Predictors of minor versus major stroke during carotid artery stenting: results from the carotid artery stenting (CAS) registry of the Arbeitsgemeinschaft Leitende Kardiologische Krankenhausärzte (ALKK).

BACKGROUND: Patient outcome, quality of life as well as health care costs differ between patients with minor versus major stroke during carotid artery stenting. Evaluation of predictors for both subtypes of strokes is of paramount importance. METHODS AND RESULTS: We analyzed data from the prospective, web-based German carotid artery stenting (CAS) registry. All patients entered in this registry were included as of January 2011. During the periprocedural period (until patient discharge or transfer) 1.5 % of the patients (85/5,794) sustained a major and 1.3 % (75/5,784) a minor stroke (total pe

Failure of old and new anticoagulants to prevent ischemic stroke in high-risk atrial fibrillation: a case report.

Rivaroxaban is an oral anticoagulant that acts as a direct, competitive factor Xa inhibitor. Large randomized clinical trials have shown that, at a daily dose of 20 mg, Rivaroxaban is at least as effective as dose-adjusted warfarin for the prevention of stroke or other embolic complications in patients with nonvalvular atrial fibrillation (AF). The safety and efficacy of combining Rivaroxaban with an antiplatelet agent for secondary stroke prevention has not been established. We report the case of an elderly patient with permanent AF and coronary heart disease, who had already suffered an isch

Frequent Ischemic Stroke as First Manifestation of Occult Colon Cancer: A Rare Case.

BACKGROUND: It is rare that occult cancer presents with frequent ischemic stroke as the sole manifestation. CASE REPORT: We report the case of a 46-year-old man with frequent stroke in different vascular areas, with diabetes and hypercholesterolemia identified as risk factors. The results of biochemistry, young stroke profiles, trans-cranial and extra-cranial carotid Doppler, and 24-h Holter electrocardiogram were within normal limits. The reports of transthoracic echocardiography (TTE) were unremarkable. Finally, a multi-detector computed tomography (MDCT) demonstrated a thrombus in the post

Aspirin and heparin in acute ischaemic stroke in older patients.

Over four-fifths of all strokes are due to thrombotic or embolic occlusion of cerebral arteries. There is a strong rationale for considering antithrombotic therapy for the treatment of patients with acute ischaemic stroke. Antiplatelet therapy with 150 to 300 mg of aspirin (acetylsalicylic acid) started within the first 48 hours of an ischaemic stroke improves patient outcome in the short and long term, with a low risk of adverse effects. Anticoagulants such as heparin may reduce the risk of developing deep venous thrombosis and pulmonary embolism in patients with stroke, but randomised contro

Increased direct oral anticoagulant use and event rates in non-valvular atrial fibrillation: a nationwide retrospective registry study in Sweden.

RATIONALE: The use of direct oral anticoagulants (DOACs) as stroke prophylaxis in patients with non-valvular atrial fibrillation (NVAF) has increased steadily since the introduction in 2011. In Sweden today, more patients are treated with DOACs than with warfarin. However, it is not shown that an increased proportion of DOAC prescriptions correlates to lower event rates of stroke and systemic embolism. OBJECTIVES: This study aims to investigate whether the increased prescription of DOACs in Sweden correlates with lower event rates for all-cause stroke, systemic embolism and bleeding complicat

Vascular distribution of stroke and its relationship to perioperative mortality and neurologic outcome after thoracic endovascular aortic repair.

OBJECTIVE: This study assessed the vascular distribution of stroke after thoracic endovascular aortic repair (TEVAR) and its relationship to perioperative death and neurologic outcome. METHODS: A retrospective review was performed for patients undergoing TEVAR between 2001 and 2010. Aortic arch hybrid and abdominal debranching cases were excluded. Demographics, operative variables, and neurologic complications were examined. Stroke was defined as any new focal or global neurologic deficit lasting>24 hours with radiographic confirmation of acute intracranial pathology. RESULTS: Perioperative

Percutaneous Left Atrial Appendage Closure in a Patient With Acquired Von Willebrand Disease and Atrial Fibrillation.

