Pectoralis Minor Transfer for Internal Rotation Reconstruction in Brachial Plexus Birth Injuries.

PURPOSE: A subset of patients with brachial plexus birth injuries experiences a loss of internal rotation, resulting either from neurological paralysis, as a complication of surgical procedures intended to restore external rotation, or because of contractures of the external rotators. Although humeral internal rotation osteotomy can address this deficit, it often results in compromised external rotation. To mitigate this drawback, in supple shoulders, we investigated the pectoralis minor tendon transfer to the subscapularis footprint as an alternative. METHODS: Five patients with active inter…

Analysis of posterior deltoid function one year after surgical restoration of elbow extension.

PURPOSE: The purpose of this study was to measure the extent and timing of elbow extension torque recovery after posterior deltoid-to-triceps tendon transfer. METHODS: Elbow extension moment was measured in 40 limbs from 23 patients who underwent surgical restoration using the posterior deltoid-to-triceps tendon transfer at times ranging from 8 weeks to 1 year after surgery. For comparison purposes, elbow extension moment also was measured in healthy controls and persons with C7 spinal cord injuries. RESULTS: Maximum extension moment was 5.89 +/- 0.24 Nm (mean +/- standard error of mean, n =…

[Reconstructive hand surgery after cervical spinal injury: a new method increases the possibilities to reconstruct important functions].

The sarcomere is the basic unit of force generation in skeletal muscle. Since muscle force generation is highly dependent on sarcomere length, it is important to know the normal range of sarcomere length if informed decisions are to be made regarding surgical procedures affecting muscle length, such as tendon transfer or lengthening. The article reports intraoperative sarcomere length measurement with a laser diffraction technique. Sarcomere length as a function of joint angle was monitored during various tendon transfers performed to restore lost finger, thumb, wrist, forearm and elbow functi…

Endoscopic carpal tunnel release with opponensplasty using ring finger flexor digitorum superficialis under WALANT with ultrasound assistance.

In patients with severe, long-standing carpal tunnel syndrome and thenar muscle atrophy, nerve decompression alone is unlikely to restore thumb opposition. A multitude of tendon transfer techniques have been described to restore thumb opposition. We describe the technique of an endoscopic carpal tunnel release with opponensplasty using ring finger flexor digitorum superficialis (FDS) tendon under Wide Awake Local Anaesthesia No Tourniquet (WALANT) and ultrasound assistance.

Avulsion fracture of the hamulus from clay gunshot sport: a case report.

We report a case of hamulus (hook of hamate) fracture nonunion and secondary flexor digitorum profundus tendon rupture caused by repetitive wrist hyperextension sustained during clay shotgun shooting. The hyperextension caused avulsion of the hamulus by the pisohamate ligament. The hamulus was excised and a flexor digitorum profundus tendon transfer of the ring to small finger side to side was performed with satisfactory outcome.

Chronic Achilles tendon rupture reconstructed using hamstring tendon autograft.

BACKGROUND: Chronic rupture of the Achilles tendon (delayed diagnosis of more than 4 weeks) can result in retraction of the tendon and inadequate healing. Direct repair may not be possible and augmentation methods are challenging when the defect exceeds 5-6 cm, especially if the distal stump is grossly tendinopathic. METHODS: We describe our method of Achilles tendon reconstruction with ipsilateral semitendinosis autograft and interference screw fixation in a patient with chronic rupture, a 9 cm defect and gross distal tendinopathy. RESULTS: Patient reported outcome measures consistently dem

[Acquired foot drop].

A dropping foot is the consequence of a variety of debilitating conditions and is oftentimes treated conservatively by general practitioners … ing foot have been treated adequately without an effect on the foot itself, then a posterior tibial tendon transfer may be considered. Generally, a posterior tibial tendon transfer has good outcomes for the treatment of dropping foot although it is partly dependent on the physiotherapy that accompanies it.

[Treatment for Patients with Charcot-Marie-Tooth Disease: Orthopaedic Aspects].

The orthopedic manifestations in patients with Charcot-Marie-Tooth disease include deformity and dysfunction of the extremities and spine. Conservative treatment is the first choice. Orthosis and rehabilitation can improve function, and are important for the prevention of joint contractures. Foot problems are most commonly observed and require surgical treatment. Foot deformities include pes cavus, cavovarus, claw toes, or drop foot. Single or combined surgeries selected for soft tissues are plantar release, tendon transfer, or Achilles tendon lengthening, and those for bones are osteotomies a…

[Early microsurgical revision of the brachial plexus in traumatic birth injuries. Patient selection and outcome].

A review of the literature reveals that with conventional treatment alone or in combination with secondary muscle/tendon transfer, about 4-43% of cases show incomplete recovery with severe functional and/or aesthetic impairment (group III). If these patients undergo early microsurgical brachial plexus revision, regeneration without significant functional and/or aesthetic impairment (shift from group III to group II) can be achieved in 80-90% of cases. Moreover, microsurgical reconstruction of the brachial plexus increases the possibilities of secondary muscle/tendon transfers. Therefore, provi…

Extensor pollicis longus rupture after distal radius fracture: results of reconstruction by transposition of the extensor indicis tendon and postoperative dynamic splinting.

Injury of the extensor pollicis longus tendon is one of the commonest extensor tendon injuries after distal radius fracture. In 2003-2005 we performed extensor indicis transfer in 31 patients with loss of function of the EPL tendon and postoperative dynamic splinting. In 25 patients, a distal radius fracture managed surgically led to the EPL rupture (wire internal fixation in 23 patients, plate internal fixation in 2 patients). In 6 patients, the cause was a distal radius fracture treated conservatively. Out of our 25 followed-up patients, 9 (36%) had a very good and 15 (60%) a good result in

Restoration of active fingers flexion with tensor fascia lata transfer in total brachial plexus palsy.

