FedSynthCT-Brain: A federated learning framework for multi-institutional brain MRI-to-CT synthesis.

The generation of Synthetic Computed Tomography (sCT) images has become a pivotal methodology in modern clinical practice, particularly in t … datasets may pose privacy concerns. To address the aforementioned issues, we introduced FedSynthCT-Brain, an approach based on the Federated Learning (FL) paradigm for MRI-to-sCT in brain imaging. This is among the first applications of FL for MRI-to-sCT, employing a cross-silo horizontal FL approach that allows multiple centres to collaboratively trai …

Brain-targeted M2 macrophage membrane-hybrid biomimetic liposomes for treatment of traumatic brain injury.

Traumatic brain injury (TBI) is highly incidental but effective solutions are absent. Moreover, the secondary injury following TBI is arising due to the Ca2+ influx of injured neural cells. Here, a Ca2+ influx inhibitor, nimodipine, was loaded in M2 macrophage membrane-hybrid biomimetic liposomes (NM2Ls). NM2Ls significantly inhibited the influx of Ca2+ into inflammatory neural cells and reduced the expression of inflammatory factors. More importantly, intravenously injected NM2Ls avoided the clearance of the immune system and targeted the brain via CCR2 following TBI; the inflammation in the …

Applying brain injury guidelines to patients with traumatic brain injury: A contemporary, consecutive series.

OBJECTIVES: Traumatic brain injury (TBI) is a significant contributor to long-term disability and imposes substantial costs, which may include excess, duplicative, or unnecessary imaging, hospitalizations or transfers, and superfluous consultations. We examined the use of the Brain Injury Guidelines (BIG) in 144 consecutive patients presenting with TBI. METHODS: Patients with TBI evaluated consecutively between April 2024 and September 2024 were prospectively classified into BIG 1, 2, and 3 categories and retrospectively assessed. The primary outcome measure was neurosurgical intervention. Se…

Perturbing whole-brain models of brain hierarchy: An application for depression following pharmacological treatment.

Determining the scale of neural representations is a central challenge in neuroscience. While localized representations have traditionally dominated, evidence suggests information is also encoded in distributed, hierarchical networks. Recent research indicates that the hierarchy of causal influences shaping functional patterns serves as a signature of distinct brain states, with implications for neuropsychiatric disorders. Here, we first explore how whole-brain models, guided by the thermodynamics of mind framework, estimate brain hierarchy and how perturbing such models enables the study of i…

Single Extracellular Vesicle Profiling to Define Brain Specific Traumatic Brain Injury Induced Neuro-Inflammation.

Traumatic Brain Injury (TBI) triggers secondary molecular processes that contribute to mortality and morbidity. Neuroinflammation is a key factor affecting patient outcomes both acutely and chronically. Traditional diagnostic tools, such as computed tomography imaging and the Glasgow Coma Scale, are limited in detecting molecular changes, particularly related to neuroinflammation. Small extracellular vesicles (sEVs) are cell-specific vesicles that enable cell-to-cell communication and are involved in TBI pathology. In this study, brain-specific sEVs are isolated by targeting brain-associated m…

Mapping the lung-brain axis: Causal relationships between brain network connectivity and respiratory disorders.

BACKGROUND: The mechanistic relationship between respiratory disorders and brain function remains poorly understood, despite growing evidence of cognitive and neurological manifestations in respiratory diseases. We aim to identify whether specific brain network connectivity patterns causally influence respiratory disease susceptibility, while respiratory conditions might reciprocally affect brain network architecture. METHODS: We performed bidirectional Mendelian randomization (MR) analyses using genome-wide association studies (GWAS) of brain network connectivity from UK Biobank resting-stat…

Deep brain stimulation and psychosis: A case series and two candidate causal brain circuits.

Schizophrenia and psychosis are debilitating conditions with suboptimal treatment options. Deep brain stimulation (DBS) offers promise, but effective treatment targets remain undefined. Examining cases in which DBS either induced or alleviated psychotic symptoms may help identify circuits causally involved in psychosis and suggest candidate targets for intervention. We systematically reviewed the literature to identify all published cases in which DBS modulated (i.e., caused or improved) psychotic symptoms, regardless of target and indication. Authors of original publications were contacted to…

Do human brain white matter and brain stem structures show direction-dependent mechanical behavior?

Since the corpus callosum and the brain stem are both vulnerable to diffuse axonal injury during head impacts, there is a high interest in modeling the mechanical behavior of these brain structures. In recent years, different versions of fiber-reinforced material models have been proposed for the corpus callosum and other white matter regions, as well as for the brain stem, even though there is currently no consensus on whether those structures exhibit a significant direction-dependent behavior during mechanical loading. Here, we present the first large-strain, multimodal experimental study on…

Preconceptional brain structure and future conception: a prospective brain MRI study among 321 women.

Brain structure may influence female fertility through its reciprocal relationship with the endocrine system, but this hypothesis is underexplored. This study investigated the association between preconceptional brain structure and the likelihood of conception in a prospective population-based neuroimaging cohort. Women intending to conceive within a year were recruited and structural brain MRI scans were collected from 321 participants between June 2019 and March 2021. During the 12-month follow-up, 185 women became pregnant, and 136 did not. Logistic regression was used to examine associatio…

MRI quantitative imaging biomarkers in differentiating brain parenchymal tuberculoma and lung cancer brain metastases.

