Carotid arterial stenosis: evaluation at CT angiography with the volume-rendering technique.

PURPOSE: To determine whether computed tomographic (CT) angiography with the volume-rendering technique (VRT) can be used to accurately quantify carotid arterial stenosis and to identify occlusions. MATERIALS AND METHODS: Spiral CT was performed in 23 patients who were referred for carotid stenosis evaluation. VRT images and shaded-surface display (SSD) images of 46 carotid arterial bifurcations were compared with findings from digital subtraction angiography (DSA). RESULTS: Agreement on stenosis category between VRT CT angiography and DSA was found in 39 (85%) of the 46 carotid arteries stu

Cerebral Vasospasm Detection and Delayed Cerebral Ischemia Prediction after Aneurysmal Subarachnoid Hemorrhage: A Scoping Review.

BACKGROUND/OBJECTIVE: Aneurysmal subarachnoid hemorrhage (aSAH) is complicated by angiographic cerebral vasospasm and delayed cerebral ischemia (DCI), distinct entities with different reference standards and surveillance targets. We mapped the evidence on (1) diagnostic accuracy of vasospasm monitoring tools using digital subtraction angiography (DSA) as the reference standard and (2) detection and prediction of DCI using the 2010 consensus definition. METHODS: We performed a Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR)-guided s

Assuming the right approach in cerebrofacial arteriovenous metameric syndrome: a case report.

Cerebrofacial arteriovenous metameric syndrome (CAMS) is a rare nonhereditary disease, amounting to only 0.5% of all cerebral vascular malformations. It is characterized by vascular malformations in a metameric distribution involving the cerebrofacial region. We present a case of a 14-year-old girl in whom the first vascular lesion appeared on the tip of her nose at the age of 18 months. This lesion reoccurred on several occasions during her childhood, with exacerbations coinciding with common rhinitis and several nose bleeds. She was unsuccessfully treated with laser therapy and topical selec

Basilar artery blister aneurysm presenting with subarachnoid hemorrhage: management challenges in the context of basilar artery duplication.

Blood blister-like aneurysms (BBAs) are rare, fragile vascular lesions most commonly arising from non-branching segments of the internal carotid artery. Basilar artery BBAs are exceptionally uncommon and present significant diagnostic and management challenges. We report the case of a woman in her late 50s who presented with sudden-onset occipital headache, neck stiffness, and subarachnoid haemorrhage. Digital subtraction angiography identified a 3 mm blister-like aneurysm in the basilar artery, considered the presumed culprit lesion, along with an incidental basilar artery fenestration. Endov

Accessory posterior cerebral artery as a duplicate anterior choroidal artery.

PURPOSE: Anatomical variations in the anterior choroidal artery (AChA) and/or the posterior cerebral artery (PCA) are rare. Hyperplastic AChA is an anatomical variant supplying both the AChA area and the PCA area. In accessory PCA, a hyperplastic AChA supplies part of the PCA territory. To our knowledge, there are no previous reports of accessory PCA associated with duplicated AChAs. METHODS: We report a case of accessory PCA as a duplicated AChA associated with contralateral accessory middle cerebral artery, diagnosed by digital subtraction angiography. RESULTS: A 34-year-old man visited ou

[Dural arteriovenous fistula is a rare, but treatable cause of transverse myelitis].

This is a case report of a 60-year-old male admitted on suspicion of relapse of idiopathic transverse myelitis (TM), who after further diagnostic workup underwent successful closure of a dural arteriovenous fistula (DAVF). Magnetic resonance imaging in DAVF usually shows longitudinal TM, which, unlike DAVF, is also seen with the more common inflammatory or infectious causes usually showing inflammation in the cerebrospinal fluid. The natural history of DAVF is progressive. Since curable options exist, timely diagnosis is most important.

Geometric characterization of stenosed human carotid arteries.

