Late-Onset Radial Nerve Palsy After Closed Treatment of a Periprosthetic Humerus Fracture: A Case Report.

CASE: A 58-year-old woman underwent successful reverse total shoulder arthroplasty for a proximal humerus fracture. Three years later, she sustained a periprosthetic humerus fracture after a fall and was treated conservatively. She healed with an abundance of callus and subsequently developed delayed-onset radial nerve palsy at 3 months, despite minimal change in humeral alignment and avoidance of the use of any compressive external braces or splints. CONCLUSIONS: Minimally displaced periprosethetic humerus fractures can lead to delayed radial nerve palsy from callus entrapment, and there sho

Evaluation and management of pediatric proximal humerus fractures.

In the pediatric population, sports participation, falls, and motor vehicle accidents can result in proximal humerus fractures. Because the proximal humeral growth plate is responsible for up to 80% of the growth of the humerus, the remodeling of these fractures in children is tremendous. Most of these injuries can be treated with a sling or hanging arm cast, although older children with decreased remodeling capacity may require surgery. Special considerations should be taken for management of proximal humerus fractures that occur in the context of Little League shoulder, lesser tuerosity avul

A slowly evolving brachial plexus injury following a proximal humeral fracture in a child.

The association of fractures and neurological injuries is well recognized, especially with certain upper limb fractures. Typically, the nerve injury occurs at the time of initial fracture displacement. A case is reported of an unusual combination of fracture and nerve injury in a child, with the extremely rare occurrence of a slowly evolving nerve injury. The potential for delayed nerve injury following a fracture needs to be appreciated to prevent delays in diagnosis and treatment.

A Stepwise Guide to Performing Shoulder Ultrasound: The Acromio-Clavicular Joint, Biceps, Subscapularis, Impingement, Supraspinatus Protocol.

Shoulder pain is a common and painful patient condition. Unfortunately, diagnostic imaging of shoulder pain in the emergency department (ED) is often limited to radiography. While diagnostic for fractures and dislocations, drawbacks of radiography include time delays and non-diagnostic imaging in the case of rotator cuff pathology. While bedside ultrasound has been incorporated into many procedural and diagnostic applications in the ED, its use for musculoskeletal complaints and specifically shoulder pain is infrequent among ED clinicians. The incorporation of shoulder ultrasound in the ED may

Assessing the performance of motorcyclists' impact protectors in simulated ATD knee and shoulder impacts.

OBJECTIVE: Fractures are a common injury among motorcycle riders and can have serious health implications. Impact protection (IP) has been designed to help prevent fractures, yet there are conflicting opinions as to whether this IP does in fact help prevent fractures in real-world crashes. This work aimed to (1) use simulated dummy impacts to examine whether existing types of IP could reduce the force transferred to the underlying bone to below fracture tolerance levels and (2) investigate whether current European Standard (EN 1621-1) test procedures for impact protectors designed for motorcyc

Nonoperative treatment compared with plate fixation of displaced midshaft clavicular fractures. Surgical technique.

BACKGROUND: Recent studies have shown a high prevalence of symptomatic malunion and nonunion after nonoperative treatment of displaced midshaft clavicular fractures. We sought to compare patient-oriented outcome and complication rates following nonoperative treatment and those after plate fixation of displaced midshaft clavicular fractures. METHODS: In a multicenter, prospective clinical trial, 132 patients with a displaced midshaft fracture of the clavicle were randomized (by sealed envelope) to either operative treatment with plate fixation (sixty-seven patients) or nonoperative treatment w

Acromioclavicular and sternoclavicular injuries and clavicular, glenoid, and scapular fractures.

Injuries to the acromioclavicular joint and the sternoclavicular joint and fractures of the clavicle, glenoid, and scapula vary widely in incidence, treatment, and prognosis. The treatment for acromioclavicular joint and clavicle injuries, which are relatively common, has significantly evolved. Controversy exists regarding the ideal treatment of type III acromioclavicular separations, whereas significant research has shown many potential benefits for surgically treating significantly displaced midshaft clavicle fractures that had traditionally been treated nonsurgically. Sternoclavicular injur

Fractured clavicle and Erb's palsy unrelated to birth trauma.

