A 21-year-old Asian man developed asymmetrical keratoconus (OD > OS) in the context of right-sided periocular vitiligo. The patient was undergoing topical pimecrolimus treatment for his vitiligo, which caused periocular eye irritation, leading to aggressive, unilateral right-eye rubbing.
