Impact of alcohol exclusion law repeal on inpatient alcohol-related diagnoses.

BACKGROUND: Alcohol exclusion laws, which allow insurers to deny coverage for intoxication-related injuries, are a form of structural stigma that may discourage clinical documentation. We examined whether state alcohol exclusion law repeal was associated with changes in documented inpatient alcohol-related diagnoses. METHODS: Data were drawn from 132,917,864 adult inpatient discharge records from the Healthcare Cost and Utilization Project State Inpatient Databases (1999-2020). Outcomes were diagnoses of alcohol use disorder, alcohol intoxication, and laboratory evidence of alcohol in blood.

Socioeconomic position, alcohol use and alcohol-attributable emergency department visits.

BACKGROUND: Differential vulnerability to alcohol contributes to socioeconomic inequities in alcohol-attributable harm. This study aimed to estimate the sex-/gender-specific joint effects of socioeconomic position (SEP) and heavy episodic drinking or volume of alcohol use on 100% alcohol-attributable emergency department (ED) visits. METHODS: We conducted a cohort study among 36 900 men and 39 700 women current and former alcohol consumers aged 15-64 from population-representative Canadian Community Health Surveys (2003-2008) linked to administrative ED visit data through 2017 in Ontario and

Nonhepatic Manifestations of Alcohol Use Disorder and Alcohol-Related Liver Disease.

Alcohol use disorder (AUD) impacts many patients with alcohol-associated liver disease (ALD), affecting more than just the liver. ALD and AUD influence multiple organ systems, including neurologic, cardiovascular, gastrointestinal, hematologic, endocrine, and psychiatric. Recognizing these extrahepatic manifestations is essential for comprehensive patient care. Understanding how alcohol use affects these systems helps health care providers manage the full spectrum of health issues in patients with ALD. Addressing both liver-related and systemic effects of alcohol use ensures a more thorough an

Effect of inhibition of reactivated alcohol-associated memories with propranolol on alcohol craving.

BACKGROUND: Alcohol craving and relapse occur after the reactivation of alcohol reward memory. Previous studies suggest that drug-associated memory undergoes reconsolidation once retrieved by drug-associated stimuli. This study hypothesized that propranolol administration during memory reconsolidation induced by conditioned stimulus (CS) would significantly attenuate alcohol craving. METHODS: A total of 40 patients with alcohol dependence who met the diagnostic criteria for alcohol dependence in DSMV were enrolled. The patients were randomized located into the memory retrievalpropranolol grou

Phosphatidylethanol and ethyl glucuronide to categorize alcohol consumption in alcohol-related cirrhosis.

BACKGROUND & AIMS: Phosphatidylethanol (PEth) is an alcohol-use biomarker that could bridge the detection windows of urinary ethyl glucuronide (uEtG) and scalp hair ethyl glucuronide (hEtG), but has been rarely validated in patients with liver disease. Reported detection windows of these biomarkers also vary significantly, and available studies have focused solely on any alcohol use. Yet, categorizing patients with liver disease based on their level of alcohol use would be highly informative. Here, we assessed the diagnostic accuracy and optimal detection windows of whole-blood PEth, uEtG, hEt

Tetramethylpyrazine: A fermented alcohol product that mitigates alcoholic liver disease in mice.

Alcoholic liver disease (ALD) is a leading cause of premature death globally yet remains under-controlled. In this study, we investigated the protective effects of tetramethylpyrazine (TMP), an aromatic compound found in fermented alcohol, against ALD in a National Institute on Alcohol Abuse and Alcoholism (NIAAA) mice model. Our results demonstrated that TMP significantly reduced alcohol-induced liver injury, steatosis, oxidative stress, and mitochondrial damage, while restoring NAD+ levels and the NAD+/NADH ratio, increasing ATP production, regulating energy metabolism disorders, and restori

Management of alcohol withdrawal syndrome in patients with alcohol-associated liver disease.

Alcohol-associated liver disease is a common and severe sequela of excessive alcohol use; effective treatment requires attention to both liver disease and underlying alcohol use disorder (AUD). Alcohol withdrawal syndrome (AWS) can be dangerous, is a common barrier to AUD recovery, and may complicate inpatient admissions for liver-related complications. Hepatologists can address these comorbid conditions by learning to accurately stage alcohol-associated liver disease, identify AUD using standardized screening tools (eg, Alcohol Use Disorder Identification Test), and assess risk for and sympto

Nutritional deficiencies in alcohol use disorder/alcohol-associated liver disease.

PURPOSE OF REVIEW: To delineate common and uncommon dietary and nutritional deficiencies in individuals with chronic heavy alcohol use and alcohol use disorder and to highlight important advances in the nutrition field in patients ranging from those with alcohol use disorder (AUD) and no liver disease to those with decompensated alcohol-associated liver disease (ALD). RECENT FINDINGS: Patients with AUD may have nutritional deficiencies, especially isolated nutrient deficiencies, such as thiamine or zinc deficiencies. This should not be surprising, as alcohol is a major source of "empty calori

Takotsubo Syndrome After Alcohol Withdrawal in a Patient With Suspected Alcoholic Cardiomyopathy.

Takotsubo syndrome is a non-ischemic cardiomyopathy characterized by transient left ventricular (LV) apical ballooning, which typically occurs after exposure to emotional or physical stress in elderly women. An 85-year-old woman with hypertension presented with a recent onset of palpitation and exertional dyspnea. The patient had a long-standing history of alcohol consumption, and transthoracic echocardiography revealed diffuse LV hypokinesia including apical area with an ejection fraction of 30%. The patient was suspected of alcoholic cardiomyopathy and was recommended to quit alcohol consump

Alcohol-Associated Liver Disease: Integrated Management With Alcohol Use Disorder.