Left atrial appendage closure (LAAC) can be used to prevent embolic events in patients with atrial fibrillation who cannot tolerate oral anticoagulants. LAAC has not yet been performed in patients with acquired von Willebrand syndrome. A 74-year-old male with von Willebrand disease presents to the emergency department because of palpitations. Atrial fibrillation with congestive heart failure, hypertension, age ≥75, diabetes, stroke, vascular disease, age between 65-74, and female sex (CHA2DS2-VASC) of 4 was diagnosed. Oral anticoagulation was withheld because of a past medical history of major

An evidence-based evaluation of left ventricular thrombus treatment, outcomes, and resolution: a systematic review, pooled analysis and meta-analysis.

Left ventricular thrombus (LVT) is a recognized complication of acute myocardial infarction which is associated with stroke. There has yet to be a published systematic review that focuses on outcomes for patients with LVT. We conducted a systematic review on treatments, adverse events and thrombus resolution in patients with LVT. Meta-analysis and numerical pooling were used to evaluate the difference in outcomes based on treatment and the presence or absence of LVT. A total of 39 studies were included (5475 patients with LVT and 356 589 patients with no LVT). The use of direct oral anticoagul

Effectiveness and safety of dual-layer stents in carotid artery disease: a systematic review.

INTRODUCTION: Carotid artery stenting (CAS) has emerged as a minimally invasive alternative to carotid endarterectomy (CEA) in atherosclerotic carotid artery disease, even though the risk for procedural stroke after CAS remains significantly higher. Recently, in order to reduce embolic cerebral events after CAS, a new generation of dual-layer stents (DLS) has been developed. This review aimed to perform a detailed analysis of the available evidence on safety and efficacy of DLS in both symptomatic and asymptomatic atherosclerotic carotid artery stenosis. EVIDENCE ACQUISITION: A systematic rev

Clinical Outcomes of Sentinel Cerebral Protection System Use During Transcatheter Aortic Valve Replacement: A Systematic Review and Meta-Analysis.

OBJECTIVE: To compare the clinical outcomes following transcatheter aortic valve replacement (TAVR) with and without the use of the Sentinel Cerebral Protection System (Sentinel CPS). BACKGROUND: Stroke occurs in 2-5% of patients at 30 days after TAVR and increases mortality >3 fold. The Sentinel CPS is the only FDA (Food and Drug Administration) approved cerebral embolic protection device. METHODS: The Cochrane Library, PubMed and Web of Science were searched for relevant studies for inclusion in the meta-analysis. Two authors independently screened and included studies comparing the clinic

Stroke after transcatheter aortic valve replacement: incidence, risk factors, prognosis, and preventive strategies.

The first transcatheter aortisc valve replacement (TAVR) was performed in 2002, and has been proven beneficial in inoperable and high-risk patients for open heart surgery. Stroke occurrence after TAVR, both periprocedure and at follow-up, has not been well described. We sought to review incidence, pathophysiology, predictors, prognosis, and current preventive strategies of cerebrovascular accidents (CVAs) after TAVR. Studies were selected from a Medline search if they contained clinical outcomes data after TAVR. Acute and subacute CVAs after TAVR have been reported in 3% to 6% of patients. App

Unprotected carotid artery stenting in modern practice.

BACKGROUND & PURPOSE: Embolic protection devices (EPD) may provide a mechanism to reduce peri-procedural strokes. They are advocated by consensus guidelines and mandated for Medicare reimbursement. However, outcomes data remain mixed. We aimed to characterize the population of patients undergoing unprotected carotid artery stenting (CAS) and assess the utility of distal filter EPD (F-EPD) in elective CAS. METHODS: We analyzed patients enrolled in the CARE Registry® undergoing CAS between May, 2005 and January, 2012. We assessed the relationship between distal F-EPD use versus no use (No-EPD)

Cervical spondylosis: a rare and curable cause of vertebrobasilar insufficiency.

