BACKGROUND: In patients with severe osteoarthritis of the knee with prior ipsilateral hip fusion who require total knee arthroplasty (TKA), a controversial issue is whether to first convert the hip fusion to a total hip arthroplasty (THA) or to perform TKA without reconstruction of the hip. Also, immobility of the ipsilateral, fused hip adds significant technical challenge because the usual positioning of the leg requires modification in order to gain access needed for the TKA.
TECHNIQUE: In such cases, we position the patient with the knee suspended, similar to how we perform knee arthroscop

CASE: A 39-year-old healthy woman without known genetic condition presented with bilateral patellar dislocations. Imaging demonstrated complete absence of the trochlear groove, defined as trochlear aplasia. Surgical treatment was initially performed on the left knee with refixation of an osteochondritis dissecans lesion, medial patellofemoral ligament reconstruction, and medializing tibial tubercle osteotomy. Intraoperative confirmation of the absence of trochlear groove made trochleoplasty unfeasible. At the 1-year follow-up, VAS was 0/10 without patellar dislocation.
CONCLUSION: Trochlear a

In the present paper the current clinical knowledge about proprioception is given for the shoulder, knee, ankle, elbow and the radiocarpal joint. Proprioceptive capabilities are decreased after joint injuries such as ACL or meniscus tears, shoulder dislocation, ankle sprain and in joints with degenerative joint disease. Some surgical procedures seem to restore the proprioceptive abilities; others do not. Elastic knee bandages or ankle braces increase different proprioceptive factors like ankle reproduction capability or sports-specific abilities. The present information on proprioception will

BACKGROUND: Medial patello-femoral ligament (MPFL) reconstruction is one of the therapeutic options to treat patellofemoral instability. Classically, a à la carte treatment of skeletal and ligament abnormalities is described. This option is difficult to achieve in children because bony procedures can damage the femoral and/or tibial growth plate. The objective was to evaluate a strategy for isolated reconstruction of the MPFL in the treatment of objective patellar instabilities in children, in a large cohort. The return to sport, knee function and pain or discomfort were studied as secondary e

We previously evaluated 119 consecutive total knee arthroplasties that were performed in eighty-six patients with use of the cemented LCS (low contact stress) rotating-platform system with an all-polyethylene patellar component. The average age of the patients at the time of surgery was seventy years (range, thirty-seven to eighty-eight years). The purpose of this study was to report the updated results at a minimum follow-up of fifteen years. Thirty-seven patients (fifty-three knees) were living, and no patient was lost to follow-up. No knee was revised because of loosening, osteolysis, or we

OBJECTIVE: Patellofemoral instability is common and affects a predominantly young age group. Chondral injury occurs in up to 95%, and includes osteochondral fractures and loose bodies acutely and secondary degenerative changes in recurrent cases. Biomechanical abnormalities, such as trochlear dysplasia, patella alta, and increased tibial tuberosity-trochlear groove distance, predispose to both recurrent dislocations and patellofemoral arthrosis.
DESIGN: In this article, we review the mechanisms of chondral injury in patellofemoral instability, diagnostic modalities, the distribution of lesion

In this report, we present the case of a Saudi baby boy with a rare de novo interstitial deletion of chromosome 2q14.1-q22.1. His karyotype … s, and horseshoe kidney. However, our proband presented with new features which included congenital knee dislocation, congenital vertical talus, bilateral hip dislocation, and myelomeningocele. Moreover, all the reported cases share GLI2 deletion which may reflect the phenotypic features in patients with the deletion of 2q14.1q22.1.

A 15-year-old boy presented with a fixed, irreducible congenital dislocation of the hip associated with other multiple lower extremity growth disturbances secondary to neonatal multifocal osteomyelitis. The affected hip had very limited abduction, and the patient had a very severe Trendelenburg gait secondary to the dislocation. The hip was reconstructed according to the Ilizarov method, by a combination of maximum proximal femoral adduction osteotomy in the subtrochanteric region and distal femoral corticotomy, to permit the gradual realignment of the knee into the new weight-bearing axis pro

Six knees in three patients with Ellis-van Creveld syndrome were treated with lateral soft tissue release and corrective osteotomy of the tibia at 10 years of age on average. The main feature was valgus deformity with lateral dislocation of the patella. All patellae were reduced. The valgus deformity improved from 35 degrees (range, 48 degrees-20 degrees) to 17 degrees (range, 35 degrees-5 degrees) of the femorotibial angle (FTA) on average, although the FTA in five of six knees was < 5 degrees after surgery. There was one recurrent case and one transient peroneal nerve palsy. The reason for u

