[Permanent deafness following typhoid fever: case report from Dakar, Senegal].

Severe infections due to Salmonella typhi and Salmonella paratyphi are still common in sub-Saharan Africa where many patients are empirically treated for malaria. In addition to the usual clinical signs and complications of typhoid fever, clinicians practicing in high incidence areas must be aware of less common manifestations. The purpose of this report is to describe a case involving an 18-year-old boy who presented with quinine-resistant febrile coma. The final diagnosis was typhoid fever. After recovery the patient presented complete deafness. Discussion raises the question of whether deaf…

Bacterial meningitic deafness: historical development of epidemiology and cellular pathology.

Meningitis resulting in labyrinthitis and its associated hearing loss was first described by several authors during 1864 and 1865 but it was … ous labyrinth. In 1988 Lebel's observation of the effectiveness of dexamethasone in preventing much deafness from meningitis stimulated the examination of the labyrinthine immune response. Immunological mechanisms can account for some of the variable morbidity of unilateral, progressive, less-than-severe deafness.

[Acute unilateral total deafness and vestibular findings after gunshot noise].

BACKGROUND: Acute acoustic trauma is usually acquired during military service after exposure to impulse or blast wave noise. The typical audiometric shape is a notch centered at about 4 kHz with some recovery above this frequency. This is the first case of an immediate induced unilateral total hearing loss in a young soldier following exposure to gunfire noise. CASE REPORT: A 25-year-old right-handed army officer during military training, after realizing a series of five shots with a rifle (G3), immediately experienced on the right ear otalgia, tinnitus and severe hearing loss, without imbala

OPA1 R445H mutation in optic atrophy associated with sensorineural deafness.

The heterozygous R445H mutation in OPA1 was found in five patients with optic atrophy and deafness. Audiometry suggested that the sensorineural deafness resulted from auditory neuropathy. Skin fibroblasts showed hyperfragmentation of the mitochondrial network, decreased mitochondrial membrane potential, and adenosine triphosphate synthesis defect. In addition, OPA1 was found to be widely expressed in the sensory and neural cochlear cells of the guinea pig. Thus, optic atrophy and deafness may be related to energy defects due to a fragmented mitochondrial network.

[Enlarged vestibular aqueduct syndrome in patients with hypoacusis and deafness].

UNLABELLED: In 1978 Valvassori and Clemis have reported that enlarged vestibular aqueduct can cause progressive perceptive hearing loss and finally deafness. AIM: Analysis of CT scans of patients with cochlear implants. Analysis of radiological data of 120 implanted patients was performed. CT examinations were performed during qualification to cochlear implantation. Most of the CT scans were performed using high resolution computed tomography (HRCT). A special attention was paid to patients with progressive perceptive hearing loss and CT of two patients with "gusher" during CI surgery. In one…

The genetic bases for syndromic and nonsyndromic deafness among Jews.

There are hundreds of different mutated genes associated with hearing loss. However, recent findings indicate that a large proportion of both syndromic and nonsyndromic forms of deafness in some Jewish populations is caused by a small number of founder mutations. This review is focused on genetic disorders such as nonsyndromic deafness, Usher syndrome and Alport syndrome, in which hearing loss is a major part of the phenotype and in which the underlying prevalent founder mutations have been recently identified in different Jewish populations. These and other examples of common mutations within…

WFS1 mutations in Spanish patients with diabetes mellitus and deafness.

Wolfram syndrome (WS) is an autosomal recessive neurodegenerative disorder characterised by early onset diabetes mellitus and progressive optic atrophy, as well as other clinical features such as deafness, diabetes insipida, renal tract abnormalities and diverse psychiatric illnesses. A gene responsible for WS was identified in 4p16.1 (WFS1). It encodes a putative 890 amino acid transmembrane protein expressed in a wide spectrum of tissues. Recently, a new locus for WS has been located on 4q22-24, providing additional evidence for the genetic heterogeneity of this syndrome. We have studied the…

Bilateral deafness as a prodromal symptom of basilar artery occlusion.

