Effects of a nutraceutical combination in patients with chronic lumbosacral radicular pain.

BACKGROUND: Despite advancements in fixation techniques, S1 pedicle screw loosening remains a common complication of lumbosacral fusion surgeries for degenerative lumbar conditions, with reported rates ranging from 15.6% to 41.9%. This complication can compromise fusion success, leading to nonunion, adjacent segment disease, and revision surgeries. Compared with other surgical challenges, less is known about the incidence and predictors of S1 pedicle screw loosening. Given the high prevalence of S1 screw loosening and its associated complications, this systematic review and meta-analysis aim t

Augmentation of (pedicle) screws with calcium apatite cement in patients with severe progressive osteoporotic spinal deformities: an innovative technique.

Screw augmentation with calcium apatite cement (CAC) was used in seven patients with a progressive osteoporotic spinal deformity. Thirty-nine spinal segments (64 screws) were augmented: 15 anteriorly (three patients) and 24 posteriorly (five patients). Dorsally, hemilaminectomy was performed at the level of all augmented screws to rule out CAC leakage. Autogenous bone graft was applied in all patients to induce fusion. Screw augmentation failure occurred in only one patient: 1 of the 16 ventral augmented screws (5.5%) was still loose after the augmentation procedure. In three other patients, 4

Development of a large animal rabbit model for chronic periprosthetic joint infection.

AIMS: Periprosthetic joint infections (PJIs) and osteomyelitis are clinical challenges that are difficult to eradicate. Well-characterized large animal models necessary for testing and validating new treatment strategies for these conditions are lacking. The purpose of this study was to develop a rabbit model of chronic PJI in the distal femur. METHODS: Fresh suspensions of Staphylococcus aureus (ATCC 25923) were prepared in phosphate-buffered saline (PBS) (1 × 109 colony-forming units (CFUs)/ml). Periprosthetic osteomyelitis in female New Zealand white rabbits was induced by intraosseous inj

Changes in acetabular component alignment due to screw fixation in patients with hip dysplasia.

INTRODUCTION: Adequate initial stability of the acetabular cup is essential for total hip arthroplasty (THA). However, changes in the alignment of the acetabular component caused by screw fixation are concerning in patients with inadequate bone stock. This study aimed to investigate the effects of screw fixation on the alignment of the acetabular component in THA patients with hip dysplasia. METHODS: We retrospectively examined 256 hips (range 28-87 years) that underwent THA using a navigation system. Patients were divided into 2 groups based on the presence or absence of changes in the align

Cellulitis related to bioabsorbable plate and screws in infraorbital rim fracture.

An absorbable plating system is most commonly used to fix the bones in craniomaxillofacial surgery. Many studies focused on the advantages and clinical utility of absorbable plate and screws, but there are no reports on the complications related to these devices. From March 2004 to March 2009, 430 patients with facial bone fractures including infraorbital rim fractures underwent craniomaxillofacial surgery, in which rigid fixation was achieved with an absorbable plate and screws (Stryker, Freiburg, Germany). Five men with a delayed inflammatory reaction and infection at infraorbital rim fractu

Suspected Hypersensitivity Reaction to Titanium Screws Following Kalish Osteotomy for Hallux Valgus.

Titanium implants are generally regarded as biocompatible and inert; however, hypersensitivity reactions, although uncommon, have been increasingly recognized in orthopedic surgery. We report a case of chronic inflammatory reaction following a Kalish osteotomy for hallux valgus, ultimately attributed to titanium hypersensitivity. A 58-year-old woman underwent fixation of the first metatarsal with two titanium screws. The postoperative course was complicated by wound dehiscence, delayed healing, recurrent granulomatous tissue, and persistent swelling, despite antibiotic therapy. Over the months

Orthognathic-Like Orthodontics: Management of Skeletal Class II Malocclusion in an Adult Patient.

