Deep learning-based classification of lower extremity arterial stenosis in computed tomography angiography.

PURPOSE: The purpose of this study is to develop and evaluate a deep learning model to assist radiologists in classifying lower extremity arteries based on the degree of arterial stenosis caused by plaque in lower extremity computed tomography angiography (CTA) of patients with peripheral artery disease. METHODS: In this retrospective study, 265 patients who underwent lower-extremity CTA between January 1, 2016 and October 31, 2019 were selected. A total of 17050 axial images of iliac, femoropopliteal and infrapopliteal artery from these patients were used for the training and validation of t

Duplex ultrasound versus CT angiography for the treatment planning of lower-limb arterial disease.

PURPOSE: This study aimed to assess the diagnostic accuracy of duplex sonography (DUS) compared with that of computed tomography angiography (CTA) in detecting occlusion and stenosis in peripheral arterial disease (PAD) in candidate patients for endovascular revascularization with intraprocedural digital subtraction angiography (DSA). METHODS: The study involved 94 patients suffering from PAD who were candidates for endovascular procedures requiring DSA. They were all submitted preoperatively to DUS and CTA. Based on image analysis, five segments of the arterial tree were assessed: iliac, com

Anaphylactic Shock After Indocyanine Green Video Angiography During Cerebrovascular Surgery.

BACKGROUND: Indocyanine green (ICG) video angiography has been widely used in cerebrovascular surgery. ICG injection is generally safe, with a low incidence of complications. ICG-related anaphylactic reactions during neurosurgery have been rarely reported. We report the cases of 2 patients who had experienced anaphylactic shock in response to intravenous ICG injection (DID Indocyanine Green [Dongindang, Inc., Gyeonggi-do, Republic of Korea]) during intracranial aneurysm (IA) surgery. CASE DESCRIPTION: The first patient, a 69-year-old woman with an unruptured IA, had been undergoing clipping s

Acute Recanalization of a Partially Thrombosed Large Intracranial Aneurysm.

BACKGROUND: Thrombosed large intracranial aneurysms (TLIAs) are not continuously contemplated as stable lesions. Spontaneous recanalization of completely occluded large intracranial aneurysms has been described previously. CASE DESCRIPTION: We report a middle-aged patient presenting with agitation, acute headache, visual field defects, and left hemiparesis. A large thrombosed posterior communicating (PCom) artery aneurysm was identified with an infarct at the same arterial territory on neuroimaging studies. Digital subtraction angiography (DSA) performed 1 week later demonstrated complete rec

Moyamoya disease with occlusion of bilateral vertebral arteries and the basilar artery fed by the collateral vessels of vertebral arteries: A rare case report.

The main change of moyamoya disease (MMD) is steno-occlusion at the terminal portion of the bilateral internal carotid arteries (ICAs). Occlusion of distal portions of the bilateral vertebral arteries (VAs) in MMD has never been reported. An 18-year old female was admitted with repeated headache for 13years. Cerebral digital subtraction angiography (DSA) demonstrated occlusion of bilateral ICAs at their terminal portions, moyamoya vessels at the skull base, occlusion of bilateral distal VAs and the basilar artery (BA) fed by the tortuous collateral vessels arising from the cervical segment of

Aetiologies of internal carotid artery pseudo-occlusions in acute stroke patients: what neurointerventionalists can expect.

In cases of acute stroke, differentiation between an occluded and a patent internal carotid artery (ICA) is crucial for diagnosis and management. Although CT angiography (CTA) can be highly accurate in defining high-grade stenosis and ICA occlusions, misleading ICA occlusion patterns are not rare in patients with acute stroke. We investigated the underlying causes of ICA pseudo-occlusions with CTA with respect to digital subtraction angiography. 11 out of 72 patients had pseudo-occlusion on CTA. Of these, there were three cases of tandem occlusions accompanying high-grade ICA stenosis, five ca

Cerebral ischemic involvement in Vogt-Koyanagi-Harada disease.