Bilateral deafness is a rare but possible symptom for ischemia of the vertebrobasilar system, primarily derived from occlusion of anterior inferior cerebellar arteries or their branches. Patients 1 and 2 developed sudden bilateral deafness, soon followed by coma. The proximal segment of the basilar artery was occluded due to atherothrombosis in Patient 1 and arterial dissection in Patient 2. Thrombolytic therapy failed to recanalize the basilar arterial flow. Both patients died of extensive infarction in the vertebrobasilar arterial territory. Sudden bilateral deafness can be a warning sign of…

Topodiagnosis of deafness: strategy for treatment of neurofibromatosis type 2.

Neurofibromatosis type 2 (NF2) causes bilateral hearing loss due to tumour growth in the cerebellopontine angle. We report the results of promontory testing and transtympanic electrocochleography on subjects with deafness due to NF2 referred for an auditory brainstem implant. All 19 ears tested revealed loss of cochlear microphonics. Nine ears (mainly without previous treatment) revealed auditory perception during promontory stimulation, indicating cochlear deafness. One of these subjects has been successfully provided with a cochlear implant. The other 10 ears (mainly after previous surgery) …

Neuroimaging study in autosomal dominant cerebellar ataxia, deafness, and narcolepsy.

Four patients affected with autosomal dominant cerebellar ataxia, deafness, and narcolepsy underwent brain CT and MRI. Radiologic findings were supratentorial atrophy (more pronounced than infratentorial atrophy), pronounced dilatation of the third ventricle, low T2 signal intensity in the basal ganglia, loss of cerebral cortex-white matter differentiation, and periventricular high-signal rims. 2-[18F]Fluoro-2-deoxy-D-glucose PET was done with one patient, without specific findings. Genetic analyses excluded SCA-1, SCA-2, SCA-3, SCA-6, SCA-7, DRPLA, and huntingtin gene mutations.

Autosomal dominant polycystic kidney disease associated with familial sensorineural deafness.

Autosomal dominant polycystic kidney disease (ADPKD) is characterized by both renal and non-renal disorders. Extrarenal involvement includes noncystic manifestations such as cardiovascular abnormalities, colonic diverticula and intracranial aneurysms. Familial sensorineural hearing loss (SNHL) has been included in the definition of Alport's syndrome. However, other types of nephropathy have been occasionally associated with hereditary deafness. The association of ADPKD with hereditary SNHL has not been previously documented. We report a family with ADPKD associated with bilateral sensorineural…

Cochlear implantation as a successful rehabilitation for radiation-induced deafness.

Radiotherapy of the head and neck can be associated with conductive and/or sensori-neural hearing loss. We report the case of a 67-year-old man who developed complete bilateral deafness caused by labyrinthitis and radiation-induced neuritis of the acoustic nerve after postoperative radiotherapy of a nasopharyngeal carcinoma. Two years postoperatively extensive clinical workup including computed tomography and magnetic resonance imaging showed no recurrence or secondary brain tumors. To facilitate sound perception a Combi 40 cochlear implant was implanted. Because of fibrosis the insertion dept…

[H.E.L.P. therapy (heparin-induced extracorporeal LDL precipitation) in sudden deafness].

BACKGROUND: The H.E.L.P. (Heparin-induced extracorporal low-density lipoprotein precipitation) system selectively eliminates fibrinogen, LDL (low-density lipoprotein) cholesterol, triglycerides, and LP (a) (lipoprotein a) from the blood plasma by means of extracorporal circulation. The reduction of these macromolecules immediately improves the hemorheological situation and leads to a significant decrease of plasma viscosity and red cell transmission time (RCTT), which correlates to the deformation of red blood cells. There are no known contraindications for this therapy. PATIENTS: Sudden hear

Blood viscosity and plasma viscosity in patients with sudden deafness.

Blood viscosity and plasma viscosity were measured in 51 patients with sudden deafness (SD) and 70 controls with normal hearing. Blood viscosity and plasma viscosity in patients with SD at their first medical examination were significantly higher than in the control group. The difference in viscosimetry results between the two groups was greater at higher shear rates. The data obtained in viscosimetry and pure-tone audiometry were analyzed after dividing the patients into a high viscosity group and a normal viscosity group. The correlation between average hearing level in pure-tone audiogram a…