Advances in orthodontic treatment, particularly with the use of temporary anchorage devices (TADs), have significantly improved outcomes for adult patients with skeletal Class II malocclusion. Traditionally reliant on orthognathic surgery, these malocclusions can now benefit from non-surgical options like maxillary molar distalization. Bone screws offer superior anchorage compared to conventional methods, enabling precise tooth movement without undesirable side effects. This case report explores the efficacy of TAD-assisted maxillary molar distalization in an adult patient while focusing on a …

Conservative Maxillary Sinus Floor Elevation via a Crestal Approach Using Vertical Expander Screw Technique (Technical Note).

OBJECTIVE: Contemporary implantology is moving toward less invasive approaches. To date, several techniques have been proposed for maxillary sinus lift surgery. In this article, we introduce a new technique for maxillary sinus floor elevation that is less invasive than the osteotome technique. METHODS AND MATERIALS: In the vertical expander screw (VES) technique, which uses a combination of drilling and bone-expander screws; a primary implant hole is created. Bone expander screws are used in lateral and vertical dimensions to elevate the sinus membrane. RESULTS: The results at the two-year f

Replacement of deficient acetabulum using Burch-Schneider cages. 22 patients followed for 2-10 years.

The absence of good bone stock with massive acetabular deficiency has been a major problem in both revision hip arthroplasty and in primary arthroplasty for congenital dislocation of the hip (CDH). 22 patients (24 hips; 21 revision and 3 primary replacements: mean age 58 years) with substantial bone loss underwent acetabular reinforcement with Burch-Schneider cages. Classification of acetabular deficiency was made according to the AAOS system. The surgical procedure involved preparation of acetabulum, filling the defect with bone autografts, placement of the cage with its flanges, fixation wit

Removal of a Fixation Screw That Was Forced Into the Postsuperior Maxillary Sinus Wall: Case Report.

We have encountered a rare case in which the subject underwent maxillary sinus floor elevation at another hospital, and a screw to fix the grafted bone substitute was forced into the maxillary sinus and intruded into the bone. Various different foreign bodies have been reported as being forced into the maxillary sinus due to dental treatment, and these foreign bodies are often retained on the maxillary sinus mucous membrane. However, no reports have described a screw forced in and intruded into the peculiar position in the bone, as seen in the present case, which we report here with additional

A locally modified phantom model for assessing the worst-case heating configuration of orthopedic implants under MRI.

This paper proposes a locally modified phantom model to numerically assess the worst-case configuration of orthopedic implants under magnetic resonance imaging (MRI). The proposed model is developed based on the standard American Society for Testing and Materials (ASTM) phantom and bone models with cancellous or cortical materials. Three orthopedic implant families, metallic rods, a nail and screw system, and a plate and screw system, are studied. The worst-case configurations of orthopedic implants are identified inside the proposed model and ASTM phantom. These worst-case heating configurati

Correlation of chronological age and bone age with the correction of ankle valgus by surface epiphysiodesis of the distal medial tibial physis.

A review of the results of medial distal tibial surface epiphysiodesis to correct valgus deformity in the growing child indicates that the bone age can be used to predict correction. When performed at the appropriate bone age, it may have some advantage over the use of staples or screws.

Distal Metaphyseal Ulnar Shortening Osteotomy Fixation: A Biomechanical Analysis.

PURPOSE: Ulnar shortening osteotomy can be used to treat ulnar impaction syndrome and other causes of ulnar wrist pain. Distal metaphyseal ulnar shortening osteotomy (DMUSO) is one technique that has been proposed to reduce the complications seen with a diaphyseal USO or a wafer resection. However, to our knowledge, the optimal fixation construct for DMUSO has not been studied. We sought to characterize the biomechanical stiffness and rotational stability of different DMUSO constructs. METHODS: A DMUSO was performed on 40 human cadaveric ulnas using 4 different fixation constructs (10 specime

Crossed-screw fixation of the unstable thoracic and lumbar spine.

An ideal spinal construct should immobilize only the unstable spinal segments, and thus only the segments fused. Pedicle fixation techniques have provided operative stabilization with the instrumentation of a minimal number of spinal segments; however, some failures have been observed with pedicle instrumentation. These failures are primarily related to excessive preload forces and limitations caused by the size and orientation of the pedicles. To circumvent these problems, a new technique, the crossed-screw fixation method, was developed and is described in this report. This technique facilit
