After several clusters of meningococcal disease arose among recruits to the British Armed Forces, vaccination against A + C meningococcal disease was introduced for military recruits in 1992. From 1993 onwards, no further clusters of group C infection were reported and incidence of meningococcal disease among trained soldiers fell, but there was no significant reduction in the overall incidence of meningococcal disease in recruits.

The lateral arm free flap can be harvested as a fascial flap or fasciocutaneous flap. In this report we describe the use of the lateral arm fascial flap for degloving injuries of the fingers and for skin loss on the dorsum of the hand with exposure of tendons and bones. Concomitant reconstruction of a missing phalanx with a portion of the distal humerus is also described. Multifocal motor neuropathy (MMN) is a newly recognized disorder with a characteristic clinical picture. Loss of heterozygosity (LOH) on the long arm of chromosome 11 had been reported in many types of solid tumors. In this study, we investigated the LOH patterns of chromosome 11 on 52 primary nasopharyngeal carcinomas using 10 microsatellite polymorphic markers. The results revealed that 28 of the 52 cases (53.8%) demonstrated LOH on at least one of the nine 11q microsatellite loci studied. Gas gangrene is a life-threatening emergency. Most cases are caused by clostridial infections, but nonclostridial causes are being increasingly recognized. Nonclostridial gas gangrene is most often due to polymicrobial organisms. Early diagnosis and therapy are required, since the disease may rapidly progress to fatal toxemia. We report a case of gangrenous, atraumatic, nonclostridial myonecrosis of the arm due to Enterobacter cloacae in a nondiabetic patient with neutropenia.

Between 1986 and 1990, 107,397 National Servicemen between the ages of 17 to 18 years were screened on enlistment for Glucose-6-Phosphate Dehydrogenase (G6PD) enzyme activity by fluorescent spot test. The overall prevalence rate was 1.6% with ethnic differences as follows: 1.62% for Chinese, 1.80% for Malay, 0.76% for Indian and 0.68% for other races. The majority had no prior knowledge of their G6PD deficiency status (89.9%), or of any significant haemolytic episode (93.3%). A family is described in which a mother and her two children carry a tandem duplication of the short arm of chromosome 8. Their phenotypes are similar and characterised by distinct facial dysmorphism, small stature and mild mental retardation. This is one of the first cases of direct familial transmission of a partial duplication of an autosomal chromosome segment.

The double armed suture (DAS) method of repairing flexor and extensor tendons allows almost immediate mobilization of the involved digits. The technique protects the tendon juncture from stress during muscle contraction and hand therapy, which is begun a few days after surgery. This technique has been used in 76 patients since 1972. Good or excellent results have been obtained in 90% of the cases over a 16-year period.

The author presents a rare instance of extra-articular chondromatosis located around the tendon of the long head of the biceps brachii. The tendon subsequently tore and retracted, resulting in the formation of numerous cartilaginous loose bodies which collected in a mid-brachial cavity communicating with the canal which had previously housed the tendon. The author comments on the peculiar mechanism of production of this rare case of synovial chondromatosis as well as possible interpretations.

We have analysed the sequence organization of the DNA in the pericentric region of the long arm of the human Y chromosome. The structures of one cosmid and three yeast artificial chromosome clones were determined. The region consists of a mosaic of the known 5, 48 and 68 base-pair tandemly repeated sequences and at least five novel repeated sequence families. A long range-map of approximately 3.5 x 10(6) base-pairs of genomic DNA was constructed that placed the clones between about 500 x 10(3) and 850 x 10(3) base-pairs from the long arm edge of the centromeric alphoid DNA array.

Controversy still exists over differences between upper and lower limb blood pressures (BP) in neonates. We measured upper arm and calf systolic (S) and diastolic (D) BP and heart rate (HR) using 2 Dinamap 847 instruments simultaneously every half minute for several measurement periods of 5-10 min. A review of the incidence of heat disorders in the Singapore Armed Forces was done in relation to the preventive and corrective measures applied. Data was compiled and analysed from records kept by the SAF Medical Services. A thorough review was made into all the preventive measures carried out thus far and the effectiveness was evaluated. Dissections in cadavers injected with latex confirm that direct septocutaneous branches from the brachial artery are the main source of irrigation for the fascial plexus of the inner arm. We report on the results of 40 digital subtraction phlebograms of the great veins of the upper extremities and the mediastinum in 32 patients. In all cases diagnostic satisfactory examinations were achieved that demonstrated the anatomy of the veins in question. The method can be either used for the primary diagnosis or for controls during lysis of thrombotic lesions. The v. cava superior could be opacified even after ipsilateral injection of diluted contrast media in one-sided thrombotic occlusions of these veins.