BACKGROUND: Brain parenchymal tuberculoma (BT) and brain metastases (BM) originating from lung cancer often exhibit overlapping clinical and imaging features, making accurate differentiation challenging. Current diagnostic approaches remain suboptimal. PURPOSE: This study aimed to develop a radiomics-based MRI model to differentiate BT from BM and enhance model interpretability using Shapley Additive Explanations (SHAP) analysis. MATERIAL AND METHODS: This retrospective study involved 175 patients (97 with BT and 78 with BM) treated at Hangzhou Red Cross Hospital from January 2018 to March 2…

Associations between Brain Alpha-Tocopherol Stereoisomer Profile and Hallmarks of Brain Aging in Centenarians.

Brain alpha-tocopherol (αT) concentration was previously reported to be inversely associated with neurofibrillary tangle (NFT) counts in specific brain structures from centenarians. However, the contribution of natural or synthetic αT stereoisomers to this relationship is unknown. In this study, αT stereoisomers were quantified in the temporal cortex (TC) of 47 centenarians in the Georgia Centenarian Study (age: 102.2 ± 2.5 years, BMI: 22.1 ± 3.9 kg/m2) and then correlated with amyloid plaques (diffuse and neuritic plaques; DPs, NPs) and NFTs in seven brain regions. The natural stereoisomer, R…

Bilingual Language Control in the Brain: Evidence from Structural and Effective Functional Brain Connectivity.

Experience in bilingual language control is often accompanied by changes in the structure and function of the brain. Brain structural changes are also often closely related to changes in functions. Previous studies, however, have not directly explored the relationship between structural connectivity and effective functional connectivity of the brain during bilingual language control, and whether the two types of connectivity are associated with behavioral performance of language control. Using behavioral performance, functional, and diffusion imaging techniques, we found that: (1) during langu…

SCG2 mediates blood-brain barrier dysfunction and schizophrenia-like behaviors after traumatic brain injury.

Traumatic brain injury (TBI), which is characterized by acute neurological dysfunction, is also one of the most widely recognized environmental risk factors for various neurological and psychiatric disorders. However, the role of TBI in neurological perturbation and the mechanisms underlying these disorders remain unknown. We evaluated transcriptional changes in cells of the frontal cortex after TBI by exploiting single-cell RNA sequencing (scRNA-Seq). We adopted the gene expression omnibus and scRNA-Seq to identify the mediation by secretogranin II (SCG2) of TBI-induced schizophrenia. Astrocy…

Temperature induces brain-intake shift of recombinant high-density lipoprotein after traumatic brain injury.

Traumatic brain injury (TBI) is one of the leading public health concerns in the world. Therapeutic hypothermia is routinely used in severe TBI, and pathophysiological hyperthermia, frequently observed in TBI patients, has an unclear impact on drug transport in the injured brain due to a lack of study on its effects. We investigated the effect of post-traumatic therapeutic hypothermia at 33°C and pathophysiological hyperthermia at 39°C on brain transport and cell uptake of neuroprotectants after TBI. Recombinant high-density lipoprotein (rHDL), which possesses anti-inflammatory, antioxidant ac…

Microstructural brain changes following prospective memory rehabilitation in traumatic brain injury: An observational study.

Prospective memory (PM) impairment is a common consequence of moderate-severe traumatic brain injury (TBI). Compensatory strategy training and rehabilitation (COMP) is the usual treatment of PM deficits through environmental modification and the use of assistive methods such as diaries and routines. The study intends to examine the changes in white matter integrity, as measured by advanced diffusion magnetic resonance imaging (dMRI) following COMP intervention in moderate-severe TBI patients. Nine COMP intervention and twelve routine care comparison cohort moderate-severe TBI patients were rec…

Radiation-Induced Brain Injury: Mechanistic Insights and the Promise of Gut-Brain Axis Therapies.

Radiation therapy is widely recognized as an efficacious modality for treating neoplasms located within the craniofacial region. Nevertheless, this approach is not devoid of risks, predominantly concerning potential harm to the neural structures. Adverse effects may encompass focal cerebral necrosis, cognitive function compromise, cerebrovascular pathology, spinal cord injury, and detriment to the neural fibers constituting the brachial plexus. With increasing survival rates among oncology patients, evaluating post-treatment quality of life has become crucial in assessing the benefits of radia

Targeting brain-peripheral immune responses for secondary brain injury after ischemic and hemorrhagic stroke.

The notion that the central nervous system is an immunologically immune-exempt organ has changed over the past two decades, with increasing evidence of strong links and interactions between the central nervous system and the peripheral immune system, both in the healthy state and after ischemic and hemorrhagic stroke. Although primary injury after stroke is certainly important, the limited therapeutic efficacy, poor neurological prognosis and high mortality have led researchers to realize that secondary injury and damage may also play important roles in influencing long-term neurological progn
