[Postero-medial elastic stable intramedullary nailing for anteriorly displaced distal diaphyso-metaphyseal fractures of the radius in children].

PURPOSE: To suggest a mechanically satisfying method to stabilize the unfrequent distal diaphyso-metaphyseal fractures of the radius, when they are anteriorly displaced, avoiding the classical use of an anterior osteosynthesis by plate and screws. MATERIAL AND METHODS: Ten cases of postero medial Elastic Stable Intramedullary Nailing are presented. Technically, a small postero medial approach with gentle retraction of extensors tendons is used. Then a hole is drilled through the postero medial metaphysis of the distal radius. A present wire is introduced intramedullary, directing its elastic

Reconstruction of fingertip amputations by partial composite toe transfer with short vascular pedicle.

Five patients under 15 years-of-age, with a fingertip amputation through the nail plate, were treated with a custom-made partial toe transfer. Two of the patients had had previous attempts at surgical reconstruction using either a local flap or replantation. Delay between initial injury and reconstruction ranged from 2 to 60 days. In all cases the flap was harvested from the second toe. This "custom-made" compound transfer included the exact amount of pulp, nail bed and bone required for reconstruction. All flaps were harvested on a short vascular pedicle, with anastomoses performed at a digit

Treatment of supracondylar femoral fractures with a retrograde intramedullary nail.

From August 1992 to January 1995, 24 patients with 26 supracondylar femoral fractures were treated with a retrograde intramedullary nail. There were 22 patients with 24 nails available for review. Eight fractures were open and 13 were intraarticular fractures. There were a significant number of associated injuries. The average followup interval was 18 months (range, 4-36 months). All fractures healed by 4 months, (average, 3 months). Only 1 patient required bone grafting. There were no implant failures or superficial or deep infections. One malunion occurred. Average knee range of motion was 1

Nail plate combination in the upper extremity: surgical technique and clinical application.

Nail plate constructs (NPC) have shown promising results in complex lower extremity peri-articular fractures as well as in peri-prosthetic fractures. The combination of both implants allows for improved mechanical stability and immediate weight bearing. The use of NPC has not been described in the upper extremity in the literature. We herein describe potential indications and surgical technique for NPC usage for complex upper extremity trauma and reconstruction.

[Fingertip injuries in children: 81 cases with at least one year follow-up].

PURPOSE OF THE STUDY: Fingertip crush injuries are frequent in young children. Rigorous care is required to avoid esthetic and functional sequelae, but treatment is often difficult. We report our experience focusing on simple therapeutic principles useful for everyday practice in the emergency room. MATERIAL AND METHODS: We reviewed retrospectively 91 fingertip injuries in 81 children followed for at least one year. Mean age of the children was 4.5 years. Crush injuries predominated (82%) and mainly involved the middle finger. We recorded the initial lesions, therapeutic interventions, and ea

Harmful lifestyles on orthopedic implantation surgery: a descriptive review on alcohol and tobacco use.

Alcohol abuse and smoking habits have adverse effects on bone health and are a risk factor for osteoporosis, fractures and impaired fracture repair. Osteointegration processes around implanted biomaterials involve a coordinated cascade of complex events that are very similar to those occurring during fracture repair and require a suitable microenvironment and the coordinated action of cells and signal molecules. Therefore, diseases and harmful lifestyles that impair the normal bone healing process can reduce the success of implant surgery and may negatively influence the osteointegration of pr

Salvage of a Broken Magnetic Tibia Nail with Plate Fixation Without Nail Removal: A Case Report.

INTRODUCTION: Magnetic intramedullary nails are used for deformity correction and limb lengthening because of their minimally invasive nature and reduced complication rates compared to external fixators. However, complications such as non-union and nail breakage remain rare but significant clinical challenges, particularly in cases with patient-specific risk factors. This case demonstrates a successful union after nail fracture without the removal of the intramedullary nail, using lateral plate fixation, offering a salvage option in this uncommon scenario. CASE REPORT: A 22-year-old female wi

Reamed versus unreamed intramedullary nailing for the treatment of femoral fractures: A meta-analysis of prospective randomized controlled trials.

BACKGROUND AND OBJECTIVE: Intramedullary nailing is commonly used for treating femoral shaft fractures, one of the most common long bone fractures in adults. The reamed intramedullary nail is considered the standard implant for femoral fractures. This meta-analysis was performed to verify the superiority of reamed intramedullary nailing over unreamed intramedullary nailing in fractures of the femoral shaft in adults. Subgroup analysis of implant failure and secondary procedure was also performed. METHODS: Electronic literature databases were used to identify relevant publications and included

RIA: one community's experience.