RATIONALE AND OBJECTIVES: The geometry of stenosed carotid bifurcations was analyzed to determine average representations for several stenosis grades. METHODS: Film angiograms of 62 patients with internal carotid artery stenoses were digitized. Residual lumen boundaries were manually outlined. The outlines were processed with a computer to extract geometric measurements. The measurements were grouped according to stenosis grade and used to create average representations. RESULTS: Accuracy and precision of the outlining technique were +/- 0.020 common carotid diameters (CCD) and +/- 0.025 CCD

[Demonstration of a vascular lesion by intraoperative portable DSA in a case of intracerebral hematoma with impending herniation].

A nine year old boy with sudden onset of loss of consciousness was brought to the emergency room where he was in deep coma with bilateral decerebrated rigidity, and fixed dilated pupils. Computerized tomographic scan showed an intracerebral hematoma in the left temporal lobe associated with signs of uncal herniation. He was taken to the operating room immediately and craniotomy was started. After aspirating a small amount of hematoma through a burr hole, left carotid angiography was carried out by using a portable digital subtraction angiography (DSA) unit. The procedure took approximately ten

[Intra-arterial DSA of the hand arteries in the diagnosis of inflammatory connective tissue diseases].

Seventy-five patients with suspected rheumatic diseases underwent intraarterial DSA of the hand arteries. In 81% of the patients we were able to establish the diagnosis of vasculitis according to angiographic criteria. Neither clinical nor chemical or immunological features allowed the diagnosis of an inflammatory disease of joint and perivascular tissue at this time. However, histological examination and immunofluorescence microscopy confirmed the angiographic findings and showed signs of an immune complex vasculitis. A strong predictor for an abnormal angiogram of the hand arteries is the pr

Extracranial internal carotid artery occlusive dissection - multimodality presentation in a case series.

Background: Carotid dissection is a rare disease, mainly affecting young and middle-aged patients potentially ending up in stroke. Multimodality imaging plays an essential role, both in terms of prompt and accurate diagnosis and follow-up of this entity. Patients and methods: We herein present a case series of patients with internal carotid artery dissection and compare the various imaging findings of ultrasonography, multidetector computed tomography angiography and magnetic resonance angiography, with a purpose to illustrate the value of multimodality imaging in the diagnosis of carotid diss

Anti-thrombogenic coatings for devices in neurointerventional surgery: Case report and review of the literature.

BACKGROUND: Stent-assisted coiling and extra-saccular flow diversion require dual anti-platelet therapy due to the thrombogenic properties of the implants. While both methods are widely accepted, thromboembolic complications and the detrimental effects of dual anti-platelet therapy remain a concern. Anti-thrombogenic surface coatings aim to solve both of these issues. Current developments are discussed within the framework of an actual clinical case. CASE DESCRIPTION: A 33-year-old male patient lost consciousness while doing sport and was administered 500 mg acetylsalicylic acid on site. Comp

Radial Artery Grafts as Rescue Strategy for Patients with Moyamoya Disease for Whom Conventional Revascularization Failed.

BACKGROUND: Failure of direct revascularization with superficial temporal artery (STA)-middle cerebral artery (MCA) bypass for Moyamoya disease (MMD) is comparatively rare. However, for those cases where a bypass fails to prevent further ischemic attacks, safe and efficient rescue strategies are needed. We present our experience with radial artery grafts for secondary revascularization of MMD. METHODS: Between April 2007 and April 2014, we have performed STA-MCA bypass in 182 patients diagnosed with Moyamoya vasculopathy. Four patients with typical MMD who had an unsuccessful STA-MCA bypass r

The A1-A2 diameter ratio may influence formation and rupture potential of anterior communicating artery aneurysms.

BACKGROUND: Specific morphological factors contribute to the hemodynamics of the anterior communicating artery (AComA). No study has examined the role of the A2 segment on AComA aneurysm presence and rupture. OBJECTIVE: To examine the possibility that the ratio between A1 and A2 segments (A1-2 ratio) represents an independent risk factor for presence and rupture of AComA aneurysms (AComAAs). METHODS: A retrospective review of an institutional aneurysm database was performed; patients with ruptured and unruptured AComAAs were identified. Two control groups were selected: group A (posterior ci