OBJECTIVE: To investigate the clinical significance of Q-angle measuring under different conditions in female recurrent patellar dislocation female patients.
METHODS: Between August 2012 and March 2013, 10 female patients (11 knees) with recurrent patellar dislocation were collected as trial group; 20 female patients (20 knees) with simple meniscus injury were collected as control group at the same time. Q-angle was measured in extension, 30 degrees flexion, 30 degrees flexion with manual correction, and surgical correction in the trial group, and only in extension and 30 degrees flexion in t

There is no guideline and consensus about when and how to treat accompanying multi-ligaments injuries, especially anterior and/or posterior cruciate ligaments, in tibial plateau fractures. We report one case of fracture and dislocation of tibial plateau, treated by open reduction and internal plates fixation, augmented by trans-articular Kirschner wire fixation to overcome instability and malrotation of the knee joints discovered intraoperatively. The Kirschner wire was removed about four weeks after the index operation, and the patient begun functional exercise from then on. The fracture unit

This study aimed at systematic documentation of lateral knee pain in a consecutive series of 1102 cruciate-substituting, guided motion total knee arthroplasties (TKA) (Journey, Smith and Nephew, Memphis, TN, USA) performed in 1085 patients; 1070 knees were available for review. Follow-up time ranged from one to five years, with a mean of 2.5 years. Symptoms mimicking the well known iliotibial band (ITB) friction syndrome were observed in 77 knees (7.2%). Initial conservative treatment consisted of anti-inflammatory medication (77 knees) and local steroid injection (35 knees). The pain persiste

Isolated dislocations of the proximal tibiofibular joint are a rare condition. Missed diagnosis can lead to chronic knee pain and disability. Early recognition should be followed by immediate closed reduction or open reduction and joint transfixation. We present a young athlete with this injury which was treated successfully by open reduction.

We describe a syndrome combining abnormalities of the pelvis, knee and foot in three related patients with a familial history of small dislocated patellae. The clinical and radiological appearance of the patella and pelvis is consistent with the 'small-patella' syndrome, a rare autosomal dominant disorder. There were also previously unreported deformities affecting the feet.

BACKGROUND: Diastrophic dysplasia results in severe disproportionate short stature, generalized joint deformities, and early osteoarthritis. The knee joint often has an abnormal valgus position and is unstable, and degeneration of all joint compartments occurs, even during growth. The purpose of our study was to evaluate the clinical and radiographic results of total knee arthroplasty in a series of patients with diastrophic dysplasia.
METHODS: Between February 1992 and March 2000, twenty-one primary total knee arthroplasties were performed on fourteen consecutive patients with severe osteoar

INTRODUCTION: Acute popliteal artery thrombosis can occur as a result of direct trauma to the knee in cases of knee dislocations, distal femur fractures, and tibial plate fractures. Traction injury can induce indirect trauma, including popliteal artery laceration. Popliteal artery transection injury can have fatal effects for patients, since irreversible ischemia can occur in as little as 6-8 h.
CASE REPORT: We report a rare case of 21-year-old male patient with closed displaced medial condyle fracture of tibia with popliteal artery thrombosis which was managed by intra-arterial thrombolysis.…

BACKGROUND: Patellar resurfacing during total knee arthroplasty (TKA) remains controversial. The aim of this study was to evaluate the long-term outcomes of a mobile-bearing TKA without patellar resurfacing.
METHODS: We assessed the outcomes in 503 patients (600 knees) who had cemented LCS (low contact stress) mobile bearing TKA without patellar resurfacing at a minimum follow-up of 10 years. Clinical outcome scores and radiological assessment were used.
RESULTS: Four knees were revised, two had bearing dislocation and nine patients (1.5%) had secondary resurfacing for anterior knee pain. Th

We have reviewed 85 knees in 71 patients after total-condylar posteriorly stabilised (Insall-Burstein) knee replacement with an average follow-up of five years. Excellent or good results were obtained in 90% with an average maximum flexion of 98 degrees. The four poor results (5%) included two with deep infection, one with patellar dislocation and one with loosening. Four other knees (5%) showed signs of probable tibial loosening, but the patients were asymptomatic, the clinical results had not deteriorated with time and lucent lines had not progressed. Varus alignment of the knee and a varus