In total brachial plexus palsy, fingers flexion restoration is a real challenge. Nerve surgery can generally restore shoulder abduction and elbow flexion. However, results of nerve grafts or nerve transfers are generally poor for hand function. As a matter of fact, the long distance between nerve sutures and terminal nerve branches in muscles decreases the rate of reinnervation. When finger flexion occurs, strength is generally weak and function remains fair. Therefore, we proposed a new technique to restore fingers flexion. The tensor fascia lata tendon is harvested and sutured between the bi

Single incision thenar muscle reconstruction using the free functional pronator quadratus flap.

BACKGROUND: Injuries to the thenar muscle mass or the thenar branch of the median nerve and resulting loss of thumb opposition lead to a massive impairment of hand function. For decades, reconstructive approaches were based on tendon transfers. To broaden the reconstructive repertoire, we present the free functional pronator quadratus flap as a viable alternative for functional reconstruction and provide a specification for its indication. We demonstrate our surgical technique to a single incision reconstruction using the free functional pronator quadratus flap. Based on a series of three pati…

Outcome from a brachialis donor for wrist extension in tetraplegia-time to reconsider the International Classification for Surgery of the Hand in Tetraplegia (ICSHT).

INTRODUCTION: Surgical reconstruction after quadriplegia represents a powerful solution to restore lost function by injury. A case is presented in which surgical reconstruction of a patient with a C4 level spinal cord injury is performed using the brachialis (BRA) muscle as the donor. CASE PRESENTATION: The patient previously had no hand function. This transfer, in combination with fusion of the thumb CMC joint and transfer of the flexor pollicis longus (FPL) tendon to the radius, gives the patient full thumb key pinch powered by BRA transferred to the wrist extensors. Theoretical analysis of

Novel Surgical Procedure for Half Palmaris Longus Transfer during Opponensplasty of the Thumb for Patients with Carpal Tunnel Syndrome: A Technical Note.

Thumb opposition is an essential movement for daily use of the hand, including precise pinching/grasping and fine and complicated hand movement. Although studies have reported use of several donor tendons for opponensplasty, opponensplasty using the palmaris longus (i.e., Camitz opponensplasty) has been used in patients with loss of opposition function due to longstanding carpal tunnel syndrome. The procedure involves a simple, useful tendon transfer and does not cause functional deficits. To obtain enough length to transfer the tendon to the metacarpophalangeal joint of the thumb, the PL tend…

Congenital bilateral absence of the elbow flexors.

In this case report, a patient with bilateral congenital absence of the elbow flexors is described, which has not been described previously. The usual methods of tendon transfers after brachial plexus injury were not possible in this case because there were no biceps muscle tendons to which to attach. The method of moving the origin of the common extensor tendon proximally to have it function as an elbow flexor was successful and the patient obtained functional elbow flexion strength.

[Secondary surgical procedures following motor nerve injuries].

BACKGROUND: The functional deficits that develop after a peripheral nerve injury mean a considerable reduction in the quality of life for the affected patients. However, interventions on the injured nerve are not always possible or effective. In this case, secondary procedures, e.g. tendon transfers, are a feasible option for functional reconstruction. OBJECTIVES: An overview of the most common secondary surgical procedures for functional reconstruction after peripheral nerve injuries. METHODS: Presentation and discussion of the most common secondary surgical procedures with emphasis on tend…

Motor and sensory nerve transfers in the forearm and hand.

BACKGROUND: Peripheral nerve injury is a significant problem affecting more than 1 million people around the world each year and poses major challenges to the plastic and reconstructive surgeon. For high upper extremity nerve injuries, distal muscle reinnervation and functional outcomes are generally poor. Tendon transfer has been the traditional reconstructive option in these cases to restore hand function. More recently, nerve transfers have been described in the forearm and hand to recover hand and wrist function and critical sensation. METHODS: This article reviews the surgical principles…

First Extensor Compartment Tendon Rupture in the Setting of a Radial Styloid and Both-Bone Forearm Fracture: A Case Report.

CASE: We report on the rare occurrence of first extensor compartment tendon injury at the myotendinous junction after fractures of the radial styloid and both-bone forearm in a healthy 59-year-old man. Here, a novel technique for repair of this unique injury and an additional proposed mechanism of tendon rupture are discussed. CONCLUSION: We advocate for the suspicion of tendon rupture in the first dorsal compartment with fractures of the radial styloid and both-bone forearm. Tubularization and transfer of the abductor pollicis longus and extensor pollicis brevis tendons to the brachioradiali

Secondary procedures for restoration of upper limb function in late cases of neonatal brachial plexus palsy.

Neonatal brachial plexus palsy is a devastating complication after a difficult delivery. The incidence of this injury has not significantly … ore distally a supination or pronation deformity is usually apparent, and available options include tendon transfers or radial osteotomy. The wrist of these patients may be ulnarly deviated or may has absent extension, so tendon transfers or free muscle transfers can also be used for correction of these deformities. In severe cases, wrist fusion is an alternative …

Transfer of the Fifth Finger to Replace an Amputated Thumb: A Preliminary Study.

Amputation of the thumb causes not only loss of significant functional use of the hand but also psychological and social problems. The procedures of toe-to-thumb transfer, pollicization, or metacarpal extension are recognized and well-documented options in thumb reconstruction. Although these techniques have been successfully applied, they have some disadvantages. This study aims to test the feasibility of the fifth finger for thumb reconstruction. The fifth finger was released following exploration of the digital artery, vein, nerve, and flexor and extensor tendons, including the flexor retin
