Pulmonary artery pulsatility and effect on vessel diameter assessment in magnetic resonance imaging.

BACKGROUND: Information about thoracic vascular sizes can crucially affect clinical decision-making in cardiovascular disease. A variety of imaging techniques such as catheter angiography, contrast enhanced computed tomography (CT) and cardiac magnetic resonance imaging (CMR) are routinely used to measure vascular diameters. Traditionally, CMR black blood sequences were the main anatomical tool for visualization of vascular anatomy and still are in many centers. More recently, the vessel diameters are measured on multiplanar reconstructions derived from static magnetic resonance angiography (M

[Velocity estimation of aortic propagation based on radial pulse wave analysis].

We analyzed the possibility of using the radial pulse wave morphology, obtained by a movement transducer, to evaluate the aortic pulse wave velocity. The radial pulse wave signals were obtained by using a transducer, located on the pulse palpation area, in 167 healthy normotensive male volunteers, ages 20 to 70. The reflected wave was identified in every case. Also, a speed coefficient was defined as the ratio between the individual's height and the time between the maximum systolic wave and the arrival time of the reflected wave. We found that the specified coefficient in normotensive individ

Aortic stiffness and aortic dilation in infants and children with tetralogy of Fallot before corrective surgery: evidence for intrinsically abnormal aortic mechanical property.

OBJECTIVE: The present study tested the hypothesis that there is an intrinsic abnormality of aortic elasticity in infants/children with tetralogy of Fallot (TOF) before corrective surgery. The study also determined the independent and quantitative effects of aortic volume load on aortic dilation in this group of TOF patients. METHODS: Aortic stiffness (pulse wave velocity; PWV) and aortic volume load (aortic volume flow) were measured during catheterization in 37 infants and children with TOF before corrective surgery and in 55 control subjects. RESULTS: PWV was significantly higher in TOF p

Investigation of spiral blood flow in a model of arterial stenosis.

The spiral component of blood flow has both beneficial and detrimental effects in human circulatory system [Stonebridge PA, Brophy CM. Spiral laminar flow in arteries? Lancet 1991; 338: 1360-1]. We investigate the effects of the spiral blood flow in a model of three-dimensional arterial stenosis with a 75% cross-sectional area reduction at the centre by means of computational fluid dynamics (CFD) techniques. The standard k-omega model is employed for simulation of the blood flow for the Reynolds number of 500 and 1000. We find that for Re=500 the spiral component of the blood flow increases bo

Correlations among critical closing pressure, pulsatility index and cerebrovascular resistance.

We attempted to explore the relationships among critical closing pressure (CrCP), resistance-area product (RAP) and traditional resistance indices of cerebral hemodynamics. Twenty healthy volunteers were studied. Blood pressure was obtained with servo-controlled plethysmography. Cerebral blood flow velocity (CBFV) was monitored by transcranial Doppler. Hemodynamic changes were induced by hyperventilation and by 5% CO(2) inhalation. Beat-to-beat CrCP and RAP values were extracted by linear regression analysis of instantaneous arterial blood pressure (ABP) and CBFV tracings. Gosling's pulsatilit

Arterial distensibility in children and teenagers: normal evolution and the effect of childhood vasculitis.

BACKGROUND: Polyarteritis nodosa is a necrotising vasculitis of the medium sized and small muscular arteries. The inflammatory and subsequent reparative processes may alter the arterial mechanical properties. The effect of vasculitic damage on arterial distensibility has never been explored however. AIM: To determine the normal values and the effect of childhood vasculitis on arterial distensibility in children and teenagers. METHODS: Distensibility of the brachioradial arterial segment was studied using pulse wave velocity (PWV proportional, variant 1/ radical distensibility), in 13 childre

Detection of sub-critical arterial stenoses by hyperaemic Doppler.

OBJECTIVES: This study assessed the potential of hyperaemic Doppler to detect sub-critical stenoses using a flowrig model. METHODS: Pulsatile flow of a blood substitute was produced in a compliant circuit. A cadaver carotid artery, constricted by a silk suture produced a variable, focal stenosis. Forty-seven stenoses were created in five arteries. Pressure gradients and Doppler measurements were recorded simultaneously across each stenosis at low (200 ml/min) and high (400 ml/min) flow rates. The change in peak velocities between the arterial segment 2cm proximal to the stenosis (V1), and the…

Transcranial Doppler sonography in craniosynostosis.

