Mandibular cortical bone graft part 2: surgical technique, applications, and morbidity.

The mandibular cortical grafts, including the ramus graft, are reliable donor sites for the reconstruction of the alveolar ridge deformities before dental implant placement. The highest concentrations of promoter proteins (e.g., bone morphogenetic proteins) are found in the mandibular cortical grafts. Part 1 of this article (April issue) reviewed the anatomical considerations of mandibular cortical grafts, the associated healing process, and the factors influencing graft incorporation. This article describes mandibular ramus graft before implant placement including the surgical technique, appl

Does the use of a pterygoid osteotome affect pterygomaxillary fracture patterns in Le Fort I osteotomy?

OBJECTIVES: Safe separation of the pterygomaxillary junction (PMJ) is a critical step during Le Fort I (LF-I) osteotomy. The use of a pterygoid osteotome remains controversial due to its potential association with unfavorable fracture patterns and serious complications. This study aimed to evaluate the effect of pterygoid osteotome use on PMJ fracture patterns in horizontal and vertical planes using computed tomography (CT). MATERIALS AND METHODS: This retrospective study included 83 patients who underwent LF-I osteotomy between 2022 and 2025 and had postoperative CT imaging within the first

Three dimensional evaluation of soft tissue after orthognathic surgery.

BACKGROUND: To evaluate the nasolabial soft tissue change three-dimensionally after orthognathic surgery, using a structured light scanner. METHODS: Thirty-two malocclusion patients, who underwent orthognathic surgery, were evaluated. CBCT and 3D facial scans were obtained before surgery and 3 months after surgery. The 3D changes in the 26 landmarks, and the relative ratio of the soft tissue movement to the bony movement, were evaluated. RESULTS: In the Le Fort I advancement patients, the nasal tip moved 17% forward, compared to the maxillary bony movement, but the nasal prominence decreased

Modified maxillary reconstruction technique for Brown's class III defects by subdividing a rhomboid shaped iliac crest into 2 subunits.

BACKGROUND: Men and women share a common maxillary morphology with heterogeneity in size. This indicated that our technique of reconstruction with a rhomboid-shaped iliac crest bone flap incorporating a 30° vertical wedge osteotomy may be widely used for Brown's class III maxillectomy defect reconstruction among a population with class I skeletal profile. The reconstruction of Brown's class III maxillary defects is extremely challenging. The purpose of this study was to closely study the maxilla geometrically in order to establish a standardized maneuver, which facilitates conversion of the il

Individualized surgical approach for Brodie's syndrome with facial asymmetry: a case report involving mandibular segmentation.

Brodie's syndrome is a rare transverse malocclusion, challenging to manage, particularly in adults with significant skeletal involvement. This report describes an unusual clinical case of surgical treatment for a unilateral presentation of Brodie's syndrome using mandibular segmentation, an approach rarely documented in the literature. The patient, an 18-year-old, presented with mandibular deviation, Class II malocclusion, and posterior scissors bite, with an adequately wide maxilla but evident mandibular constriction. Three-dimensional planning guided a bimaxillary surgical intervention that

Crestal Sinus Augmentation in the Presence of an Antral Pseudocyst.

PURPOSE: Antral pseudocysts classically have a well-defined, hyperdense, unilocular, dome-shaped appearance in radiographs. The best therapeutic approach for treating these cysts in the context of sinus floor augmentation remains controversial. This article presents a new technique that allows both sinus membrane elevation and cyst removal through a crestal approach in patients with pseudocysts in the maxillary sinus. MATERIALS AND METHODS: The technique combined aspiration of the cysts during sinus floor elevation with a sinus augmentation procedure using a hydraulic sinus elevation system.

Saphenous vein graft for bypass of the maxillary to supraclinoid internal carotid artery: an anatomical short study.

The use of a saphenous vein graft for bypass of the maxillary artery (MA) to the supraclinoid internal carotid artery (ICA) in internal carotid occlusions is investigated. Five adult cadaver sides were used. Dissection required zygomatic arch osteotomy and a pterional craniotomy with extensive removal of the floor of the middle cranial fossa. The MA was found easily medial to infratemporal crest. The clinoidal segment of the ICA was exposed with the removal of the anterior clinoid process intradurally. The bypass graft was 4 to 5 cm long and was sutured end-to-end to the MA and end-to-side to

Safe Crestal Sinus Elevation Below 3 mm Residual Bone with Tissue-Level Implant Placement: A Case Report.

Background and Clinical Significance: Maxillary sinus augmentation is a well-established surgical procedure for dental implant placement in the posterior maxilla when the residual alveolar bone height is insufficient. Traditionally, the lateral approach has been preferred in cases with less than 4 mm of bone; however, the crestal approach has emerged as a less invasive alternative, particularly with the advent of advanced techniques and tools such as hydraulic pressure systems and dedicated osteotomy kits. Case Presentation: This case report presents the clinical management of a 68-year-old fe

Long-term patient-reported outcomes after anterior distraction osteogenesis of the maxilla in patients with cleft.

Maxillary growth inhibition in patients with cleft lip and palate (CLP) is an undesired effect that may occur in the teens despite proper primary care. Dental malocclusion and distortion of facial appearance can be treated with external distraction osteogenesis (DO) of the maxilla. This entails a Le Fort I osteotomy, fastening a semi-circular distractor to the skull, distraction for three weeks, and fixation for three months before removal of the device. The aim of this descriptive long-term follow-up study was to evaluate DO of the maxilla from the patient-reported long-term perspective.Fourte

Maxillary Changes Following Facial Bipartition - A Three-Dimensional Quantification.

INTRODUCTION: Children with Apert syndrome have hypertelorism and midfacial hypoplasia, which can be treated with facial bipartition (FB), often aided by rigid external distraction. The technique involves a midline osteotomy that lateralizes the maxillary segments, resulting in posterior cross-bites and midline diastema. Varying degrees of spontaneous realignment of the dental arches occurs postoperatively. This study aims to quantify these movements and assess whether they occur as part of a wider skeletal relapse or as dental compensation. METHODS: Patients who underwent FB and had high qua

[Orthognathic surgery in adults: state of the art].

The objective of orthognathic surgery is the repositioning of the osseous bases of the jaws. Its consequences are both aesthetic and functional. As adults are increasingly concerned with appearance, orthognathic surgery is today, more than ever, a close collaboration between the orthodontist, surgeon and general dentist. The whole team must be capable of coordinating its approach with an aesthetic objective. For the surgeon, a perfect knowledge of complementary techniques (rhinoplasty, bone grafts, plastic surgery of the face, lipostructure or fillers) makes orthognathic surgery a completely s
