Solitary caecal diverticulitis as an unusual cause of a right iliac fossa mass: a case report.

Inflammation of a solitary caecal diverticulum is an uncommon pathological condition. Preoperatively the condition is almost indistinguishable from appendicitis, and is often confused with carcinoma of the caecum during operation. The typical patient with this condition is male, Asian, and in the fourth decade of life. This case is unusual in that the patient was a 26-year-old Caucasian man.

Management of acute diverticulitis in the East Anglian region: results of a United Kingdom regional survey.

PURPOSE: Acute diverticulitis is the most common presentation of diverticular disease; however, no published guidelines for management are available in the United Kingdom. This survey was designed to assess the current United Kingdom regional practice compared with the guidelines published by The American Society of Colon and Rectal Surgeons. METHODS: A questionnaire survey focused on the management of acute diverticulitis was sent to all consultants, specialist registrars, and staff-grade surgeons in general surgery in one United Kingdom region. RESULTS: Eighty-two questionnaires were retur…

Right colon, sigmoid colon, and transverse colon diverticulitis in the same patient: report of a case.

Although sigmoid colon diverticulitis is frequently seen, right colon and transverse colon diverticulitis remain rare forms of the disease. This case report examined the disease course of a 46-year-old female who first presented to our institution in 1990 with perforated right-sided diverticulitis. During the next 11 years, she developed sigmoid colon diverticulitis and then transverse colon diverticulitis. The right and sigmoid colon diverticulitis were treated with surgery and the transverse colon diverticulitis was managed conservatively. This is the first reported case of a single patient …

[An unusual complication of perforated sigmoid diverticulitis: gas in the portal vein with miliary liver abscesses].

A patient presented with gas in the portal vein and miliary liver abscesses due to perforation of a sigmoid diverticulitis abscess. The outcome was favorable after surgical treatment. Gas in the portal vein, a sign of extreme seriousness, is generally the result of intestinal necrosis from ileo-mesenteric artery infarction, responsible for more than 75% of deaths. Diagnosis of this exceptional complication, suggested from a straight abdominal film, was confirmed by hepatic ultrasound and scan imaging. Urgent surgical intervention can hopefully, as in the present case, result in the patient's s…

Epidemiological study of colovesical fistula as a complication of colonic diverticulitis in Japan: an analysis of claims data.

PURPOSE: Colovesical fistula, a serious complication of colonic diverticulitis, often requires surgical intervention. As the epidemiological data for colovesical fistula associated with colonic diverticulitis in Japan are limited, this study investigated its incidence and characteristics using a claims database. METHODS: This retrospective study analyzed the JMDC Claims Database (January, 2005 to August, 2023). Patients with colonic diverticulitis were identified based on diagnosis codes and concurrent antibiotic treatment. Patients diagnosed with colovesical fistulas who underwent surgery wer…

Duodenal Diverticulitis as a Cause of Acute Severe Pancreatitis In the Patient With Clinical Obesity - a Case Report.

BACKGROUND: Diverticula of the gastrointestinal tract are common in older adults, most often affecting the colon. While typically asymptomatic, acute diverticulitis represents the most frequent complication and may lead to severe outcomes, including perforation, abscess formation, and diffuse peritonitis. Duodenal diverticula on the other hand is a rare clinical entity, and is frequently detected incidentally during proximal endoscopy. While most cases are asymptomatic, inflammation of periampullary duodenal diverticula may result in serious complications. Acute pancreatitis secondary to duode…

Non-elective colectomy for diverticulitis in the U.S.: a retrospective comparison of robotic, laparoscopic, and open approaches.

BACKGROUND: Minimally invasive techniques are increasingly used in emergency surgery, including in acute diverticulitis. While studies have shown that laparoscopic colectomy in the emergent/urgent management of acute diverticulitis has better outcomes compared to open surgery, data on robotic surgery outcomes remain limited. This study aims to compare the outcomes of patients with acute diverticulitis requiring non-elective operations across open, laparoscopic, and robotic surgical approaches. METHODS: A retrospective analysis from the Premier healthcare database (2019-2023) was conducted, in…

Appendiceal diverticulitis mimicking uncomplicated acute appendicitis on CT: A case report and review of the literature.

