A detailed clinical examination is an essential component in the assessment of the cavus foot. A complex interaction of pathologic conditions can only be assessed completely with physical examination. Imaging such as computed tomography or magnetic resonance imaging (MRI) may confound the physician, such as in anterior talofibular ligament tears on MRI while the ankle is stable or arthritic joints that are asymptomatic but abnormal on imaging. At the end of the day, the physical examination supersedes all other investigations. After investigations have been performed, the patient needs to be r…

Synovial chondromatosis of the ankle is a rare condition, particularly secondary chondromatosis. In view of a possible traumatic pathogenesis, chondromatosis should be kept in mind in daily trauma and orthopedics practice. Diagnostic imaging gives a first indication. The key to differentiating between the primary and secondary forms is histological identification. This case shows the necessity of exact differentiation, even in cases of a causal link with a specific injury.

Neuropathies can be a cause of chronic foot and ankle pain. The diagnosis can be elusive given the sometimes nonspecific clinical presentation. Although electrodiagnostic studies are primarily relied on for the diagnosis of nerve impairment, imaging is sometimes helpful in helping define the exact site of the entrapment and whether any masses are present. It is critical for the imager to understand the complex anatomy of these nerves and their adjacent structures, to know the most common locations for their entrapments or injury, and to select the proper imaging modality to improve detection o…

BACKGROUND: The supination line is a fictive line along the foot and ankle, on which over twenty fracture types and approximately ten different ligamentous sprain-injuries have been identified.
OBJECTIVE: The current study was conducted to evaluate the incidence of different types of supination line injuries visible at the initial radiographs at the Emergency Department and to determine the type and percentage of misdiagnosed injuries.
METHOD: Retrospective study of consecutive patients who visited the Emergency Department, between January 1 and June 30, 2009, after sustaining an injury of t…

Most osteochondral lesions (defects) of the talar dome are caused by trauma, which may be a single event or repeated, less intense events (microtrauma). A lesion may heal, remain asymptomatic, or progress to deep ankle pain on weight bearing, prolonged joint swelling, and the formation of subchondral bone cysts. During loading, compression of the cartilage forces water into the microfractured subchondral bone. The increased flow and pressure of fluid in the subchondral bone can cause osteolysis and the slow development of a subchondral cyst. The pain does not arise from the cartilage lesion bu…

PURPOSE OF REVIEW: The incidence of ankle osteoarthritis has increased in recent years, in part, secondary to vehicular trauma. This review describes conservative and operative intervention strategies along with current research related to the management of ankle osteoarthritis.
RECENT FINDINGS: Self-reported physical function in patients with ankle osteoarthritis is equivalent to or worse than that of patients with endstage kidney disease, congestive heart failure, or cervical-spine pain and radiculopathy. Nonoperative-intervention strategies such as assistive devices, orthoses, and viscosup…

BACKGROUND: Researchers have found it difficult to recreate a Lauge-Hansen supination-external rotation-type ankle fracture in experimental settings. We hypothesized that a pronation-external rotation mechanism could cause both distal, short oblique and high fibular fractures and that the fracture type would be affected by associated, laterally directed forces applied to the foot. Methlods: Twenty-three cadaver ankles were subjected to fracture loading that replicated the Lauge-Hansen pronation-external rotation mechanism with or without applying an external lateral force. In Phase I, an axial…

Ankle fractures in patients with diabetes mellitus have long been recognized as a challenge to practicing clinicians. Complications of impaired wound healing, infection, malunion, delayed union, nonunion, and Charcot arthropathy are prevalent in this patient population. Controversy exists as to whether diabetic ankle fractures are best treated noninvasively or by open reduction and internal fixation. Patients with diabetes are at significant risk for soft-tissue complications. In addition, diabetic ankle fractures heal, but significant delays in bone healing exist. Also, Charcot ankle arthropa…

Patients with diabetes mellitus have higher complication rates following both open and closed management of ankle fractures. Diabetic patients with neuropathy or vasculopathy have higher complication rates than both diabetic patients without these comorbidities and nondiabetic patients. Unstable ankle fractures in diabetic patients without neuropathy or vasculopathy are best treated with open reduction and internal fixation with use of standard techniques. Patients with neuropathy or vasculopathy are at increased risk for both soft-tissue and osseous complications, including delayed union and …

The main views and indications in adult foot and ankle radiography are detailed. Among foot and ankle diseases, mechanical ones are most frequent, including usual podologic changes (commonly followed by peculiar involvement of certain bones, ligaments, joints, tendons and other soft tissues), synostoses, occult and stress fractures, sprains, and tendon changes. Although they are less frequently encountered in common clinical practice, the radiographic appearance of inflammatory, infectious, and tumoral diseases of foot and ankle are disclosed as well. The correlation existing between some subt…

Traumatic injuries of the foot and ankle are frequent and constitute a true public health problem with over 6000 daily cases in France. The complex anatomy of this area and its association to other injuries in polytraumatized patients can lead to delayed diagnosis with worsened functional prognosis. Accurate diagnosis relies on good knowledge of osseous and ligamentous lesions and their associations and sometimes requires the use of additional imaging techniques including US and CT.

Syndesmosis sprains have received increasing recognition during recent years because of a heightened awareness of the mechanism, symptoms, and signs of injury. Syndesmosis injuries take longer to recover than lateral ankle sprains, and no consensus exists regarding optimal treatment of these injuries. Therefore, we undertook a systematic review of the literature to evaluate the effect of treatment on outcome following syndesmosis injury. We identified six articles that evaluated treatment of syndesmosis injuries. All studies were case series including prospectively collected data of young, act…

CONTEXT: Lateral ankle sprains occur more frequently than any other orthopedic injury. Athletes often report sustaining more injuries late in competition when fatigue is present.
OBJECTIVE: To evaluate neuromuscular function of the ankle musculature after fatigue.
DESIGN: Experimental, pretest-posttest.
SETTING: Research laboratory.
PARTICIPANTS: Ten female and 9 male college-aged subjects.
INTERVENTION: Fatigue was induced via continuous concentric and eccentric muscle actions of the ankle: inversion (INV), eversion (EV), plantar flexion (PF), and dorsiflexion (DF).
MAIN OUTCOME MEASURE…