OBJECTIVES: Our purpose was to determine the perinatal factors associated with clavicular fracture or Erb's palsy in neonates and to document the percentage of cases where no risk factors were involved. STUDY DESIGN: We reviewed the medical records of all live-born singleton infants admitted to the newborn nurseries between 1992 and 1995. Mothers and infants with clavicular fracture or Erb's palsy were compared with those without these birth injuries. RESULTS: Of 11,636 neonates, there were 236 (2.03%) with clavicular fracture and 51 (0.44%) with Erb's palsy. Clavicular fracture was signific

Obstetric clavicular fracture: the enigma of normal birth.

OBJECTIVE: To determine the main risk factors involved in neonatal clavicular fracture, the most common injury to the neonate. METHODS: Two hundred fifteen cases of clavicular fracture of 65,091 vaginal deliveries (0.4%) occurring between January 1983 and December 1988 were pair-matched with controls based on mode and date of delivery, race, and maternal age. Incidences, odds ratios, and stratified analysis were used to identify and control for confounding between risk factors. RESULTS: Shoulder dystocia, increasing birth weight, and increasing gestational age were identified as risk factors

Acute traumatic quadrilateral space syndrome following scapula fracture.

The acute tunnel syndrome of the quadrilateral space of Velpeau is a very rare entity in which the axillary nerve and the posterior humeral circumflex artery experience brutal compression in shoulder injuries. We report the case of a burst fracture of the right scapula with great displacement of the lateral border occasioning a total isolated paralysis of the right axillary nerve by axillary nerve compression. The lateral scapula border fracture was reduced and stabilized with neurolysis of the axillary nerve through a posterior approach. The recovery of the axillary nerve occurred in ten week

Incidentally discovered monostotic Paget's disease involving the scapula inducing brachial plexopathy: A diagnostic dilemma.

Paget disease of bone is a chronic skeletal disorder characterized by abnormal bone remodeling. This condition, which predominantly affects older adults, can result in a variety of complications, including bone pain, fractures, and deformities. This case report discusses the incidental discovery of monostotic Paget's disease involving the scapula, leading to brachial plexopathy in a 58-year-old female with a history of colon cancer. The patient had severe bilateral shoulder pain, predominantly on the left, for the past ten years. This pain, exacerbated by overhead activity and limiting daily f

Posterior bone block procedure for posterior shoulder instability.

The posterior bone block procedure is an uncommon surgical procedure used in the treatment of posterior shoulder instability. The purpose of this study is to report the results of the posterior bone block procedure in the treatment of posterior shoulder instability. We retrospectively reviewed 21 shoulders that had undergone a posterior bone block procedure in the treatment of recurrent posterior shoulder instability between 1984 and 2001. Fifteen patients (16 shoulders) had a prior traumatic posterior glenohumeral dislocation and 5 patients (5 shoulders) had a prior traumatic posterior glenoh

Excellent clinical and radiological outcome following locking compression plate fixation of displaced medial clavicle fractures.

BACKGROUND: Treatment of medial clavicle fractures is still controversially discussed in the community of upper extremity surgeons. An increasing number of symptomatic non-unions following conservative treatment of displaced fractures led to the development of various surgical approaches. Aim of this study was to evaluate the clinical and radiological outcome following operative treatment of displaced medial end clavicle fractures. METHODS: Patients who presented with a displaced fracture of the medial clavicle between September 2012 and December 2019 were retrospectively enrolled in this stu

Analysis of occupant kinematics and dynamics in nearside oblique impacts.

OBJECTIVE: The objective of this article is to analyze the kinematics and dynamics of restrained postmortem human surrogates (PMHS) exposed to a nearside oblique impact and the injuries that were found after the tests. METHODS: Three male PMHS of similar age (64 ± 4 years) and anthropometry (weight: 61 ± 9.6 kg; stature: 172 ± 2.7 cm) were exposed to a 30° nearside oblique impact at 34 km/h. The test fixture approximated the seating position of a front seat occupant. A rigid seat was designed to match the pelvic displacement in a vehicle seat. Surrogates were restrained by a 3-point seat belt

Pedicled Pectoralis Major Transfer for Axillary Nerve Palsy and Greater Tuberosity Nonunion: A Case Report.

CASE: A 46-year-old male patient presented with shoulder pain, limited range of motion, and loss of sensation for a year after undergoing hemiarthroplasty for a proximal humerus fracture. Workup demonstrated a greater tuberosity nonunion and deltoid paralysis secondary to upper trunk brachial plexopathy. The pectoralis major (PM) for deltoid transfer successfully restored shoulder function and resolved pain without requiring any subsequent intervention. CONCLUSION: A PM transfer for deltoid and rotator cuff insufficiency restored shoulder stability and improves biomechanics for shoulder abduc
