Related factors of bloodstream infections associated with urinary tract infections and pathogenetic characteristics analysis.

OBJECTIVES: To explore correlations between biomarker indices and urosepsis severity, and investigate the prevalence of drug-resistant Escherichia coli in a patient population at the General Hospital of Ningxia Medical University in the Ningxia region of China. METHODS: Patients with urinary tract infection-associated sepsis were categorized into three groups: a septic non-shock group (NSSPU), a septic shock group (USG), and a control group with non-sepsis cases of simple urinary tract infections (CG). The study analyzed various biomarkers, including the percentage of neutrophils (N%), neutro…

Cancer immunotherapy with immune checkpoint inhibitors and infections: A particular focus on mycobacterial infections.

Cancer treatment is undergoing a major transformation with the advent of immunotherapy with immune checkpoint inhibitors. These drugs, which have a different mechanism of action from conventional cytotoxic chemotherapy, are transforming treatment paradigms for many patients suffering from advanced cancer. On the other hand, they are often complicated by specific adverse events, known as immune-related adverse events (irAEs). Infections occurring during immunotherapy with immune checkpoint inhibitors have recently received increasing attention and sometimes are seen as part of irAEs. Amongst th…

Donor-Derived Infections: Monitoring of Posttransplant Infections and Safety Lessons From the Opioid Epidemic.

OBJECTIVES: The inadequate supply of transplantable organs necessitates new approaches to donor screening while avoiding transmission of infections. Donor-derived infections are well described. Multiple changes have occurred in donor management and organ utilization, including increased recognition of and therapies for viral infections, the emergence of multidrug antimicrobial-resistant organisms, and identification of some uncommon viral infections transmitted with allografts to clusters of recipients. Donor evaluation has been impacted by substance use-associated deaths and routine use of se…

Frequency of serious bacterial infections in young infants with and without viral respiratory infections.

BACKGROUND: The goal of our investigation was to describe the incidence of serious bacterial infection (SBI, defined as bacteremia, urinary tract infection (UTI), or meningitis) in young infants with and without documented viral pathogens. METHODS: This was a retrospective cross-sectional study (1/2016-12/2017) in 3 emergency departments (EDs). Previously healthy 0-60-day-old infants were included if at least respiratory viral testing and a blood culture was obtained. The frequency of SBI, the primary outcome, was compared among infants with/without respiratory viral infections using the Pear…

Are solo infections of the diphyllobothriidean cestode Schistocephalus solidus more virulent than multiple infections?

We performed a long-term natural experiment investigating the impact of the diphyllobotriidean cestode Schistocephalus solidus on the body condition and clutch size (CS) of threespine stickleback Gasterosteus aculeatus, its second intermediate host, and the growth of larval parasites in host fish. We tested the hypothesis that single S. solidus infections were more virulent than multiple infections. We also asked whether the metrics of mean and total parasite mass (proxies for individual and total volume, respectively) were consistent with predictions of the resource constraints or the life hi…

Evaluation of bloodstream infections, Clostridium difficile infections, and gut microbiota in pediatric oncology patients.

Bloodstream infections (BSI) and Clostridium difficile infections (CDI) in pediatric oncology/hematology/bone marrow transplant (BMT) populations are associated with significant morbidity and mortality. The objective of this study was to explore possible associations between altered microbiome composition and the occurrence of BSI and CDI in a cohort of pediatric oncology patients. Stool samples were collected from all patients admitted to the pediatric oncology floor from Oct.-Dec. 2012. Bacterial profiles from patient stools were determined by bacterial 16S rRNA gene profiling. Differences i…

C-reactive protein may misdiagnose prosthetic joint infections, particularly chronic and low-grade infections.

BACKGROUND: Periprosthetic tissue cultures, sonication and synovial fluid cultures remain the gold standard for prosthetic joint infection (PJI) diagnosis. However, some 15-20% culture-negative PJI are still reported. Therefore, there is the need for other diagnostic criteria. One point of concern relative to the different definitions of PJI is as to the inclusion of the c-reactive protein (CRP) and the erythrocyte sedimentation rate (ESR) as diagnostic criteria for PJI despite them being non-specific inflammatory blood tests. PURPOSE: The purpose of the present study was to determine the rel

[Coxiella burnetii infections and infections with bacteria of the genus Chlamydia in dairy cattle].

Comparative studies on the prevalence of infections caused by Coxiella burnetii (C. burnetii) and Chlamydia were carried out with 592 cattle older than 2 years and 234 cattle younger than 2 years. Of these 477 originated from 24 dairy herds with considerable fertility problems (positive herds) and 349 from 14 dairy herds without major fertility problems (control herds). For the direct detection of these pathogens in the genitals capture ELISAs were employed, for the demonstration of antibodies the complement fixation test (CFT). Direct detection of C. burnetii and Chlamydia single as well as m…

[Prevention of opportunistic infections in HIV seropositive patients. Prevention of viral infections and mycoses].

CYTOMEGALOVIRUS INFECTION: Primary prophylaxis is logical in high-risk subjects (CD4 < 50) who are asymptomatic; per os ganciclovir is under evaluation. Systematic secondary prophylaxis relies on intravenous ganciclovir or foscarnet. HERPES SIMPLEX AND ZOSTER: Secondary prevention with acyclovir is not recommended due to the risk of resistance. Herpes zoster retinitis is an absolute indication for continuous oral treatment with acyclovir. CRYPTOCOCCUS: Oral fluconazol is required for secondary prophylaxis. CANDIDIASIS: Due to the risk of resistant strains, neither primary nor secondary prophyl

Infections in children with sickle cell anemia. Special reference to pneumococcal and salmonella infections.

Pneumococcal sepsis and/or meningitis are major causes of morbidity and mortality in young children with sickle cell disease. Abnormal complement activity, poor splenic function and a lack of type-specific pneumococcal antibody are responsible for the severity and frequency of these infections. A program consisting of early institution of antibiotic therapy for febrile episodes, antimicrobial prophylaxis, and administration of pneumococcal vaccine may be effective in reducing the incidence of pneumococcal disease. Specific guidelines for infection prevention are presented. Other infections tha…

Epidemiology and prevention of group A streptococcal infections: acute respiratory tract infections, skin infections, and their sequelae at the close of the twentieth century.

Infections of the upper respiratory tract and skin due to group A Streptococcus are common, and the organism is highly transmissible. In industrialized countries and to some extent in developing countries, control efforts continue to emphasize that group A streptococcal pharyngitis should be properly diagnosed and appropriately treated. In developing countries and in indigenous populations where the burden of group A streptococcal diseases appears greatest, the epidemiology is less completely defined and may differ from that in industrialized countries. There is a need for accurately collected…

Viral Respiratory Infections: COVID-19.

On March 11, 2020, the World Health Organization declared COVID-19 a pandemic. By late 2024, COVID-19 had become endemic, with regional surges caused by new variants and declining population immunity. Case fatality rates have decreased because of vaccination and the emergence of less virulent variants, but long COVID persists as a public health concern. Because of structural issues, including those related to employment, housing, health care access, and mistrust, minority and low-income individuals are at higher risk for COVID-19 infection and mortality. Preventive strategies include shared de

Infections in mechanical circulatory support.

Mechanical circulatory support (MCS) devices represent a major advance in the management of advanced heart failure and cardiogenic shock. Despite significant improvements in device technology and patient outcomes, infection remains the most common adverse event, affecting approximately 37% of patients and contributing substantially to morbidity and mortality. This state-of-the-art, multidisciplinary review comprehensively examines infections in patients who require durable or temporary mechanical circulatory support devices with contributions from experts in advanced heart failure, cardiovascu…
