In Vivo Base Editing Partially Rescues Bone Dysplasia in a Mouse Model of Hutchinson-Gilford Progeria Syndrome.

Hutchinson-Gilford progeria syndrome (HGPS) is a premature aging disorder affecting tissues of mesenchymal origin. Most patients harbor a c.1824C>T/p.G608= variant, commonly described as G608G, in exon 11 of LMNA that leads to aberrant splicing and production of the toxic progerin protein. In addition to cardiovascular, dermal, and adipose tissue deterioration, HGPS mouse models also develop progressive bone dysplasia that occurs in patients. Here we characterize the efficacy of in vivo mutation correction with an adenine base editor (ABE) to rescue structural and functional defects in HGPS tr

Severe dysphagia after a posterior cervical spine fusion at the C1-C3 level and its improvement after correction surgery for malalignment: a case report.

BACKGROUND: The development of dysphagia and trismus following posterior C1-C3 fusion is rare compared to occipitocervical fusion, and there are very few reports in the literature. CASE PRESENTATION: A 75-year-old Thai man who had an extradural tumor at the C1/C2 level developed severe dysphagia and trismus immediately after tumor resection and C1-C3 fusion. During the surgery for malalignment correction, the C1-C2 screws were re-aligned into an increased lordotic position. The symptoms resolved immediately post-surgery. CONCLUSIONS: Short upper cervical fusion can produce trismus and dyspha

Metabolic effects of obesity causing disease in childhood.

PURPOSE OF REVIEW: Childhood obesity is rising to epidemic proportions throughout the world, and much emphasis has been placed on the long-term consequences that can result later, in adulthood. This article reviews the metabolic consequences of obesity that can manifest as disease during the childhood years. RECENT FINDINGS: Obese children suffer from many disease processes once thought to affect only adults. They can have type 2 diabetes mellitus, and potentially early β cell failure with rapid progression to an insulin requirement. There is a high prevalence of fatty liver disease in obese

Familial occurrence of glenoid dysplasia: report of two cases in two consecutive generations.

Glenoid dysplasia is a rare abnormality of the shoulder. We report glenoid dysplasia in two consecutive generations: a boy and his father. Both suffered recurrent shoulder dislocations, and radiological examination revealed bilateral glenoid dysplasia. Our cases confirm dominant inheritance of this osseous malformation.

New form of bone dysplasia with multiple fractures associated with monosomy X.

We report on the clinical, radiologic, and pathologic findings in a 20-week-old fetus with monosomy X and severe hydrops associated with fetal dwarfism. The fetus presented with osteoporosis, bent bones, multiple fractures, and distinctive symmetric submetaphyseal transverse bone interruptions or pseudofractures. We excluded by radiologic and histopathologic examination the diagnoses of osteogenesis imperfecta, hypophosphatasia, campomelic dysplasia, achondrogenesis, hypochondrogenesis, and other types of bone dysplasia. To our knowledge, this is a previously undescribed bone dysplasia associa

Hip abnormalities in children with Charcot-Marie-Tooth disease.

Hip dysplasia was recently observed in association with Charcot-Marie-Tooth disease (CMT). We retrospectively reviewed available radiographs of 74 of 100 children with clinically and electrodiagnostically proven CMT and noted six cases of hip dysplasia. Minor hip abnormalities, most commonly increased neck shaft angles, were noted in 21 other patients. Type I usually causes more weakness and had more hip dysplasia than type II. A female predominance was noted but may have resulted from sampling of more immature radiographs in males. Most dysplasia was asymptomatic and was detected only on scre

Hirschsprung disease associated with polydactyly, unilateral renal agenesis, hypertelorism, and congenital deafness: a new autosomal recessive syndrome.

An association of Hirschsprung disease with polydactyly, unilateral renal agenesis, hypertelorism, and congenital deafness is described in sibs (brother and sister) of consanguineous parents. It is suggested that this might represent a new autosomal recessive syndrome.

Pattern recognition in bone dysplasias.

Genetically different bone dysplasias may manifest themselves in similar patterns of skeletal abnormalities. It is proposed to group these similar dysplasias in 'families' for two reasons: 1 The knowledge of developmental patterns shared by different genetic disorders cautions the diagnostician and encourages a two-step procedure: a) provisional recognition of a pattern and b) more careful analysis of the pattern to reach a final, specific diagnosis. 2 Families of bone dysplasias may be the result of similar pathogenetic mechanisms. Once the mechanism is discovered in one member of the family,

A newly recognized syndrome of connective tissue dysplasia in siblings (previously described as a variant of Morquio disease).