The Reamer Irrigator Aspirator (RIA) has three main indications in our community trauma practice. The most common indication for RIA is harvesting of autololgous bone graft from the femur for nonstructural bone graft. The second most common indication is for irrigation and debridement of intramedullary osteomyelitis. The final indication for RIA is for acute nailing of femoral shaft fractures in patients with multiple long bone fractures with or without pulmonary injury. If one pays careful attention to the details of the technique, RIA is a safe and effective method of harvesting autologous b

[Hip fractures].

Hip fractures are ranked among the frequent injuries. These fractures have been often coupled with high energy trauma in children and in patients with normal bone structure, low energy trauma and osteoporotic fracture (fragility fracture) is typical in elder patients. Hip fractures are divided into five groups: femoral head fracture, femoral neck fracture, pertrochanteric, intertrochateric and subtrochanteric fracture. Surgical treatment is indicated in all patients unless contraindications are present. Long bed rest has been accompanied by a high risk of development of thromboembolic disease,

Role of poly-herbo-mineral combination in management of kunakha (paronychia).

BACKGROUND: Paronychia, a bacterial or fungal infection affecting the area where the nail meets the skin, often impacts individuals engaged in frequent water-related work. Typically resolving within a week, the condition can persist and worsen with exposure to water, chemicals, or unclean substances, leading to chronic infection and significant cosmetic concerns. In Ayurveda, this condition is referred to as Kunakha, a term originating from "Ku" (bad) and "Nakha" (nail) in Hindi. Kunakha, associated with Asthi dushti (Bone tissue) and characterized by nail discoloration, pain, and chronicity,

Reamed compared with unreamed nailing of tibial shaft fractures: Does the initial method of nail insertion influence outcome in patients requiring reoperations?

BACKGROUND: Patients with a tibial shaft fracture experiencing their first postoperative complication following treatment with intramedullary nails may be at greater risk of subsequent complications than the whole population. We aimed to determine whether the initial method of nail insertion influences outcome in patients with a tibial shaft fracture requiring multiple reoperations. METHODS: Using the Study to Prospectively Evaluate Reamed Intramedullary Nails in Tibial Shaft Fractures trial data, we categorized patients as those not requiring reoperation, those requiring a single reoperation

TFNA Implant Failure with Helical Blade Cut-out.

INTRODUCTION: Trochanteric femoral nail-advanced (TFNA) was introduced in the market with better nail design, better alloy (titanium molybdenum) and both sliding and static locking options of the helical blade. Although, it was devised to overcome the shortcomings of roximal Femoral Nail Anti-rotation (PFNA), it still can have complications, if the principles of fracture management are not met. Here, we report a case of a TFNA implant failure with helical blade cut-out in an elderly osteoporotic patient treated for inter-trochanteric femur fracture. To the best of our knowledge, this is the fi

A prospective randomised controlled trial of the fibular nail versus standard open reduction and internal fixation for fixation of ankle fractures in elderly patients.

AIMS: The fundamental concept of open reduction and internal fixation (ORIF) of ankle fractures has not changed appreciably since the 1960s and, whilst widely used, is associated with complications including wound dehiscence and infection, prominent hardware and failure. Closed reduction and intramedullary fixation (CRIF) using a fibular nail, wires or screws is biomechanically stronger, requires minimal incisions, and has low-profile hardware. We hypothesised that fibular nailing in the elderly would have similar functional outcomes to standard fixation, with a reduced rate of wound and hardw

Longitudinal erythronychia: the value of cosmetic alterations in nail findings.

A 71-year-old man presented to the authors' clinic for evaluation of a red line under his right thumb. He noticed a red streak develop during the past year. It slowly grew in width and become more prominent in color (Figure 1). It did not cause pain. He delayed presentation because he perceived it to be only a cosmetic issue. Medical history included a metastatic atypical carcinoid tumor to the liver, lung, and the bone diagnosed 9 years ago. He had undergone multiple debulking surgeries and was currently taking octreotide and zoledronic acid. He had not started any new medications in the past

A new anatomical repair method for the treatment of ingrown nail: prospective comparison of wedge resection of the matrix and partial matricectomy followed by lateral fold advancement flap.

Currently, there are various surgical treatment modalities for ingrowing nail. None of these procedures are perfect to achieve esthetic results with low cost, recurrence, and complication rates. Eighty-seven toenails of 77 patients were operated in our clinic; 49 wedge matrix resections (WMR) and 38 partial matricectomy followed by lateral fold advancement flap (LFAF) were applied. Average follow-up period of the patients was 13 months. The recurrence rates, spicule formation, immobilization periods, and patient satisfaction for cosmetic result and discomforting symptoms were investigated. Nai
