BACKGROUND: This article reports on the reharvesting potential of newly formed bone in previously used symphysial donor sites.
METHODS: This study included five patients who were scheduled for sequential onlay bone augmentations prior to dental implant placement using the symphysis area as a donor site. At 5 months after bone augmentation, computed tomography scans of donor sites revealed healing from the first surgery. Because additional bone augmentation was required, the same sites were revisited for bone harvesting. Reharvesting was performed 5 months after the first harvesting. A small p

BACKGROUND: In this study the potential of a new and entirely synthetic, nano-structured hydroxyapatite-based biomaterial for sinus floor augmentation is evaluated.
METHODS: 20 sinus floor elevations were carried out in a total of 20 patients. After a healing period of 6 months, in 10 cases cylinder-shaped bone biopsies were taken from the augmented maxillary region using trephine burs.
RESULTS: The healing period progressed without any complications. General and specific histological analysis of the bone biopsies showed a high osteoclast activity at the margin of the biomaterial which was w

The present report describes the oral rehabilitation of a female patient suffering from type III osteogenesis imperfecta. Agenesis of the mandibular anterior teeth and malocclusion caused partly by heavily worn teeth made dental rehabilitation necessary. Before a decision to use implants following a grafting procedure, the osseointegration of microimplants was tested. After 4 and 12 months, bone plugs containing two microimplants were retrieved from the patient. Histologic evaluation showed very sparse/poor osseointegration, and the healing time seemed to be much longer than normal. A decision

PURPOSE: Vastly different surgical techniques have been advocated for osseous reconstruction of the severely atrophic mandible. Endosseous implants placed in autologous bone grafts have been proposed to minimize graft resorption and restore function; however, sufficient bone must exist to support the implants and prevent pathologic fracture. The purpose of this retrospective analysis was to assess the efficacy of autologous bone grafting and the subsequent placement of endosteal implants as a staged procedure in patients with severely atrophic mandibles.
MATERIALS AND METHODS: The records of

The introduction of mixed dentition bone grafting of alveolar clefts means that the alveolar process can be fully restored, permitting adjacent teeth to migrate or be orthodontically moved into the grafted bone. Thus a complete dental arch can be obtained without prosthodontics in the great majority of patients. In addition, oral-nasal fistulae are closed, mucosal recesses eliminated (facilitating oral hygiene) and the long-term periodontal health of the teeth adjacent to the former cleft is improved. Alveolar bone grafting with subsequent orthodontic treatment, together with advances in denta

Achieving functional and aesthetic implant-supported restorations in the partially edentulous patient can be particularly challenging when the implant sites are in the anterior region and not in harmony with the adjacent dentition. The learning objective of this article is to focus on presurgical evaluation of anterior implant sites and systematic development of related therapeutic modalities. Utilization of guided bone regeneration and connective tissue grafting predictably facilitates implant positioning that is compatible with required aesthetic restoration and soft tissue contours. A new method is described (Sailer, 1988) whereby endosseous implants are inserted in the totally atrophic maxilla, the intermaxillary relationship and vertical dimension corrected and a vestibuloplasty performed during one surgical procedure. The method can also be used to treat minor degrees of maxillary alveolar atrophy. The procedure enables 20 mm-long Titanium screws to be used in totally atrophic maxillae. The following surgical procedures are carried out during one operation: A Le Fort I osteotomy, removal of the mucous membrane of the sinus floor, obturation of the denuded maxillary sin

Four partially edentulous patients received particulate autogenous bone and platelet-rich plasma (PRP) in one sinus and particulate autogenous bone alone in the contralateral sinus. After 6 months of healing, two or three Osseotite implants were inserted, and an additional Osseotite mini-implant was placed into the graft through the lateral wall of the sinus. At abutment connection, the mini-implants were retrieved for histologic examination. Despite similar clinical and radiographic healing patterns, a higher bone-to-implant contact rate was observed on the implants placed in bone and PRP tha

BACKGROUND: When a tooth is lost, a general lack of stimulation of the remaining alveolar bone results in a decrease in trabeculae and subsequently, bone density in the post-extraction alveolus. In recent years, numerous bone augmentation techniques employing a range of bone graft materials have been used to reconstruct the deficient vertical and/or, horizontal alveolar ridge dimensions.
OBJECTIVE: To evaluate the efficacy of autogenous dentine grafts in socket preservation post-exodontia.
METHODS: This was a split-mouth study design which included 60 subjects, while the bone density was ass

Background: Patients with a thin gingival phenotype and a narrow buccal alveolar plate are highly susceptible to periodontal complications during orthodontic expansion. Traditional biomechanics often fail to maintain root control in thin alveolar housing. This report presents two clinical cases illustrating soft- and hard-tissue responses to a novel biomechanical approach, the Bone Protection System (BPS), designed to reduce buccal cortical overload during expansion. Case Presentation: Two adult patients with a thin gingival phenotype assessed by a standardized periodontal probe transparency t

Sinus floor elevation can be done by lateral maxillary approach or crestal approach by osteotome techniques as a function of residual bone height. The osteotome technique has been developed for minor sinus floor elevation. This is a method with relatively high predictability but is technically demanding. During the past decade, different osteotome-mediated sinus floor elevation techniques have been developed to augment the sinus for predictable implant placement. All these methods aim to access a better, predictable sinus augmentation with low morbidity. Some of these techniques require specia
