Artificial intelligence across the continuum of care for the aging musculoskeletal patient: From longitudinal phenotyping to personalized surgery and recovery monitoring.

Musculoskeletal care is being reshaped by population aging, multimorbidity, frailty, osteoporosis, sarcopenia, polypharmacy and increasing vulnerability to postoperative loss of independence. Artificial intelligence (AI) has entered orthopaedic research, but its clinical translation remains constrained. Many models address isolated diagnostic, imaging or short-term prediction tasks rather than the longitudinal care trajectories that determine outcomes in older musculoskeletal patients. The central limitation is therefore not simply whether older adults are represented in datasets, but whether

Accuracy of Real-Time Data Provided by Mechanical Insufflation-Exsufflation Devices.

Background: Mechanical insufflation-exsufflation (MI-E) is crucial to assist patients with impaired cough, especially those with neuromuscular diseases. Despite recent advancements that enable real-time display of peak expiratory flow (PEF) and inspiratory volume, accurately monitoring these parameters with MI-E devices during treatment can still present challenges. Methods: A bench study that used a mechanical lung connected to 3 MI-E devices (EOVE-70; E-70 and Comfort Cough II) was conducted to evaluate PEF and inspiratory volume monitoring accuracy. Two clinical conditions were tested, low

The impact of hypoxia and hyperventilation on outcome after paramedic rapid sequence intubation of severely head-injured patients.

BACKGROUND: An increase in mortality has been documented in association with paramedic rapid sequence intubation (RSI) of severely head-injured patients. This analysis explores the impact of hypoxia and hyperventilation on outcome. METHODS: Adult severely head-injured patients (Glasgow Coma Scale score of 3-8) unable to be intubated without neuromuscular blockade underwent paramedic RSI using midazolam and succinylcholine; rocuronium was administered after confirmation of tube position. Standard ventilation parameters were used for most patients; however, one agency instituted use of digital

Biomechanical changes after reverse total shoulder arthroplasty: a systematic review of advanced measurement technologies.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) consistently improves pain and function; however, the biomechanical mechanisms underpinning post-operative function and joint-level adaptations remain incompletely defined. Advanced measurement tools, including motion capture, electromyography (EMG), inertial measurement units, and accelerometry, enable objective assessment of joint motion, muscle activation, and real-world arm use. This systematic review synthesizes evidence from studies using advanced biomechanical methods to evaluate post-operative kinematics, upper-limb activity, and m

Morphological and Thermographic Factors of the Lower Limbs Before Competition and Their Impact on Performance at the Spanish National Cross Country Championships.

INTRODUCTION: Cross-country running performance is influenced by a complex interaction of physiological, biomechanical, and morphological factors. Recently, infrared thermography (IRT) has emerged as a non-invasive method to assess skin temperature (TSK) and detect potential asymmetries associated with neuromuscular status, fatigue, and injury risk. However, limited evidence exists regarding its relationship with competitive performance in endurance athletes. METHODS: An observational study, conducted with STROBE guidelines, included 24 national-level cross-country athletes competing in the 2

In vivo characterization of Achilles subtendon function and morphology within the tendon cross section and along the free tendon.

The Achilles tendon is composed of three distinct fascicle bundles, or subtendons, each originating from the head of one of the three triceps surae muscles. In a healthy tendon, these subtendons slide relative to each other during muscle contractions. This subtendon sliding is reduced in older adults and individuals who suffer from an Achilles tendon injury. However, subtendon sliding is challenging to quantify in low-load scenarios that are critical for monitoring subtendon biomechanics in patients with mechanically compromised tendons, such as following an Achilles tendon rupture and repair.

Comparison of onset time, duration of action, and intubating conditions after cisatracurium 0.15 mg/kg in young and elderly patients.

INTRODUCTION: Tracheal intubation during anesthesia can be facilitated by the neuromuscular blocking agent cisatracurium. However, limited data exists about onset time, duration of action and effect on intubating conditions in elderly patients above 80 years of age. We hypothesized that elderly patients would present a longer onset time and duration of action compared to younger adults. METHODS: This prospective observational study included 31 young (18-40 years) and 29 elderly (≥ 80 years) patients. Patients were given fentanyl 2 μg/kg and propofol 1.5-2.5 mg/kg for induction of anesthesia a

[Primary hyperparathyroidism: clinical symptoms, diagnostic significance and localisation--a retrospective analysis].

BACKGROUND: The primary hyperparathyroidism (pHPT) is one of the most frequent metabolic diseases. Due to improvement in diagnostics it is a point of interest whether patients with pHPT still suffer from the typical symptoms. Moreover, the question of the best localisation-diagnostics and the most frequent localisation of the adenoma is not yet clarified. New operation strategies and more cost-efficient strategies for diagnostic and therapy could be developed due to the clinically established electrochemiluminescence immunoassay for intraoperative monitoring of intact parathyroid hormone (iPTH

Creatine kinase and neuromuscular fatigue responses following differing spells of simulated cricket fast bowling.

The fast-bowling action demands repetitive high-intensity whole body movements, imposing complex physical and perceptual demands on players that vary significantly throughout the season. This study aimed to assess and establish practical methods and metrics for quantifying fatigue after four simulated fast bowling spells. Eleven senior club male fast bowlers (age 27.3 ± 7.0 y; body mass 83.7 ± 11.6 kg; height 1.80 ± 0.06 m) completed four different bouts of the modified Cricket Australia-Australian Institute of Sport bowling skills test over consecutive weeks. Cholecalciferol (vitamin D₃) improves functional recovery when delivered during the acute phase after a spinal cord trauma.

In a previous study, based on a rat model of thoracic spinal cord compression, we demonstrated that cholecalciferol (Vitamin D3), delivered at the dose of 200 IU/kg/day, significantly improved ventilatory frequency and spasticity. In order to confirm the restorative potential of vitamin D, we performed a new study, using a rat model of left cervical hemisection (C2). From Day 1 or Day 7, animals received, during three months, a weekly oral bolus of either cholecalciferol, at the dose of 500 IU/kg/day, or vehicle, namely triglycerides. Rats were assessed every month, using a ladder test for sen

Effect of a vecuronium-induced partial neuromuscular block on hypoxic ventilatory response.

BACKGROUND: A previous study has demonstrated a decrease in the hypoxic ventilatory response in volunteers partially paralyzed with vecuronium. However, in this study, hypocapnia was allowed to occur. Because hypocapnia counteracts the ventilatory response to hypoxia during partial vecuronium-induced neuromuscular block and isocapnia, the hypoxic ventilatory response (HVR) was tested in 10 awake volunteers. METHODS: To avoid hypocapnia, the resting hyperoxic control end-tidal PCO2 was increased to 43.3 +/- 2.4 mmHg, raising inspiratory minute ventilation (VI) to 140 ml.kg-1.min-1. Hypoxic ven

Effect of enflurane and fentanyl on the clinical characteristics of long-term succinylcholine infusion.

The characteristics of the neuromuscular block produced by prolonged succinylcholine infusion were compared in 40 patients anaesthetized with either nitrous oxide with enflurane (1-2 per cent inspired) or nitrous oxide and fentanyl. Neuromuscular transmission was monitored using train-of-four stimulation and the infusion rate was adjusted to keep the first twitch at 10-15 per cent of its control value. Initially, all patients, exhibited a depolarizing-type block all twitches of the train-of-four being roughly the same size, and the infusion rates were similar in the enflurane (54 microgram X k