Siblings (one male and one female) with a striking combination of multiple skeletal abnormalities, hypermobility in some joints with a restricted range of movements in others, mesodermal dysgenesis of the iris and cutaneous atrophy with thin skin, multiple telangiectases, shallow ulcers, and café au lait lesions are described. The patients were reported in early childhood as cases of Morquio disease (mucopolysaccharidosis IV) with previously unrecognized skin changes. The results of specific enzyme assays exclude a diagnosis of both of the known biochemical types of Morquio disease; the evolut

Hip dysplasia without dislocation in one-year-old boys.

Six boys were examined during the second year of life, each with symptoms in one hip. The affected femoral capital epiphysis was smaller or more irregular (or both) than that of the unaffected hip but was neither subluxed nor dislocated. The subsequent radiographs revealed changes similar to those in severe Perthes' disease. Nevertheless, we give reasons for believing that this disorder in boys under two years of age should be distinguished from Perthes' disease.

Case Report: Late-onset liver cirrhosis in an elderly patient with FBN1-related Geleophysic Dysplasia.

BACKGROUND: FBN1-related geleophysic dysplasia (GD2) is an ultrarare autosomal dominant disorder caused by dominant-negative missense variants in the TGFβ-binding protein-like domain 5 (TB5) of FBN1. It is characterized by severe short stature, brachydactyly, progressive joint stiffness, and cardiac valvular disease. Although hepatomegaly is known to occur in GD, progression to cirrhosis with portal hypertension in the elderly has never been documented. CASE PRESENTATION: A 76-year-old Chinese man presented with a 1-year history of abdominal distension and was found to have liver cirrhosis du

Olfactory bulb anomalies in KBG syndrome mouse model and patients.

ANKRD11 (ankyrin repeat domain 11) is a chromatin regulator and the only gene associated with KBG syndrome, a rare neurodevelopmental disorder. We have previously shown that Ankrd11 regulates murine embryonic cortical neurogenesis. Here, we show a novel olfactory bulb phenotype in a KBG syndrome mouse model and two diagnosed patients. Conditional knockout of Ankrd11 in murine embryonic neural stem cells leads to aberrant postnatal olfactory bulb development and reduced size due to reduction of the olfactory bulb granule cell layer. We further show that the rostral migratory stream has incomple

Eight years experience from a skeletal dysplasia referral center in a tertiary hospital in Southern India: a model for the diagnosis and treatment of rare diseases in a developing country.

We report on a series of 514 consecutive diagnoses of skeletal dysplasia made over an 8-year period at a tertiary hospital in Kerala, India. The most common diagnostic groups were dysostosis multiplex group (n = 73) followed by FGFR3 (n = 49) and osteogenesis imperfecta and decreased bone density group (n = 41). Molecular confirmation was obtained in 109 cases. Clinical and radiographic evaluation was obtained in close diagnostic collaboration with expert groups abroad through Internet communication for difficult cases. This has allowed for targeted biochemical and molecular studies leading to

Postoperative complications after surgical management of incomplete ossification of the humeral condyle in dogs.

OBJECTIVE: To describe incidence and type of postoperative complications in the surgical management of incomplete ossification of the humeral condyle (IOHC) and identify any risk factors associated with development of these complications. STUDY DESIGN: Case series. METHODS: Clinical records of dogs (n=57) that had prophylactic transcondylar screw insertion for treatment of IOHC (79 elbows) at 6 UK referral centers were reviewed. Signalment, presentation, surgical management, postoperative care, and complications were recorded. Postoperative complications were divided into seroma, surgical si

D-2-hydroxyglutaric aciduria in association with spondyloenchondromatosis.

D-2-hydroxyglutaric aciduria is a rare metabolic disorder, first reported in 1980, and does not yet have a clinically specific presentation pattern nor any specific treatment regime. We report a girl born with this uncommon metabolic disorder, who, at the age of 12 months, was also found to have a severe crippling form of skeletal dysplasia, spondyloenchondromatosis.
