A rare combination of a segmental ulnar fracture with fracture dislocation of the head of the radius and intraarticular fracture of the distal radius concomitant with an ipsilateral scaphoid, lunate and coronoid process fractures is presented. The mechanism of injury could possibly be a tremendous impact on the outstretched hand with a dorsiflexed wrist, fracturing the carpal bones and the distal radius. Transmitted axial forces on the ulna in a pronating forearm resulted in the other fractures. To the best of our knowledge, no such case has been reported. Open reduction with screw fixation of

OBJECTIVE: Scaphoid fracture was the most common carpal fracture and the most challenging. The purpose of this study was to investigate and compare the clinical effects of closed reduction and percutaneous cannulated screw internal fixation under fluoroscopy and arthroscopy-assisted percutaneous cannulated screw internal fixation in the treatment of Herbert B2-type of acute scaphoid fractures.
METHODS: A retrospective controlled study was conducted on 29 patients with Herbert B2-type acute scaphoid fracture with a displacement of >1 mm admitted to our hospital from January 2017 to June 2021.

OBJECTIVE: To conduct a bibliographic review of the infrequent fractures of the carpal scaphoid in pediatric cases based on a clinical case. The diagnosis is usually a difficult one and treatment should be stringent in order to minimize the chance of delayed healing or pseudoarthrosis.
MATERIAL AND METHODS: Ten year-old child with a carpal scaphoid fracture visible in the plain films. The patient did not comply with treatment and removed the cast at three weeks. Despite this healing occurred without any clinical sequelae.
DISCUSSION: This case is especially infrequent given the excellent res

Carpal fractures of bones other than the scaphoid occur at a much lower rate than scaphoid fractures. The close relationship between the carpus, intrinsic and extrinsic wrist ligaments, and wrist kinematics makes a thorough history, clinical examination, and interpretation of imaging for carpal malalignment essential. Carpal malalignment should be addressed with reduction and fixation. Nondisplaced fractures are often treated nonoperatively and displaced intraarticular fractures are almost always treatment operatively. The physician should keep in mind the athlete's specific goals and needs. T

The purpose of this study was to evaluate the compressive capabilities of the Herbert scaphoid screw system. A transverse osteotomy was performed at the waist of eight scaphoids removing a 3 mm wafer of bone. A custom-designed load washer was inserted within this defect to measure intrascaphoid compression during Huene guide application, during screw insertion, after screw insertion and after guide release. The intrascaphoid compression was noted to peak during screw insertion; however it dramatically decreased without any clinical subjective evidence of decreased insertional torque by the sur

Sixty patients after Matti-Russe procedures for scaphoid non-union were investigated. In 85%, autologous corticocancellous bone grafting was successful. Wrist function was good in 65% of the cases. 68% of the patients were satisfied with the result of the operation. Patients younger than 30 years at the time of the operation and with the diagnosis of scaphoid non-union not older than two years showed the best results. In cases with long-time follow-up investigation after Matti-Russe procedure (more than nine years), wrist function as well as the patient's self-estimation become significantly p

There is given a survey of recent findings of the biomechanics of the carpal bones of which the complex of the os scaphoideum, lunatum and capitatum has the key role. The mechanism of injury and posttraumatic instability of the wrist are presented. Rare injuries like subluxation of the os scaphoideum with DISI instability, "scapho-capitate fracture" syndrome, and perilunar dislocation associated with scaphoid fracture (de Quervain) with a proximal fragment of the scaphoid which is turned over about 180 and 90 degrees, are reported in form of clinical cases. In case of subluxation of the os sca

We present a case of volar luxation of the scaphotrapezial bone, in which radiographs suggested luxation of the scaphoid, but unclear relations to the other carpal bones. Computer tomographic reconstruction showed an isolated scaphotrapezial luxation. This was treated by closed reduction and a plaster cast, after which the patient regained normal function of her wrist.

Background: Occult fractures may cause multiple morbidities. If occult fractures were detected earlier, complications may be preventable. This umbrella review and updated meta-analysis will aim to evaluate the use of imaging modalities in detecting occult scaphoid and hip fractures. Methods: The protocol for this study is available in the International Prospective Register of Systematic Reviews (PROSPERO) database (CRD42024525388). The literature search started and ended on 17 March 2024. We searched seven academic databases: MEDLINE, Cochrane Library, Pubmed, Science Direct, Google Scholar, W

Scaphoid nonunions can exist with or without avascular necrosis of the proximal pole, and waist fractures may have an associated humpback deformity. CT best shows the deformity and bone loss, whereas MRI will show avascular necrosis. Operative treatment should be directed at correcting the deformity with open reduction and internal fixation and bone grafting. Vascularized bone grafts should be used in cases of avascular necrosis.

Background Excision of the distal pole of the scaphoid is used to treat arthritis of the scaphotrapezial trapezoid (STT), radioscaphoid joint, and arthritis following scaphoid nonunion. Some patients develop midcarpal instability limiting utilization of this technique. Why some wrists develop postoperative instability while others do not, remains unclear. Purpose To identify the wrists prone to developing midcarpal joint instability we evaluated the effect of midcarpal joint structure on force transfer through the wrist, we hypothesized that the force transfer will be further altered when a

The scaphoid is the common carpal bone to be fractured. Proper clinical and radiological evaluation is required to establish it's diagnosis. The management of acute fractures includes conservative treatment with cast in minimally displaced to open reduction and internal fixation in case of displaced ones. The established nonunion requires open reduction, bone grafting and internal fixation.

This article presents historical aspects, rationale, indications, planning, and execution of anterior interpositional bone grafting technique for unstable scaphoid nonunions. The author's original technique considers four points: (1) preoperative planning based on comparative anteroposterior radiographs in maximal ulnar deviation was used to calculate resection zone, size of the graft, and scaphoid length; (2) a volar approach was used; (3) an iliac crest wedge-shaped corticocancellous graft was interposed; and (4) Kirschner wires were inserted for fixation. Contemporary refinements of the tec

BACKGROUND: Radial styloid fractures (RSFs) and scaphoid fractures often occur together and are considered part of Mayfield stage 1 perilunate fracture dislocation. Proper management is crucial to maintain wrist stability. Interposition of an RSF within a scaphoid fracture is less common and can lead to nonunion.
CASE DESCRIPTION: We report a case of a 35-year-old patient with nonunion of a scaphoid fracture following failed conservative management 2 months' postinjury. Imaging revealed increased flexion of the proximal pole due to the interposition of the RSF, which prevented proper approxim

Background An important surgical landmark in the distal radius is the watershed line. The watershed line is a landmark for the positioning of volar locking plates (VLP) in the distal radius. Inconsistencies remain in the literature as to the presence and dimensions of landmarks in this compact area. We studied the detailed anatomy and dimensions of the distal radius with reference to bony anatomy around the watershed line, with special attention to the area between the pronator quadratus (PQ) and radial styloid. Materials and Methods The distal radius regions of 31 cadavers (23 right sided a
