably traffic, and several recent studies have shown spatial correlations between noise and air pollution. The temporal association between these exposures, however, has yet to be thoroughly investigated despite the importance of time series studies in air pollution epidemiology and the potential that correlations between these exposures could at least part

Composite Biofidelity: Addressing Metric Degeneracy in Biomechanical Model Validation and Machine Learning Loss Design.

Spectral similarity is often judged with a single metric such as RMSE, yet this can be misleading: physically different errors can produce similar scores. This is a critical limitation for computational biomechanics, where spectral agreement underpins both model validation and machine-learning loss design. Here, we develop a multi-metric framework for objective spectral biofidelity and test whether it better captures meaningful disagreement across complex frequency-domain responses. We evaluated 12 complementary similarity metrics, including CORA and ISO/TS 18571, using controlled spectral per

The novel associations between frailty and neurodevelopmental outcomes in CHD.

OBJECTIVES: "Frailty" is associated with worse outcomes in adult cardiology. There is limited data on the associations between frailty and outcomes in paediatric cardiology. We aimed to define the prevalence of frailty and identify associations between frailty and neurodevelopmental and quality-of-life outcomes in high-risk paediatric cardiac populations. STUDY DESIGN: This cross-sectional study included patients 4-18 years seen in a neurodevelopmental programme between 6/2017 and 11/2022. Demographic and clinical data were obtained from medical records. As part of the routine care, physical

Functional stability analyses of maxillofacial skeleton bearing cleft deformities.

The symmetrically stable craniofacial bony structure supports the complex functions and delicate contour of the face. Congenital craniofacial deformities are often accompanied by bony defects and have been repetitively correlated with compromised dento-maxillary stability, but neither the extent nor the pattern of cleft-related maxillary instability has been explored in detail. Furthermore, it is largely unknown if the bony defect and related instability are correlated with secondary maxillary deformity common among patients with orofacial clefts. With the aid of finite element modeling, we st

Does the Severity of Arm Tremor in Essential Tremor Correlate with Impaired Balance?

BACKGROUND: Motor features aside from tremor are increasingly recognized in essential tremor (ET) patients. The relationship between these features and tremor has received sparse attention. We examined whether the severity of action tremor in the arms was correlated with the severity of tandem gait difficulty and balance confidence. METHODS: 212 ET cases enrolled in a prospective clinical study, from which baseline data on the following variables were analyzed: severity of action tremor (total tremor score [TTS] from the Washington Heights-Inwood Genetic Study of Essential Tremor rating scale

Clinical considerations for oral appliance use in obstructive sleep apnea patients with dental implants: a scoping review.

PURPOSE: Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder characterized by recurrent upper airway obstruction during sleep and associated with important systemic health risks. Oral appliance (OA) therapy, particularly mandibular advancement devices (MADs), is an established treatment option for patients with mild to moderate OSA and for those who are unable to tolerate continuous positive airway pressure. However, in patients with dental implants, prosthodontic and biomechanical factors may influence the indication, design, retention, and safety of OA therapy. Therefo

Effect of muscle activation scheme in human head-neck model on estimating cervical spine ligament strain from military volunteer frontal impact data.

Cervical spine (c-spine) injuries are a common injury during automobile crashes. The objective of this study is to verify an existing head-neck (HN) finite element model with military volunteer frontal impact kinematics by varying the muscle activation scheme from previous literature. Proper muscle activation will allow for accurate percent elongation (strain) of the c-spine ligaments and will serve to establish ligamentous response during non-injury frontal impacts. Previous human research volunteer (HRV) frontal impact sled tests reported kinematic data that served as the input for HN model

Vitiligo: a possible model of degenerative diseases.

Vitiligo is characterized by the progressive disappearance of pigment cells from skin and hair follicle. Several in vitro and in vivo studies show evidence of an altered redox status, suggesting that loss of cellular redox equilibrium might be the pathogenic mechanism in vitiligo. However, despite the numerous data supporting a pathogenic role of oxidative stress, there is still no consensus explanation underlying the oxidative stress-driven disappear of melanocytes from the epidermis. In this study, in vitro characterization of melanocytes cultures from non-lesional vitiligo skin revealed at

New insights into spontaneous intestinal perforation using a national data set: (2) two populations of patients with perforations.

BACKGROUND: Spontaneous intestinal perforation (SIP) is increasingly common in the premature infant and has been demonstrated to be associated with early postnatal administration of glucocorticoids and indomethacin. Before survival of the extremely low birth weight (ELBW) infant, SIP was thought to be a rare, congenitally acquired disease sporadically affecting the muscularis of the distal intestine. These disparate views of etiology have not been previously reconciled in the literature. OBJECTIVES: (1) To establish a cohort of SIP patients in a national data set. (2) To use timing of diagnos
