A 20.1 kg adrenal cavernous haemangioma presenting as a giant retroperitoneal mass: diagnostic difficulties and operative strategy - a case report.

BACKGROUND: Adrenal haemangiomas are exceptionally rare benign vascular tumours, usually discovered incidentally and measuring only a few centimetres. But in rare cases, lesions exceeding 20 kg are distinctly uncommon and pose major diagnostic and operative challenges. CASE PRESENTATION: A 67-year-old woman presented with six months of progressive abdominal distension, early satiety, and exertional dyspnea on a four years background of intermittent left loin pain. Her medical history includes HTN, T2DM, AF and heart failure. No surgical history was reported. Imaging revealed a giant left retro

[Mesogastrium theory of D2 lymphadenectomy for advanced gastric cancer: cogitation and skills].

D2 lymphadenectomy combined with complete mesentery excision (CME) for advanced gastric cancer in recent years was a hotspot issue in China, while its safety and effectiveness have been proved. According to the Membrane anatomy of the stomach, both surgical approach and mesogastrium interval is particularly important in Laparoscopic radical gastrectomy. We summarized and shared the following clinical experience for medical colleagues. (1) Lymph nodes of right abdominal aorta-No.7,8,9,12-should be resection as an indivisible whole. This integrity tissue above the portal vein was supposed to the

Dynamic and Static Vessel Malperfusion as a Consequence of Acute Type B Aortic Dissection.

BACKGROUND: Acute type B aortic dissection (TBAD) is relatively uncommon with an estimated incidence of up to 8 cases per 100,000 individuals annually. It can be classified based on presenting clinical and radiological features into complicated and uncomplicated. Following the initial tear, the direction and extent of the dissection flap is unpredictable, possibly propagating proximally or distally. One consequence of a dissection flap is the occurrence of organ malperfusion by occlusion of the origin of the branch vessel. The 2 types of malperfusion in TBAD are static and dynamic, with the la

The role of thoracic endovascular repair in elective, symptomatic and ruptured thoracic aortic diseases.

OBJECTIVES: Thoracic endovascular aortic repair (TEVAR) has emerged as a safe procedure in the treatment of a wide spectrum of descending thoracic aortic pathologies, with satisfactory results both in elective and urgent settings. We investigated the results of our elective, urgent and emergency TEVAR interventions. METHODS: A single-centre retrospective analysis of all consecutive patients undergoing TEVAR from 2010 to 2016 was performed. Primary end point of the study was early mortality, whereas the secondary end points included major complications according to the urgency of the procedure

Anterior Retroperitoneal Spine Exposure following Prior Endovascular Aortic Aneurysm Repair.

BACKGROUND: We describe successful anterior retroperitoneal spine exposure to facilitate anterior lumbar interbody fusion (ALIF) in a patient with a prior endovascular aneurysm repair (EVAR). METHODS: A 74-year-old male with an extensive spine surgical history presented with progressive neurogenic claudication and paresthesia involving both feet. In addition, his surgical history was notable for an EVAR performed elsewhere 5 years earlier, with subsequent right renal stent placement for encroachment of the right renal artery. Diagnostic evaluation identified severe L3-4 and L4-5 canal stenosi

The Footprint of Kynurenine Pathway in Cardiovascular Diseases.

Kynurenine pathway is the main route of tryptophan metabolism and produces several metabolites with various biologic properties. It has been uncovered that several cardiovascular diseases are associated with the overactivation of kynurenine pathway and kynurenine and its metabolites have diagnostic and prognostic value in cardiovascular diseases. Furthermore, it was found that several kynurenine metabolites can differently affect cardiovascular health. For instance, preclinical studies have shown that kynurenine, xanthurenic acid and cis-WOOH decrease blood pressure; kynurenine and 3-hydroxyan

Protease inhibitor 15, a candidate gene for abdominal aortic internal elastic lamina ruptures in the rat.

The inbred Brown Norway (BN) rat develops spontaneous ruptures of the internal elastic lamina (RIEL) of the abdominal aorta (AA) and iliac arteries. Prior studies with crosses of the BN/Orl RJ (susceptible) and LOU/M (resistant) showed the presence of a significant QTL on chromosome 5 and the production of congenic rats proved the involvement of this locus. In this study, we further dissected the above-mentioned QTL by creating a new panel of LOU.BN(chr5) congenic and subcongenic lines and reduced the locus to 5.2 Mb. Then we studied 1,002 heterogeneous stock (HS) rats, whose phenotyping revea

Acute chylous peritonitis: Report of a case.

INTRODUCTION: Acute chylous peritonitis, a rare condition, is defined as an abrupt chylous fluid accumulation in the peritoneal space in the absence of a significant underlying pathology. Diagnosis is difficult due to abrupt onset and accompanying acute abdomen signs. PRESENTATION OF CASE: Here we presented the case of a 32-year-old female patient who underwent laparoscopy due to acute abdomen findings and was diagnosed with acute chylous peritonitis. No underlying pathologies were detected, and only irrigation and drainage were applied. She was discharged unproblematically with a low-fat die

Abdominal aortic aneurysm repair via percutaneous endovascular stenting in the swine model.

