The ineffective cleaning of surgical instruments may be a vector for the transmission of hospital-acquired infections. The aim of this research was to investigate whether further decontamination procedures need to be instigated in sterile-service departments (SSDs) to reduce the risk of nosocomial illnesses, such as endotoxemia, sepsis, or iatrogenic Creutzfeldt-Jakob disease (to date, 1,147 cases of confirmed Creutzfeldt-Jakob disease deaths in the United Kingdom since 1990 have been reported). Instrument sets were obtained from nine anonymous United Kingdom National Health Service (NHS) prim

We report the findings of the study of 4185 food samples and 958 environmental samples collected in Italy in the period 1990-1999 and tested for the presence of Listeria monocytogenes. The strains isolated were biochemically and serologically characterised. We found a fairly high percentage of L. monocytogenes contamination in food (12.8%), whereas the level of contamination was lower in the environment (environment and work surfaces in food processing plants) (6.1%). Serotyping showed a prevalence of a few serotypes (i.e., 1/2a, 1/2b, 1/2c and 4b), which were the same as those found in clinic

Hepatitis C virus (HCV) infections are frequent in hemodialyzed patients and are mainly related to transfusions and nosocomial contamination. HCV-related infection may result in cirrhosis in 10% of dialysis patients and is worsened by transplantation because of the immunosuppressive therapy for prevention of graft rejection. Because there is a risk for significant liver disease and because cirrhosis contraindicates a renal transplantation, a liver biopsy should be performed early in HCV-RNA positive hemodialysis patients to evaluate histologic impact of the liver disease. A combined liver-kidn

Fungal endocarditis is an exceedingly rare complication of indwelling central venous catheters in adults. Here we describe what appears to be the first case of a right atrial thrombus superinfected with the yeast Torulopsis (Candida) glabrata and attached to an indwelling superior vena cava catheter that was not used for parenteral nutrition. A large vegetation-like mass adherent to the catheter tip was visualized by transesophageal echocardiography in a patient who presented with signs of septic pulmonary embolism. Following open-heart surgery, the definitive diagnosis was established by hist

Intravascular catheters play an important role in infections in intensive care and hemodialysis patients. This becomes evident only if full microbiological diagnoses are made. Difficulties in the diagnosis and treatment of microbially colonized catheters make the prevention of infection particularly important. The most important preventive measures are a strict evaluation of the indications for the use of the catheter and strict hygienic precautions during insertion and maintenance of the central venous catheter. Other measures, some of which are controversial, may be considered, such as the s

Wooden sticks used to suspend feces obtained for surveillance cultures were found to be the source of Rhizopus microsporus var. rhizopodiformis causing a pseudo-outbreak among 17 immunocompromised patients cared for in three different wards. Nonsterile wooden products should therefore not be used for collecting, handling, and processing specimens for microbiological examination.

OBJECTIVE: To investigate the ability of a silver-releasing device to protect the catheterized bladder from infection by blocking the ascending migration of bacteria from contaminated urine-drainage bags.
MATERIALS AND METHODS: A simple physical model of the catheterized bladder and drainage system was used with the device located in the drainage-tube just below the sampling port. Urine was supplied to the model at 1.0 mL/min and the drainage bag was contaminated with Pseudomonas aeruginosa, Escherichia coli and Proteus mirabilis. Over 10 days, urine from the bag, catheter sampling-port and b

PURPOSE: We performed a systematic examination of the rate and level of contamination of contact lens cases in a population of asymptomatic contact lens wearers using commercially prepared chemical disinfection solutions.
METHODS: Solutions from the lens cases and from the conjunctiva of 21 asymptomatic contact lens wearers were examined.
RESULTS: Contamination was found in five (24%) of the lens cases; four of the isolated microorganisms were recognized as potential ocular pathogens, namely Xanthomonas maltophilia, Pseudomonas cepacia, Serratia liquefaciens, and Serratia plymuthica. Serrati

The action of microwaves on stainless steel scalpel blades contaminated with Mycobacterium bovis was investigated. The complete destruction of M. bovis was obtained with 4 min of microwave exposure. When the preparations were subjected to scanning electron microscopy, the bacteria had undergone a progressive series of alterations consisting, initially, of the formation of deep pits in the bacterial body and eventually the complete disintegration of the microorganisms. Such phenomena are less evident when this mycobacterium is exposed to other sterilization methods such as dry heat or autoclavi

