[The role of aldosterone in hypertension].

Hyperaldosteronism is associated with elevated cardiovascular risk. Using mineralocorticoid receptor antagonists a significant reduction in mortality was archived in patients with heart failure. In addition, in refractory hypertension and in patients with metabolic syndrome aldosterone seems to play an important role. Therapy with mineralocorticoidreceptor (MR) antagonists is feasible when aldosterone levels are elevated, in particular in patients with aldosterone-escape. Of particular interest is primary aldosteronism (PA). PA is one of the major causes of secondary hypertension. Since most p…

Case of Bartter syndrome presenting with hypokalemic periodic paralysis.

Hypokalemic periodic paralysis can occur secondarily to excessive potassium loss. Thyrotoxicosis, diuretic ingestions, hyperaldosteronism, barium poisoning, Gitelman syndrome, and Bartter syndrome are among the disorders causing secondary hypokalemic periodic paralysis. Clinical presentation of Bartter syndrome with hypokalemic periodic paralysis is rare. A 12-year-old boy was admitted to our hospital because of transient paralysis. He had been suffering from transient weakness attacks for 2 years and had had a total of 10 attacks, lasting 1 to 3 days. He had growth retardation, polyuria, and …

Adrenal gland inclusions in patients treated with aldosterone antagonists (Spironolactone/Eplerenone): incidence, morphology, and ultrastructural findings.

BACKGROUND: Spironolactone is often used to treat hypertension caused by hyperaldosteronism, and as a result, can form concentrically laminated electron dense spironolactone body inclusions within the adrenal gland. Spironolactone bodies have not been investigated in a contemporary cohort or in patients treated with the more recently approved aldosterone antagonist, eplerenone. METHODS: Spironolactone bodies were retrospectively investigated in patients treated for hyperaldosteronism (n=15) from 2012-2013 that underwent a subsequent adrenalectomy. RESULTS: Inclusions were identified in 33% o…

Urinary aldosterone to creatinine ratio in cats before and after suppression with salt or fludrocortisone acetate.

BACKGROUND: The endocrine diagnosis of primary hyperaldosteronism in cats currently is based on an increased plasma aldosterone to renin ratio, which has several disadvantages for use in veterinary practice. OBJECTIVES: To establish a reference range for the urinary aldosterone to creatinine ratio (UACR) and to determine whether oral administration of either sodium chloride or fludrocortisone acetate is effective for use in a suppression test. ANIMALS: Forty-two healthy cats from an animal shelter and 1 cat with primary hyperaldosteronism from a veterinary teaching hospital. METHODS: Mornin…

Primary Hyper-aldosteronism in pregnancy: a case report with review of the literature.

OBJECTIVES: Primary hyperaldosteronism can present during pregnancy. Due to its rarity and to the physiological changes occurring during pregnancy, diagnosis might be delayed with potential complications imposed on the mother and newborn. CASE PRESENTATION: We report a 46-years-old middle eastern patient with chronic hypertension, who conceived with assisted reproductive technologies and was diagnosed with PA. She was referred back to cardiology for re-evaluation of her resistant hypertension. Serum electrolytes showed low potassium, high aldosterone, and high renin to aldosterone ratio. Magn…

Malignant hypertension due to a large reninoma: a case report.

A thirty-year-old-man was admitted due to visual loss from malignant hypertension. Hypokalemia and urinary potassium loss were found. Plasma renin activity (PRA) and aldosterone were investigated and found to be elevated compatible with secondary hyperaldosteronism. A computed tomography of the abdomen showed a 11.7 x 11.3 x 12 cm ill-defined, nonhomogeneous mass at the middle part of right kidney. The preoperative diagnosis was renal cell carcinoma and the patient underwent right radical nephrectomy. Following nephrectomy, plasma PRA and plasma aldosterone levels declined and serum potassium …

[Conn's syndrome].

The primary hyperaldosteronism (PHA) is the most prevalent form (8-10%) of secondary causes of hypertension among hypertensive subjects. This will result in 2 to 2.5 million people suffering from PHA in Germany. Screening for PHA should be performed by measuring the aldosterone-renin ratio in the morning. The current antihypertensive medication should be taken into account. A suspicious ratio must lead to a confirmatory test to validate the diagnosis of PHA. For further PHA subtype investigation adrenal imaging is performed using CT or MRI. Adrenal venous sampling is often required to definiti…

Inhibition of furosemide-induced kaliuresis in the rat by trilostane, an inhibitor of adrenal steroidogenesis.

In rats treated with furosemide, urinary losses of water, sodium and potassium were accompanied by increased circulating levels of aldosterone. Trilostane, an inhibitor of adrenal 3 beta-hydroxysteroid dehydrogenase activity, prevented furosemide-induced hyperaldosteronism which resulted in a partial inhibition of diuretic-induced kaliuresis without a change in sodium and water excretion. Spironolactone, an antagonist of mineralocorticoid action with inherent diuretic activity, produced qualitatively similar effects to those of trilostane on urinary electrolyte excretion in furosemide-treated …

Use of computed tomography in diagnosing the cause of primary aldosteronism.

We performed computed tomography (CT) in 22 consecutive patients with primary aldosteronism to evaluate the usefulness of this technique in diagnosing and locating aldosterone-producing adenomas. Sixteen patients had severe hypokalemia, hyperaldosteronism, and elevated plasma levels of 18-hydroxycorticosterone suggestive of an adenoma. In 12 of these 16, a unilateral adrenal mass was demonstrated clearly, and in all 11 who had surgery in adenoma was confirmed. In the other four patients in this group, one adrenal gland was normal and the other was either not seen adequately or had minor abnorm…

Early-onset hypertension associated with a CACNA1H variant of uncertain significance: a case report and literature review.

BACKGROUND: The prevalence of early-onset hypertension is rising annually and is accompanied by progressive target organ damage, contributing to a higher risk of cardiovascular mortality. In patients with early-onset hypertension characterized by refractory hypertension, elevated plasma aldosterone levels, and a family history of hypertension, monogenic hereditary hypertension, such as familial hyperaldosteronism, should be suspected, although this condition is rare. CASE PRESENTATION: A 36-year-old male patient with hypertension fails to achieve target blood pressure despite receiving four an…

Subtype prediction in primary aldosteronism: measurement of circadian variation of adrenocortical hormones and 24-h urinary aldosterone.

OBJECTIVE: Currently, adrenal venous sampling (AVS) is the only reliable method to distinguish unilateral from bilateral hyperaldosteronism in primary aldosteronism (PA). However, AVS is costly and time-consuming compared with simple blood tests. In this study, we conducted a retrospective study to determine whether circadian variation in plasma adrenocortical hormone levels (i.e. aldosterone, cortisol and ACTH) and a 24-h urinary aldosterone could contribute to the clinical differentiation between unilateral hyperaldosteronism (UHA) and bilateral hyperaldosteronism (BHA). DESIGN: In 64 patie…

Eplerenone as a treatment for resistant hypertension in pregnancy.

Mineralocorticoid receptor antagonists are highly effective in the management of resistant hypertension and primary hyperaldosteronism. Recent studies demonstrate that mineralocorticoid receptor antagonists significantly reduce blood pressure, severity of obstructive sleep apnoea and arterial stiffness in patients with resistant hypertension and moderate-severe obstructive sleep apnoea. Eplerenone is a selective mineralocorticoid receptor antagonist that does not act as an androgen receptor blocker, thus reducing the risk of fetal anti-androgenic effects. Rat and rabbit studies demonstrated th…
