Radiation dose in pneumatic reduction of ileo-colic intussusceptions--results from a single-institution study.

BACKGROUND: Air enema under fluoroscopy is a well-accepted procedure for the treatment of childhood intussusception. However, the reported radiation doses of pneumatic reduction with conventional fluoroscopy units have been high in decades past. OBJECTIVE: To compare current radiation doses at our institution to past doses reported by others for fluoroscopic-guided pneumatic reduction of ileo-colic intussusception in children. MATERIALS AND METHODS: Since 2007 radiologists and residents in our department who perform reduction of intussusceptions have received a radiation risk training. We re

Sparing the posterior surgical site when planning radiation therapy for thoracic metastatic spinal disease.

BACKGROUND CONTEXT: Most patients with painful spinal metastases are sufficiently palliated by hypofractionated radiotherapy. However, a small group of patients will need surgical intervention to treat symptomatic spinal cord compression and/or gross mechanical instability. Irradiation of a (prospective) surgical area may lead to postsurgery complications, including wound dehiscence, infection, and chronic wound ulcers. Decreasing the radiation dose to the surgical area could reduce radiation-induced toxicity and associated surgical complications. PURPOSE: To investigate an alternative radiat

Clinical behavior of radiation-induced thyroid cancer: factors related to recurrence.

BACKGROUND: Whether thyroid cancer is more aggressive in radiation-exposed patients is not resolved. The frequency of aggressive features in post-Chernobyl patients suggests this may be the case. Our aim was to address this question by re-examining the pattern of risk factors for recurrence of thyroid cancers found in a cohort exposed to external radiation. METHODS: The study population was drawn from a cohort of 4296 people, followed since 1974, who were treated before the age of 16 with conventional external radiation for benign conditions of the head and neck between 1939 and the early 196

Crohn's disease: factors associated with exposure to high levels of diagnostic radiation.

AIMS: Exposure to diagnostic radiation may be associated with increased risk of malignancy. The aims of this study were to (1) examine patterns of use of imaging in Crohn's disease; (2) quantify the cumulative effective dose (CED) of diagnostic radiation received by patients; and (3) identify patients at greatest risk of exposure to high levels of diagnostic radiation. METHODS: 409 patients with Crohn's disease were identified at a tertiary centre. CED was calculated retrospectively from imaging performed between July 1992 and June 2007. High exposure was defined as CED>75 mSv, an exposure le

Nasopharyngectomy after failure of 2 courses of radiation therapy.

BACKGROUND: Recurrence of nasopharyngeal carcinoma after initial therapy has been reported to range between 18% and 54%. As an alternative to surgical salvage, patients with recurrent nasopharyngeal carcinoma are offered a second course of radiation therapy. If this second course fails, patients may be candidates for surgical resection. OBJECTIVE: To identify the effectiveness and morbidity of surgical resection of recurrent nasopharyngeal carcinoma in patients who have received 2 cycles of external beam radiation. DESIGN AND SETTING: Retrospective survey of 6 patients in a university-based

Selective neck dissections for squamous carcinoma of the upper aerodigestive tract: patterns of regional failure.

BACKGROUND: Surgeons have been using selective neck dissections in the treatment of squamous carcinoma of the upper aerodigestive tract for over 20 years. To date, no data is available that can answer the question "What are the patterns of failure in the neck following a selective neck dissection and is a selective neck dissection a reliable procedure for metastatic disease?" METHODS: To answer this question, the medical records of all patients with squamous carcinoma of the oral cavity, oropharynx, larynx, and hypopharynx treated at The University of Texas M. D. Anderson Cancer Center from J

[Long-term radiation damage to the skin and eye after combined beta- and gamma- radiation exposure during the reactor accident in Chernobyl].

BACKGROUND: In April 1986, numerous reactor workers and firemen were exposed to high doses of ionizing radiation during the Chernobyl nuclear power plant accident. Apart from high ambient gamma-ray exposures they received inhomogeneous contamination with beta-rays from fission products, resulting in severe skin exposure. PATIENTS AND METHODS: Sixteen of these so called Liquidators were repeatedly examined between 1991 and 1996. Their doses ranged from 0.35 to 9 Gy, partly confirmed by determination of chromosomal aberrations. Ophthalmologic examination included non-subjective assessment of le

Pathologic femoral fracture after periosteal excision and radiation for the treatment of soft tissue sarcoma.

BACKGROUND: Surgical resection and adjuvant radiation therapy are standard therapy for soft tissue sarcomas. When the tumor approximates bone, periosteal excision may be necessary. It was hypothesized that periosteal stripping and radiation therapy would increase the rate of pathologic fracture. METHODS: The soft tissue sarcoma data base at the Memorial Sloan-Kettering Cancer Center was used to identify a consecutive series of 205 patients who were treated over a 15-year period (1982-1997). All patients had a soft tissue sarcoma of the thigh, which was managed by limb-sparing surgery and radi

Childhood leukaemia in Belarus, Russia, and Ukraine following the Chernobyl power station accident: results from an international collaborative population-based case-control study.

BACKGROUND: There is little evidence regarding the risk of leukaemia in children following exposure to radionuclides from the Chernobyl Nuclear Power Plant explosion on April 26, 1986. METHODS: This population-based case-control study investigated whether acute leukaemia is increased among children who were in utero or <6 years of age at the time of the Chernobyl accident. Confirmed cases of leukaemia diagnosed from April 26, 1986 through December 31, 2000 in contaminated regions of Belarus, Russia, and Ukraine were included. Two controls were matched to each case on sex, birth year, and resi

Interventional Management of Chronic Neuropathic Cervical Pain Following Nerve Sheath Tumor Resection and Radiation: A Case Report.

BACKGROUND: Chronic neuropathic pain after head and neck cancer is common and difficult to treat, especially when compounded by surgical and radiation-related neural injury. This case illustrates persistent superficial cervical plexopathy after tumor resection and adjuvant radiation with limited response to standard therapy. CASE REPORT: A man in his 30s developed constant, allodynic left anterolateral neck pain after resection of a carotid triangle nerve sheath sarcoma and postoperative radiation. Conservative measures (physical therapy, topical agents, gabapentin) provided minimal relief. A

Targeting p65 to inhibit Cas3 transcription by Onjisaponin B for radiation damage therapy in p65+/- mice.

BACKGROUND: In response to radiation injury, p65 becomes activated. The formation of p65 is one target of Onjisaponin B (OB), but it has not been studied in radioprotection. In addition, there is a binding site for p65 in the promoter region of Cas3. This study evaluates the use of OB as an intervention to modulate p65/Cas3 following radiation exposure. PURPOSE: This study aimed to confirm that OB regulated the transcription of Cas3 via p65 to overcome radiation-induced damage. STUDY DESIGN AND METHODS: Cells and mice were exposed to X-rays at a dose of 6 Gy. Immunofluorescence was used to l