In the interval September 1993-April 1994, 38 catheters from children admitted to the Children's Hospital of Iaşi were examined microscopically and by semiquantitative culture, in the children with sepsis and/or positive catheters hemocultures being performed concurrently. Twenty eight of these catheters proved to be sterile, the other 10 being positive to coagulase-negative staphylococci, gram-negative bacilli from Klebsiella-Enterobacter group, Candida species, the number of colonies/plate varying generally in concordance with the microscopic positive score. Of the 6 cases with > 50 colonies

Since Aubaniac (1) described the puncture of the subclavian vein in 1952, and specially after the standardization of parenteral nutrition by Dudrick et al. (11) in 1968, much has been published about complications caused by percutaneous central venous catheterization. Among the various complications provoked by this procedure, a very important one is "primary sepsis" or "catheter-related sepsis", both because of its frequency and because of the morbidity and mortality it causes (18,19). It is, however, difficult to diagnose this complication. The main difficulty lies in differentiating cathete

An intraluminal culture method was evaluated for central venous catheters and compared to conventional catheter tip cultures and cultures from the insertion site. The colonization-rate of the catheters was 43.7% using conventional tip culture, 34.4% with the intraluminal method and 40.1% at the skin puncture site. Only 37.5% of the catheters showed identical bacteria at the skin puncture site and catheter-tip, compared with 87.5% between intraluminal culture and catheter-tip culture (P less than 0.05).

Following use with halothane, ten anaesthestic machines were sampled using infrared analysis for halothane contamination. Baseline measurements of halothane were made in the room and at the machine's common gas outlet. Five per cent halothane with four litres per minute oxygen flow was delivered for ten minutes into a scavenged breathing circuit. Halothane was then discontinued, an oxygen flow rate of 12 litres per minute was begun, and continuous measurements were made until the halothane concentration became undetectable. Baseline measurements of the rooms and anaesthestic machines ranged fr

Following a definition of epidemic keratoconjunctivitis (EKC) and the etiologic agent responsible, a brief history of the disorder dating back to 1889 is given. The clinical course, signs, symptoms, and corneal involvement of this disease, which sets it apart from other adenovirus conjunctivitis, are described. During the late summer of 1981, epidemics of viral conjunctivitis were reported in India and Miami, Florida. At this same time, in our office in Memphis Tennessee, we examined and treated 15 patients who had EKC of the adenovirus type 8 variety. A brief history, objective findings, and

The results of a simplified quantitative broth dilution quantitative tip culture (QTC) of 331 central venous catheters were compared with clinical data prospectively recorded in critically III patients to diagnose bacteremic or nonbacteremic catheter-related sepsis (CRS) (36 catheters), as opposed to contamination (42 catheters) or simple colonization from a distant septic focus (seven catheters). Thirty-five of 36 catheters associated with CRS yielded 10(3) colony-forming units per milliliter (CFU/mL) or more, and 3.8 X 10(2) Candida organisms grew from one. In contrast, 5 X 10(2) CFU/mL or l

In a study of 50 peripheral venous cannulae removed from neonates in the special care baby unit, quantitative and qualitative techniques for cannula culture were compared and the results correlated with detailed clinical information. High-density colonization was demonstrated on the external and internal surfaces of nine, and on the external surface alone of four, cannula tips. Internal surface colonization was always associated with heavy external surface growth. In predicting high-density colonization by quantitative methods, qualitative culture in broth was 100% sensitive and 89% specific.

Adrenal incidentaloma refers to an asymptomatic adrenal mass detected through an imaging procedure performed for reasons unrelated to adrenal dysfunction or suspected dysfunction. In general, adrenal incidentalomas are non-functioning adrenal adenomas, but in some cases, may require therapeutic intervention: eg., adrenocortical carcinoma, pheochromocytoma, primary aldosteronism, cortisol hypersecretion, or adrenal insufficiency. Hormone assessment is crucial to characterize adrenal incidentaloma. Nowadays, various hormone assay methods are available, such as immunoassay and mass spectrometry.