BACKGROUND: Vogt-Koyanagi-Harada (VKH) disease is an autoimmune disorder characterized by ocular, auditory and neurological manifestations (headache, meningismus and/or aspeptic meningoencephalitis). PATIENT: We describe a 12-year-old African boy with bilateral uveitis who presented with acute unilateral hearing loss and neurological symptoms such as left-sided dyskinesias, unsteady gait and throbbing headache. Brain magnetic resonance imaging showed ischemic lesions of the right basal ganglia in the territory of lenticulostriate and thalamic arteries. He improved after treatment with intrave

Coil embolization of an aneurysm associated with an infraoptic anterior cerebral artery in a child.

We report an unusual case of a ruptured aneurysm from an infraoptic anterior cerebral artery in an 11-year-old child. To our knowledge, this is the second such case treated with Guglielmi detachable coils in the pediatric population that has been described in the literature, excluding that of a vein of Galen aneurysm that resulted in remarkable recovery. We conclude that the endovascular technique is a feasible treatment option for children with ruptured cerebral aneurysms, especially when more complex vascular anomalies are present.

Intracranial aneurysm: anatomic factors that predict the usefulness of intraoperative angiography.

PURPOSE: To correlate the size and location of intracranial aneurysm with the need to reposition the aneurysm clip after intraoperative angiography. MATERIALS AND METHODS: In 199 consecutive patients with 234 clipped intracranial aneurysms, 273 intraoperative angiographic studies were retrospectively reviewed. Aneurysm size and location, determined with preoperative angiographic and surgical reports, were correlated with the frequency of clip repositioning because of parent- or branch-vessel compromise or unexpected residual aneurysm. RESULTS: Findings from intraoperative angiograms resulted

[Diagnostic angiography in life-threatening colonic hemorrhage in Crohn disease].

Life-threatening colonic hemorrhage is a very rare condition in Crohn's disease. Lower gastrointestinal hemorrhage in Crohn's disease occurs in 0.6-2.5% of cases. In a patient presenting with severe gastrointestinal bleeding after ileocolectomy, mesenteric angiography provided precise localisation of the bleeding site in the area of the splenic flexure.

Painless lateral neck swelling revealing a carotid body tumour.

A female in her mid-60s complained of a painless, slowly growing swelling on the right side of the neck. A firm pulsating expansile mass was noted on clinical examination without neurological restriction. Imaging studies (CT angiography, MRI and digital subtraction angiography) demonstrated a well-defined hypervascular lesion in the carotid bifurcation and a sign of easy splaying of the internal and external carotid arteries, which confirmed the diagnosis of a carotid body tumour (CBT). Transcervical surgical removal of the tumour was performed. Histopathologic examination findings were consis

Resolution of Trigeminal Neuralgia After Transvenous Embolization of a Cavernous Sinus Dural Arteriovenous Fistula.

BACKGROUND: Trigeminal neuralgia (TN) resulting from a dural arteriovenous fistula (DAVF) are rare. A case of cavernous sinus dural arteriovenous fistula (CSDAVF) in a patient with TN that resolved immediately after transvenous embolization (TVE) is described. CASE DESCRIPTION: A 75-year-old woman presented with continuous facial pain. On time-of-flight magnetic resonance angiography (MRA), a CSDAVF was suspected, and digital subtraction angiography (DSA) showed a CSDAVF. The TN completely resolved immediately after TVE, with no recurrence. Follow-up MRA showed complete obliteration of the fi

Carotid body tumors.

The surgical management of carotid body tumors requires identification and preservation of neural and vascular structures without compromising resection of the neoplasm. Fifteen patients were examined and treated for carotid body tumors at the Cleveland (Ohio) Clinic Foundation from 1979 through 1987. The benchmark of diagnosis is bilateral carotid angiography. When neural structures are free of tumor, meticulous dissection facilitates their preservation. Large tumor size increases risk for arterial resection necessitating reconstruction. The use of a vascular shunt minimizes the risk of cereb

Cardiovascular evaluation before circulatory arrest for removal of vena caval extension of renal carcinoma.

The use of cardiopulmonary bypass, deep hypothermia and circulatory arrest has decreased the risks of hemorrhage, tumor embolization, incomplete thrombus resection, and warm hepatic and renal ischemia associated with resection of renal cell carcinoma extending into the inferior vena cava above the hepatic veins. Patients about to undergo this operation frequently have significant coronary artery and carotid artery disease, and are at risk for perioperative myocardial infarction and stroke. Preoperative evaluation of the coronary artery and carotid artery circulation by coronary angiography, du