Abdominal contrast-enhanced computed tomography (portal venous phase, thin-slice reconstruction) demonstrated a distended appendix measuring 10 mm in diameter with peri-appendiceal fat stranding, without visible appendicolith, focal outpouching, or mural defect, suggesting uncomplicated acute appendicitis. Appendiceal diverticula were not identified preoperatively. The patient underwent laparoscopic appendectomy. Intraoperatively, multiple inflamed diverticula were observed. Histopathological examination confirmed appendiceal diverticulitis with peridiverticulitis, without evidence of neoplasm…

Long-term outcomes in recurrent diverticulitis: a systematic review of treatment strategies and recurrence rates.

BACKGROUND: Diverticulitis prevalence is on the rise, with recurrences occurring in up to one-third following an uncomplicated initial episode. While sigmoid resection was historically routine after a second episode, current guidelines recommend individualization. This review evaluates long-term outcomes for recurrent diverticulitis, comparing different treatment approaches. METHODS: A systematic review (PROSPERO ID CRD42024600674) adhering to PRISMA guidelines searched several electronic databases. Clinical trials and observational studies published in English over the past decade that repor…

A case report of coexistence of acute appendicitis and Meckel's diverticulitis causing generalized acute peritonitis.

INTRODUCTION: The coexistence of appendicitis and Meckel's diverticulitis is a rare condition that can lead to acute peritonitis. It is difficult to diagnose preoperatively and requires emergency surgical intervention. We report a case presenting with both lesions. CASE PRESENTATION: A 43-year-old female patient was admitted with generalized peritoneal irritation syndrome, developing in a febrile context and associated with a biological inflammatory syndrome. Computed tomography revealed signs of peritonitis and an inflamed appendix. Laparoscopy, converted to laparotomy, revealed coexisting a…

Ciprofloxacin and Metronidazole As Rare Culprits of Drug-Induced Immune Thrombocytopenia in the Setting of Diverticulitis Treatment.

Drug-induced immune thrombocytopenia (DITP) is a syndrome in which antibodies bind to platelets only in the presence of a drug, leading to platelet destruction. A variety of medications, herbs, vaccines, and foods have been commonly associated with DITP. Here, we present a case of DITP in a 61-year-old female who was receiving two widely prescribed antibiotics, ciprofloxacin and metronidazole, which are not commonly linked to DITP. The patient developed thrombocytopenia within 24 hours of initiating these antibiotics, and her platelet count rapidly normalized following discontinuation of the d

A Retrospective Study on Use of Neutrophil-Lymphocyte Ratio as a Prognostic Measurement to Assess Acute Diverticulitis.

BACKGROUND: Acute diverticulitis (AD) is a common surgical condition and the Neutrophil-lymphocyte ratio (NLR) is an emerging biomarker ratio used to guide its management. The aim of this study is to validate and assess the utility of the NLR in AD in the Australian population. METHODS: This is a single centre retrospective observational study of patients who presented to the emergency department with the diagnosis of AD between September 2018 and September 2023, in Ipswich, Queensland. One thousand five hundred and forty patients were screened against exclusion/inclusion criteria and 634 pat…

Complicated sigmoid diverticulitis in a 16-year-old patient in Saudi Arabia: a case report.

Diverticular diseases uncommonly occur at a young age. However, its prevalence in younger individuals is rising. Atypical presentations will lead to misdiagnoses and unindicated surgery. Colovesical fistulas remain the most common type that is associated with diverticular diseases. A 16-year-old female patient presents with 2 months history of left-sided lower abdominal pain, in addition to, longstanding history of urinary incontinence since early childhood. Imaging studies, including computed tomography and magnetic resonance imaging, revealed complicated sigmoid diverticulitis by fistula for…
