Mathematical Model to Assess Potential Reduced Specificity When Switching to New Screening Assays at Blood Donation Centers.

BACKGROUND: Switching to new infectious disease blood donor screening assays can precipitate an initial decrease in specificity in an established donor population followed by an increase of specificity, referred to as a "cleaning effect". We developed a mathematical model to simulate this and to measure the stabilization of specificity. METHODS: A modified exponential distribution curve was created to show the impact of donation frequency on the cleaning of the donor pool. Other parameters (e.g., number of donations from repeat donors/donations per month, average and minimum times between don

Accuracy of Highly Limited Echocardiographic Screening Images for Determining a Structurally Normal Heart: The Quick-Six Study.

INTRODUCTION: Experienced echocardiographers can quickly glean diagnostic information from limited echocardiographic views. The use of limited cardiac ultrasound, particularly as a screening tool, is increasing. During the COVID-19 pandemic, limited cardiac ultrasound has the major advantage of reducing exposure time between sonographer and patient. The sensitivity and negative predictive value of a "screening" echocardiogram with highly limited views is uncertain. AIM/METHOD: We examined the accuracy of limited echocardiography in 203 consecutive, de novo studies. We used six images: paraste

Applying a novel approach to scoping review incorporating artificial intelligence: mapping the natural history of gonorrhoea.

BACKGROUND: Systematic and scoping literature searches are increasingly resource intensive. We present the results of a scoping review which combines the use of a novel artificial-intelligence-(AI)-assisted Medline search tool with two other 'traditional' literature search methods. We illustrate this novel approach with a case study to identify and map the range of conditions (clinical presentations, complications, coinfections and health problems) associated with gonorrhoea infection. METHODS: To fully characterize the range of health outcomes associated with gonorrhoea, we combined a high y

Screening for sickle cell disease in newborns: a systematic review.

BACKGROUND: Sickle cell disease (SCD) is an inherited autosomal recessive disorder caused by the replacement of normal haemoglobin (HbA) by mutant Hb (sickle Hb, HbS). The sickle-shaped red blood cells lead to haemolysis and vaso-occlusion. Especially in the first years of life, patients with SCD are at high risk of life-threatening complications. SCD prevalence shows large regional variations; the disease predominantly occurs in sub-Saharan Africa. We aimed to systematically assess the evidence on the benefit of newborn screening for SCD followed by an earlier treatment start. METHODS: We sy

Applying Ethical Principles When Discussing Alcohol Use During Pregnancy.

Over the past 2 decades, more women in the United States are engaging in excessive alcohol use, including women of reproductive age. Consuming alcohol in amounts greater than recommended limits is associated with an increased risk for adverse health effects, such as breast cancer, hypertension stroke, spontaneous abortion, and infertility. No safe time, safe amount, or safe type of alcohol to consume during pregnancy has been identified. Contradictory beliefs about alcohol use, fear of stigmatization, and potential legal consequences can provide challenges for health care providers who communi

Atherosclerosis.

Atherosclerosis, the formation of fibrofatty lesions in the artery wall, causes much morbidity and mortality worldwide, including most myocardial infarctions and many strokes, as well as disabling peripheral artery disease. Development of atherosclerotic lesions probably requires low-density lipoprotein, a particle that carries cholesterol through the blood. Other risk factors for atherosclerosis and its thrombotic complications include hypertension, cigarette smoking and diabetes mellitus. Increasing evidence also points to a role of the immune system, as emerging risk factors include inflamm

Systematic review with meta-analysis: ultra-thin gastroscopy compared to conventional gastroscopy for the diagnosis of oesophageal varices in people with cirrhosis.

BACKGROUND: Haemorrhage from ruptured oesophageal varices is a common cause of death in people with cirrhosis. Guidelines recommend screening for varices at time of cirrhosis diagnosis and throughout the course of the disease. Conventional gastroscopy is the criterion standard for variceal screening; however, is invasive, costly, and carries risks related to use of sedation. Ultra-thin gastroscopy (using endoscopes with a shaft diameter ≤6 mm) has been proposed as an alternative method of variceal screening that mitigates these risks. AIM: To determine the diagnostic accuracy of ultra-thin ga

Implementation of a Risk Assessment Tool to Reduce Aspiration Pneumonia in Nonstroke Patients.

BACKGROUND: Aspiration pneumonia is one of the most common forms of pneumonia in hospitalized adults. Aspiration is potentially preventable when healthcare providers take action to decrease its occurrence. Standardized protocols can improve consistency with identification, assessment, and treatment of hospitalized patients at risk of aspiration pneumonia. PROBLEM/PURPOSE: There was no process in place to identify patients at risk of aspiration who did not present with stroke symptoms. The aims of this project were to assess that action steps were taken in patients who assessed positive for as

Carcinoid Heart Disease: a Comprehensive Review.

PURPOSE OF REVIEW: Carcinoid heart disease is a rare disorder that is associated with significant morbidity and mortality. In this review of the literature, we will present current concepts in diagnosis and management of carcinoid heart disease. RECENT FINDINGS: Recent expert consensus guidelines highlight the role of echocardiography and screening with NT-proBNP for the evaluation of carcinoid heart disease. Advances in medical therapy along with better surgical outcomes highlight the experience and expertise that has been gained in the treatment of carcinoid heart disease. Carcinoid heart d
