Although unusual, placement of a Foley catheter into the renal pelvis or ureter has been reported. Proposed predisposing factors for errant placement include pre-existing hydronephrosis or congenital ureteral orifice enlargement. We report a case where aberrant placement of a Foley catheter into the renal pelvis of a transplanted kidney was detected by ultrasound in a patient being evaluated for acute kidney injury. The Foley had been in place for 2 weeks. This is the only reported case we are aware of involving accidental catheterization of a transplant's renal pelvis.

Pneumocystis carinii infection is rare in infants, and raises strong concerns of immune deficiency. This report describes the unusual case of a male infant with concurrent chest infections caused by P carinii and cytomegalovirus. The first case of sporotrichosis from Nepal is reported in a 25-year-old man from a village about 60 km east of Kathmandu. He never travelled outside of Nepal before and had acquired the lymphocutaneous form of the disease after an accidental injury to the right foot while cutting wood. The diagnosis of the case was made by culturing Sporothrix schenckii from the lesions, proving the dimorphic character of the fungus in vitro, its pathogenicity in mice, and its serology. Oral potassium iodide therapy resulted in complete cure.

A 43-year-old man accidentally shot himself with a nail gun. He was seen in the other hospital. Chest computed tomography showed nail penetrating the thorax, and a small amount of pericardial effusion. He was transferred to our hospital and underwent an operation on emergency. After sternotomy and pericardiotomy, the nail was found to reach the right ventricle, and was almost pulled out. Herein, we report a rare instance of paraplegia following percutaneous nephrolithotomy under thoracic epidural anesthesia in a conscious patient. During the period 1 January 1965 to 31 December 1986, 27 patients were admitted to the Rehabilitation Hospital in Hornbaek after traumatic spinal cord injury (SCI) sustained in Greenland. The commonest cause of injury was attempted suicide (7 patients), of which 6 were jumps from buildings. Six resulted from accidental falls to a level below and even 2 out of the 3 cases of SCI following violence were due to falls. There was only one traffic injury, a motor cycle accident. An anterior cerebral artery traumatic aneurysm in a 15-month-old infant is described. The diagnosis was confirmed by computed tomographic angiography. The patient recovered fully after successful clipping of the aneurysm. As the signs of traumatic intracranial aneurysm may be subtle especially in young children, a high degree of suspicion is essential in patients presenting with delayed neurological deterioration after head trauma.
