Background Pediatric patients with atrioventricular valve disease have limited options for prosthetic valve replacement in sizes <15 mm. Based on successful experience with the stented bovine jugular vein graft (Melody valve) in the right ventricular outflow tract, the prosthesis has been modified for surgical valve replacement in pediatric patients with atrioventricular dysfunction with the intention of subsequent valve expansion in the catheterization laboratory as the child grows. Methods and Results A multicenter, retrospective cohort study was performed among patients who underwent atriov

BACKGROUND: From May 1994 through October 2000, a total of 1,146 patients underwent valve replacement with the ATS Medical mechanical cardiac valve prosthesis under a study protocol approved by international ethics committees (non-United States participants) or under a United States Food and Drug Administration-approved Investigational Device Exemption study. The study took place at 19 domestic and three international centers.
METHODS: As required by the Food and Drug Administration's Heart Valve Guidance Document, only isolated implants were included in the study (double-valve implants were

Acute prosthetic valve dysfunction is a critical condition for any patient, and is associated with a high mortality. A 24-year-old man who had undergone mitral valve replacement with a TRI bileaflet valve four months previously at another center was admitted with acute-onset left ventricular failure. Echocardiography showed massive mitral insufficiency which was suggestive of a stuck valve. Emergency surgery was carried out, at which the cranial leaflet was found to be stuck open. There was no tissue impingement and thrombosis, the caudal leaflet was absent, and there were no signs of endocard

BACKGROUND: The clinical effects of prosthesis-patient mismatch (PPM) after aortic valve replacement, with respect to morbidity and survival, remain controversial, particularly in high-risk patient subgroups.
METHODS: Patients undergoing aortic valve replacement from January 1992 to December 2010 were classified according to effective orifice area index into severe PPM (effective orifice area index<0.65 cm²/m²), moderate PPM (effective orifice area index 0.65 to 0.85 cm²/m²), and absent PPM (effective orifice area index>0.85 cm²/m²). Analyses examined major morbidity and total all-cause death

The transcatheter aortic valve replacement procedure is used in patients with aortic stenosis. Transcatheter aortic valve replacement devices are quite versatile; thus, they are increasingly being used for nonaortic applications, such as tricuspid valve-in-valve implantation. This case series describes a transcatheter aortic valve replacement procedure in 4 patients with anatomic challenges (eg, aortic tortuosity, high valvular calcium burden, highly calcified bicuspid valve, low coronary artery takeoff, left main coronary artery occlusion, and large aortic annulus) and a fifth patient who had

OBJECTIVE: To describe outcomes of patients who underwent transcatheter aortic valve replacement (TAVR) in a native valve or failed bioprosthetic valve or transcatheter heart valve (THV) and a transcatheter mitral valve replacement procedure (TMVR; valve-in-valve [mViV], valve-in-ring [mViR], and valve in mitral annulus calcification [ViMAC]) either concomitantly (same procedure) or staged (different procedures).
BACKGROUND: Patient characteristics, procedural details, and outcomes of concomitant or staged TAVR and TMVR procedures are largely unknown.
METHODS: Data were extracted from the ST

Bioprosthetic valve dysfunction (BVD) and bioprosthetic valve failure (BVF) may be caused by structural or nonstructural valve dysfunction. Both surgical and transcatheter bioprosthetic valves have limited durability because of structural valve deterioration. The main objective of this summary of experts participating in a virtual workshop was to propose standardized definitions for nonstructural and structural BVD and BVF following aortic or mitral biological valve replacement with the goal of facilitating research reporting and implementation of these terms in clinical practice. BACKGROUND: Aortic root abscess (ARA) is a major complication of infective endocarditis that is associated with increased morbidity and mortality. Limited data are present about patient characteristics and outcomes in this lethal disease. We aimed to study the clinical and echocardiographic characteristics of patients with ARA compared to patients with left-sided infective endocarditis without ARA. We included patients with a definite diagnosis of left-sided infective endocarditis according to modified Duke's criteria. The patients were classified into two groups according to the presence of A

OBJECTIVES: Structural valve deterioration, mainly caused by calcification, is the Achilles heel of the bioprosthetic heart valves. While clinical long-term experience of surgical aortic valve (SAV) bioprostheses exists, transcatheter heart valve (THV) requires further investigations into biological degeneration. Therefore, this in vitro study aimed to analyse the calcification pattern of different SAV and THV models under standardized conditions.
METHODS: SAVs (Perimount Magna Ease, Perimount, Mosaic Ultra and Hancock II) and THVs (Evolut PRO and SAPIEN XT) models were calcified with a calci

BACKGROUND: A range of non-contact injuries such as anterior cruciate ligament tear, and patellofemoral pain syndrome are caused by disordered knee joint loading from excessive dynamic knee valgus (DKV). Previous systematic reviews showed that DKV could be modified through the influence of hip strength and ankle range of motion. Therefore, the purpose of this systematic review was to examine the effects of exercise intervention which involved either top-down or bottom-up kinetic chains on minimizing DKV in male and female adults and adolescents, with and without existing knee pain.

Pulmonary valve replacement and right ventricular outflow tract reconstruction with valved conduits have been the shortcomings of paediatric cardiac surgeons in the treatment of CHD. In recent decades, encouraging achievements have been made in right ventricular outflow tract technology. Since Klinner reported the first right ventricle-to-pulmonary artery connection using unvalved conduits made of autologous pericardium in 1964, various right ventricle-to-pulmonary artery conduits have gradually been used in the treatment of various complex CHD. Compared with other materials, valved homograft

A 52-year-old male was admitted to the hospital in congestive heart failure. Mitral stenosis as well as an aortic valve prosthesis were the underlying cardiac abnormalities. A Swan-Ganz catheter was inserted and various vasodilators were administered. Phentolamine, nitrates, nitroprusside, and hydralazine all produced an unexpected and at times an alarming increase in the pulmonary artery pressure. The explanation of this paradoxical hemodynamic response is unknown but demonstrates the importance of hemodynamic monitoring with vasodilator therapy

The growing population of patients expected to outlive their index aortic valve replacement (AVR) demands a fundamental shift in treatment planning to one that treats the first valve as the opening commitment in a lifetime management sequence of aortic stenosis. In this review, we propose a longitudinal framework for AVR selection that integrates age, anticipated survival, valve durability, coronary access preservation, and prosthesis-patient mismatch into a unified approach-one that evaluates the index procedure not in isolation, but by the downstream options it preserves or forecloses.

Robotic cardiac surgery continues to evolve, offering patients a less invasive alternative to traditional median sternotomy, even for complex procedures. This report presents a case of robotic totally endoscopic aortic valve replacement using a rapid deployment valve combined with mitral valve repair, demonstrating the feasibility and safety of this approach.