Alcohol-associated liver disease (ALD) is the most common cause of cirrhosis and liver-related mortality in many regions worldwide. Around 75% of patients with cirrhosis are unaware of their disease until they are referred to the emergency department. An innovative, noninvasive screening approach is required for an earlier diagnosis of liver fibrosis. In patients with ALD the physician is inevitably dealing with 2 major disorders: the liver disease itself and the alcohol use disorder (AUD). Focus only on the liver disease will inevitably lead to failure because transient improvements in liver

Alcohol use and hepatocellular carcinoma risk in patients with alcohol-related cirrhosis.

OBJECTIVES: Insights into risk factors for hepatocellular carcinoma (HCC) among patients with alcohol-related cirrhosis (ALD cirrhosis) are important for decisions about HCC surveillance. We studied the effects of continued hazardous alcohol use in ALD cirrhosis on HCC risk. METHODS: Within a nationwide registry-based cohort of patients with ALD cirrhosis, we compared HCC risk between patients with a continued hazardous alcohol use and matched comparators. We used Fine-Gray regression to compare the risk of HCC and Cox regression to compare all-cause mortality. We also included patients with

Prognostic models in alcohol-related liver disease and alcohol-related hepatitis.

Alcohol-associated liver disease (ALD) and alcohol-associated hepatitis (AH) are dynamic disorders whose prognosis can be challenging to determine. A number of prognostic models have been developed to determine likelihood of death, when to refer for liver transplant (LT) and the role for glucocorticoids. Often these models were created with a specific application in mind but were found to have additional applications with further study. Those prognostic models that have stood the test of time are easy to use, have clear interpretations and employ objective parameters. These parameters most oft

Effects of Moderate Alcohol Consumption in Non-Alcoholic Fatty Liver Disease.

Alcoholic liver disease (ALD) and non-alcoholic fatty liver disease (NAFLD) have emerged as leading causes of chronic liver diseases worldwide. ALD and NAFLD share several pathophysiological patterns as well as histological features, while clinically, they are distinguished by the amount of alcohol consumed daily. However, NAFLD coexists with moderate alcohol consumption in a growing proportion of the population. Here, we investigated the effects of moderate alcohol consumption on liver injury, lipid metabolism, and gut microbiota in 30 NAFLD-patients. We anonymously assessed drinking habits,

Alcohol dependence and the ventral hippocampal influence on alcohol drinking in male mice.

Examining neural circuits underlying persistent, heavy drinking provides insight into the neurobiological mechanisms driving alcohol use disorder. Facilitated by its connectivity with other parts of the brain such as the nucleus accumbens (NAc), the ventral hippocampus (vHC) supports many behaviors, including those related to reward seeking and addiction. These studies used a well-established mouse model of alcohol (ethanol) dependence. After surgery to infuse DREADD-expressing viruses (hM4Di, hM3Dq, or mCherry-only) into the vHC and position guide cannula above the NAc, male C57BL/6J mice wer

Patterns of Alcohol Use After Early Liver Transplantation for Alcoholic Hepatitis.

BACKGROUND & AIMS: Early liver transplantation (LT) for alcoholic hepatitis (AH) is lifesaving but concerns regarding return to harmful alcohol use remain. We sought to identify distinct patterns of alcohol use post-LT to inform pre-LT candidate selection and post-LT addiction care. METHODS: Detailed post-LT alcohol use data was gathered retrospectively from consecutive patients with severe AH at 11 ACCELERATE-AH sites from 2006-2018. Latent class analysis identified longitudinal patterns of alcohol use post-LT. Logistic and Cox regression evaluated associations between patterns of alcohol us

Cost-effectiveness of alcohol use treatments in patients with alcohol-related cirrhosis.

BACKGROUND & AIMS: Alcohol use treatment such as medication-assisted therapies (MATs) and counseling are available and effective in promoting alcohol abstinence. We sought to explore the cost-effectiveness of different alcohol use treatments among patients with compensated alcohol-related cirrhosis (AC). METHODS: We simulated a cohort of patients with compensated AC receiving care from a hepatology clinic over their lifetimes. We estimated costs (in 2017 US$) and benefits in terms of quality-adjusted life years (QALYs) gained from healthcare and societal perspectives. Transition probabilities

[The role of Cytochrom P4502E1 in Alcoholic Liver Disease and alcohol mediated carcinogenesis].

Various factors are involved in the pathogenesis of alcoholic liver disease (ALD) and ethanol-mediated carcinogenesis. In addition to genetic, epigenetic and immunologic mechanisms, acetaldehyde-associated toxicity, oxidative stress as well as cytokine-mediated inflammation are of major importance. Oxidative stress, with the generation of reactive oxygen species (ROS), develops either in inflammation (alcoholic hepatitis) or during oxidation of ethanol via cytochrome P4502E1 (CYP2E1). CYP2E1 is induced by ethanol, oxidizes ethanol to acetaldehyde, and generates ROS during this process. ROS res

Exploring the utility of alcohol flushing as an instrumental variable for alcohol intake in Koreans.

Previous studies have indicated an association of higher alcohol intake with cardiovascular disease and related traits, but causation has not been definitively established. In this study, the causal effect of alcohol intake on hypertension in 2,011 men and women from the Ansan-Ansung cohort was estimated using an instrumental variable (IV) approach, with both a phenotypic and genotypic instrument for alcohol intake: alcohol flushing and the rs671 genotype (in the alcohol dehydrogenase 2 [ALDH2] gene), respectively. Both alcohol flushing and the rs671 genotype were associated with alcohol intak