BACKGROUND: Spondylotic vertebral artery (VA) compression is a rare cause of vertebrobasilar insufficiency and stroke. CASE DESCRIPTION: A 53-year-old man experienced multiple brief vertebrobasilar transient ischemic attacks (TIAs) and strokes, not apparently triggered by neck movements. Brain magnetic resonance imaging (MRI) documented consecutive infarcts, first in the left then right medial posterior inferior cerebellar artery (PICA) territories. Angiography showed two extracranial right vertebral artery (VA) stenoses, left VA hypoplasia, absence of left PICA and a dominant right PICA. Com

Impact of embolic protection methods versus access types on outcomes of carotid artery stenting.

OBJECTIVE: Carotid artery stenting (CAS) is commonly performed through transfemoral or transcarotid access. Neurologic protection methods utilized during the procedure include distal embolic protection (DEP) and flow reversal devices. Multiple studies demonstrated more favorable outcomes associated with transcarotid artery revascularization with flow reversal (TCAR) in comparison to transfemoral carotid artery stenting with DEP (TFCAS-DEP). However, not many studies compared TCAR with transcarotid artery stenting with DEP (TCAS-DEP) or TFCAS with flow arrest by proximal balloon occlusion (TFCA

Percutaneous left atrial appendage suture ligation using the LARIAT device in patients with atrial fibrillation: initial clinical experience.

OBJECTIVES: The purpose of the study was to determine the efficacy and safety of left atrial appendage (LAA) closure via a percutaneous LAA ligation approach. BACKGROUND: Embolic stroke is the most devastating consequence of atrial fibrillation. Exclusion of the LAA is believed to decrease the risk of embolic stroke. METHODS: Eighty-nine patients with atrial fibrillation were enrolled to undergo percutaneous ligation of the LAA with the LARIAT device. The catheter-based LARIAT device consists of a snare with a pre-tied suture that is guided epicardially over the LAA. LAA closure was confirme…

Spontaneous healing of cervical pseudoaneurysm in vertebral artery dissection under anticoagulant therapy.

We report a 41-year-old woman with embolic stroke of the mid-pons attributed to embolism from vertebral artery dissection. Angiography revealed an occluded artery on one side and an incidental pseudoaneurysm of the midcervical portion of the vertebral artery on the other. After 3 months of warfare in therapy control angiography showed complete occlusion of the pseudoaneurysm. We discuss therapeutic choices and review the literature.

[Two-dimensional echocardiography in 100 cases of unexplained cerebral ischemic complications].

One-hundred patients with unexplained cerebral ischaemic events were explored by two-dimensional echocardiography. Thirty-nine patients showed abnormalities considered causative of embolic stroke. The most frequent cardiopathy found was mitral valve prolapse (38.5 per cent). This study suggests that unexplained cerebral ischaemic events should be evaluated by two-dimensional echocardiography and that the most frequent source of emboli to be expected is mitral valve prolapse.

Paradoxical brain embolism in a young man: is it only a patent foramen ovale?

Paradoxical embolism is considered the major cause of cerebral ischemic events in young patients. The most common cause of paradoxical embolism, which has been widely described, is right-to-left shunting (RLS) at cardiac level through a patent foramen ovale (PFO). Rarely paradoxical embolism can also be caused by RLS at pulmonary level due to pulmonary arteriovenous fistula (PAVF). Herein, we present a case of a young man, who experienced transient ischemic attack (TIA) due to paradoxical embolism, in whom both abovementioned abnormalities coexisted. This coincidence is very rare (noted in onl

[Paradoxical brain embolism].

The presence of right-to-left shunt (RLS), which can result in paradoxical brain embolism, is an important etiology of ischemic stroke especially in young adults. However, the characteristic features of RLS and paradoxical brain embolism have not been recognized in detail, especially in Japan. Patent foramen ovale (PFO) and pulmonary arterio-venous fistula are the most important causes of RLS. According to the registered clinical data of 7,245 acute stroke patients obtained by the Strategies Against Stroke Study for Young Adults in Japan (SASSY-Japan), PFO is more frequent in young patients (n