Abdominal aortic aneurysms (AAAs) are the 13th leading cause of death in the United States. Endovascular stent grafts are currently being investigated in clinical trials as an alternative method for treatment of AAA. Twelve pigs underwent creation of AAA using the abdominus rectus fascia. Postoperatively, all 12 animals underwent angiography, which demonstrated turbulent flow within the created aneurysms. Four of the animals were not stented and served as controls. Eight animals underwent subsequent percutaneous placement of a covered endovascular stent via a right femoral sheath, and the sten

Median Arcuate Ligament Syndrome - Literature Review and Case Report.

INTRODUCTION: Median arcuate ligament syndrome (MALS) or Dunbar syndrome is a rare clinical entity characterized by celiac trunk compression by median arcuate ligament and variable gastrointestinal symptoms (postprandial epigastric pain, nausea, weight loss, anorexia and diarrhea). However, some degree of radiographic compression is observed in 10%-24% of asymptomatic patients. Besides the extrinsic vascular compression, MALS has a multifactorial etiology and it has been suggested as a neurogenic disease resulting in altered sensation and pain from the somatic nerves in the splanchnic plexus.

Hypertriglyceridemia Does Not Worsen Thoracic Aortic Aneurysm and Dissection in GPIHBP1 Knockout Mice-Brief Report.

BACKGROUND: Hypertriglyceridemia is an established risk factor for atherosclerotic cardiovascular disease and has recently been implicated in abdominal aortic aneurysm pathogenesis. However, its role in thoracic aortic aneurysm and dissection (TAAD) remains undefined. METHODS: We investigated the impact of hypertriglyceridemia on experimental TAAD using GPIHBP1 (glycosylphosphatidylinositol-anchored high-density lipoprotein-binding protein 1) knockout mice as a hypertriglyceridemia model. TAAD was induced in 3- to 4-week-old mice via oral β-aminopropionitrile administration for 28 days. RESU

A Case of Left Retroexternal Iliac Artery Megaureter Associated with Additional Renal and Vascular Congenital Anomalies.

Introduction. A number of rare anatomical anomalies, including retroexternal iliac ureter, extrarenal calyces (ERCs), and vascular anomalies, were observed in a 96-year-old female cadaver during a routine dissection. Description. A markedly dilated left extrarenal pelvis (ERP) with a diameter of 3.15 cm was noticed. Three major calyces were found outside of the normal-sized left kidney. The abdominal aorta (AA), instead of normal bifurcation, branched to the right common, left external, and left internal iliac arteries. The median sacral artery was a direct branch from the right common iliac a

Top 10 candidate aortic disease trials.

Aortic pathologies such as aneurysm, dissection and trauma are relatively common and potentially fatal diseases. Over the past two decades, we have experienced unprecedented technical and medical developments in the field. Despite this, there is a great need, and great opportunities, to further explore the area. In this review, we have identified important areas that need to be further studied and selected priority aortic disease trials. There is a pressing need to update the AAA natural history and the role for endovascular AAA repair as well as to define biomarkers and genetic risk factors a

Noninvasive diagnosis of thrombus in the heart and large vessels--usefulness of two-dimensional echocardiography and X-ray CT.

The usefulness of two dimensional echocardiography (2-D echocardiography) and x-ray computed tomography (CT) for the diagnosis of thrombi in the cardiac cavity and large vessels was studied by comparing them with the findings of invasive methods. Among 56 subjects with mitral stenosis, left atrial thrombi were noted in 12 cases (16 regions) by CT and 8 cases (9 regions) by 2-D echocardiography. In 16 subjects who underwent operations, one false negative case by CT and 3 false negative and one false positive cases by 2-D echocardiography were found. In 80 subjects with myocardial infarction 2-D

Type B Aortic Dissection Masquerading As Acute Pyelonephritis: Think Beyond Measures.

Aortic dissection (AD) is a life-threatening medical emergency with a high mortality rate if misdiagnosed; therefore, an urgent and precise diagnosis is crucial for prompt treatment. This article presents a rare case report of AD with an atypical clinical presentation that led to delayed diagnosis and a complicated clinical course. Herein, we aim to contribute to the existing literature by providing insights into the varied presentations of AD and offering valuable lessons for clinicians faced with similar diagnostic scenarios. A 64-year-old female with an extended history of hypertension and

Aortic Tissue Proteome Alterations in Vascular Ehlers-Danlos Syndrome.

Vascular Ehlers-Danlos syndrome (vEDS) is a rare connective tissue disorder caused by mutations in the COL3A1 gene, leading to life-threatening vascular complications. This study presents a proteomic analysis of aortic wall tissue from a 60 year-old vEDS patient with a confirmed COL3A1 mutation, who died from an aortic dissection consequent to an abdominal aortic aneurysm. Compared to healthy controls, the patient's tissue showed an imbalance in collagen isoforms such as elevated types I and VI and reduced types VIII, XIV, and XVIII, with no significant change in type III collagen. Alterations

Bifurcated endovascular graft for spontaneous acute aortic dissections: report of a case using a simple technique to help safe deployment.

This paper presents the description of the author's experience with bifurcated endovascular stent graft in a patient affected by spontaneous infrarenal acute aortic dissection (SIAAD). The authors report a case of SIAAD occurring in the normal aorta of a patient who presented with severe lower back pain radiated to the abdomen, not responding to common pain-killers. A complete exclusion of the dissected aorta was accomplished with a bifurcated endovascular graft using a simple technique. SIAAD is a rare event. Endovascular therapy is a safe option and can be considered the treatment of choice