In the management of craniosynostosis, there is a need for quantitative assessment of treatment methods and outcome. Radiology and pressure studies are well documented, but so far little attention has been given to cerebral blood flow changes and their possible relevance. This paper reports our initial experience using transcranial Doppler sonography to calculate the cerebrovascular resistance and cerebral blood velocities in the major basal vessels before and after surgery for craniosynostosis. Ten patients were studied using the Scimed 2-MHz probe through the temporal and frontal "windows" o

Brain pulsation demonstrated by CSF flow artifacts on FLAIR images might be associated with brain function.

The glymphatic system works to excrete waste products from the brain. If it breaks down, cognitive impairment can occur. The system is mainly driven by arterial and brain pulsation. Brain pulsation is recognized as cerebrospinal fluid (CSF) flow artifacts on fluid-attenuated inversion recovery (FLAIR) imaging. We investigated the factors associated with human brain pulsation. CSF flow artifact grade (score) was defined by comparing the highest intensity in a given region of interest (ROI) to those in reference ROIs of normal white matter in the centrum semiovale on FLAIR imaging. CSF flow scor

Respiratory influence on cerebrospinal fluid flow - a computational study based on long-term intracranial pressure measurements.

Current theories suggest that waste solutes are cleared from the brain via cerebrospinal fluid (CSF) flow, driven by pressure pulsations of possibly both cardiac and respiratory origin. In this study, we explored the importance of respiratory versus cardiac pressure gradients for CSF flow within one of the main conduits of the brain, the cerebral aqueduct. We obtained overnight intracranial pressure measurements from two different locations in 10 idiopathic normal pressure hydrocephalus (iNPH) patients. The resulting pressure gradients were analyzed with respect to cardiac and respiratory freq

Morphological and hemodynamic analysis of mirror posterior communicating artery aneurysms.

BACKGROUND AND PURPOSE: Hemodynamic factors are commonly believed to play an important role in the pathogenesis, progression, and rupture of cerebral aneurysms. In this study, we aimed to identify significant hemodynamic and morphological parameters that discriminate intracranial aneurysm rupture status using 3-dimensional-angiography and computational fluid dynamics technology. MATERIALS AND METHODS: 3D-DSA was performed in 8 patients with mirror posterior communicating artery aneurysms (Pcom-MANs). Each pair was divided into ruptured and unruptured groups. Five morphological and three hemod

Management of an iatrogenic radial artery perforation: a case report.

A 73-year-old female patient underwent transradial coronary angiography with stable angina and signs of significant myocardial ischemia revealed by exercise stress test. After insertion of a 6F radial sheath into the right radial artery and intra-arterial administration of heparin plus verapamil, the hydrophilic guidewire could not be advanced under fluoroscopic guidance. Immediately afterwards, radial angiography was performed, which displayed a radial artery perforation with significant contrast extravasation. The perforated segment was crossed meticulously with the same guidewire after addi

[Post-traumatic aneurysm of the dorsal nasal artery. A rarity following blunt trauma].

We report the case of a 69-year-old male patient who appeared in our clinic with persistent swelling on the right middle nasal bridge of 3 months' standing following blunt trauma. On physical examination the swelling was pulsatile and a flow was identified on color Doppler sonography. MRT of the head demonstrated an aneurysm of the dorsal nasal artery. We treated the aneurysm in an open surgical procedure under local anaesthesia. Due to the superficial position of this artery, open injuries are common after blunt nasal trauma. Some cases of aneurysm of the temporal artery have been described.

Pulse wave encephalopathy: a spectrum hypothesis incorporating Alzheimer's disease, vascular dementia and normal pressure hydrocephalus.

The disorders of Alzheimer's disease, vascular dementia and normal pressure hydrocephalus are all causes of dementia in the elderly population. It is often the case that it is clinically very difficult to tell these diseases apart. All three forms of dementia share the same risk factors, which for the most part are vascular risk factors. This paper proposes that there is an underlying vascular pathophysiology behind these conditions, which is related to the strength of the pulse waves induced in the craniospinal cavity by the arterial vascular tree. It is proposed the manifestation of the deme
