Neglected trans-scaphoid trans-styloid volar dislocation of the lunate. Late result following open reduction and K-wire fixation.

A rare case of radiocarpal dislocation is presented. The lunate and proximal pole of the scaphoid were displaced in a volar and proximal direction. The injury was missed initially and the patient was subsequently operated on six weeks later. Open reduction and internal fixation of the scaphoid was performed and this was followed by an uneventful postoperative period, with a satisfactory functional outcome at the eight-year follow-up, despite carpal instability non-dissociative-dorsal intercalated segmental instability configuration of the carpus. We believe that although open reduction in negl

[Computerized tomography as a diagnostic complement of standard radiograms in the evaluation of traumatic lesions of the carpal scaphoid. A study of 10 cases].

The authors investigated the value of direct coronal CT as a diagnostic complement to plain radiography in the assessment of post-traumatic scaphoid conditions. The series included 10 patients who were selected either for disagreement of clinical and radiological findings, the latter being negative (3 patients), or for better assessment of radiological findings, which had diagnosed bone lesions, in order to plan treatment (7 cases). CT provided additional information in 5 patients, demonstrating 2 nonunited fractures in the first group and 2 pseudoarthroses with no significant bone deformity a

Functional outcome after percutaneous screw fixation of acute scaphoid fracture.

Scaphoid fractures are common in adults and are prone to complications such as nonunion and avascular necrosis due to the bone's retrograde blood supply. Hence, this study evaluated the prevalence of excellent wrist functional outcomes following percutaneous Herbert screw fixation for acute scaphoid fractures in a tertiary care hospital. A total of 67 patients were assessed using the Mayo wrist score four weeks postoperatively. Most patients were young males with high postoperative functional scores (mean 94.48 ± 9.42), indicating favorable recovery. Percutaneous fixation with a Herbert screw

Percutaneous management of scaphoid nonunions.

Scaphoid nonunions pose a formidable challenge to surgeons because of the multiple factors that may contribute to their causation. The etiology of the nonunion may be because of anatomic variations, fracture configuration, vascular problems, underlying metabolic problems, or the inadequacy of initial treatment. Percutaneous management of scaphoid nonunions offers the advantage of inducing minimal trauma to the soft tissues while adequately stabilizing the fracture site to induce union in a high percentage of cases. These minimally invasive techniques of fixation can also be combined with arthr

[The Herbert screw in pseudoarthrosis of the carpal scaphoid].

For nearly three years, the authors have ensured scaphoid union by fixation with a double thread T.J. Herbert's screw. In these 25 cases a cancellous bone graft was used to fill the defect which would have left shortening of the scaphoid Twenty-two of the 25 cases, obtained rapid union, and 20 had a good or very good clinical result. One doubtful union and two cases of non-union occurred. One of these non-unions required revision. The main advantage of this procedure is to allow early mobilization of the wrist. This early is very appreciated by the patients who were immobilized several months

Radiological and anatomical study to investigate possibility of relationship between scaphoid fracture and lunate morphology.

BACKGROUND: A connection between lunate type has been described for medical conditions like Kienbock's disease as well as for wrist osteoarthritis, e.g. STT osteoarthritis. Aim of the Study to investigate the possibility of a relationship between lunate type and the type of wrist injury mainly localised to the scaphoid bone. MATERIAL AND METHODS: 394 PA and lateral radiographs of the wrist (n = 394) were subjected to a detailed assessment by two independent surgeons. The X-rays were selected and analyzed from a computer and archive database run by St. Hedwig Hospital in Trzebnica, from the be…

Comparison of two percutaneous volar approaches for screw fixation of scaphoid waist fractures: radiographic and biomechanical study of an osteotomy-simulated model.

BACKGROUND: When a surgeon uses a percutaneous volar approach to treat scaphoid waist fractures, central screw placement is complicated by the shape of the scaphoid and by obstruction by the trapezium. In this study, we used radiographs and biomechanical tests to compare the standard volar percutaneous approach with the transtrapezial approach, with regard to central screw placement at the distal pole of the scaphoid. METHODS: Fourteen matched pairs of cadaveric wrists were randomly assigned to two treatment groups. Under fluoroscopic control, a guidewire was drilled into the scaphoid, either

Screw fixation of scaphoid fractures: a biomechanical assessment of screw length and screw augmentation.

PURPOSE: To assess the biomechanical stability relative to screw length and K-wire augmentation in scaphoid fracture fixation using a flexibility testing protocol and cadaver scaphoids whose soft tissue attachments remained undisturbed. Our hypothesis was 2-fold: increasing screw length and augmenting fixation with a K-wire would improve fracture fragment stability, individually and in combination. METHODS: Flexion and extension loading applied through wrist tendons was performed on 10 cadaveric wrists after volar wedge scaphoid osteotomy and internal fixation. Each wrist participated in 3 ex

[Fresh fractures! Classification, management, results and problems: the wrist joint].

Carpal bone fractures are rare, with an incidence of 13% compared to that of fractures and luxations of the hand bones. Generally they are intraarticular fractures. Fracture of the scaphoid bone occurs most frequently, with an incidence of 75%. The greater number of fractures is found in the middle-third of the wrist bordering on the os lunatum. They are usually treated conservatively by splinting with a cast. Surgery is indicated only when the fracture is open, instable, dislocated or combined with carpal instabilities.

Technical factors related to Herbert screw fixation.

We reviewed 24 patients treated for an acute fracture or a nonunion of the scaphoid bone using the Herbert screw. Mean follow-up in 22 patients who returned for examination was 17 months. The overall union rate was 67% for both acute fractures and nonunions. Fracture healing correlated strongly with technical factors of the procedure. The fracture failed to heal in seven of nine cases with poor scaphoid realignment, inaccurate jig placement, or improper screw length for a nonunion rate of 78%. Conversely, without these technical problems, 14 (93%) of 16 fractures achieved union. Applying Herbe…

A finite element analysis of two novel screw designs for scaphoid waist fractures.

The scaphoid is the most often fractured carpal bone. Scaphoid fracture repair with a headless compression screw allows for early functional recovery. The rotational stability of a single screw may be limited, having a potential negative impact on the healing process. Two novel screws have been designed to provide improved rotational stability compared to the existing ones. Using a computational finite element model of a scaphoid osteotomy, we compared the efficacy of one simple screw and the two new screws in restricting inter-fragmentary motion (IFM) in three functional positions of the wris

The role of carpal instability in scaphoid nonunion: casual or causal?

We studied twenty consecutive scaphoid nonunions in twenty patients between the ages of eighteen and thirty-eight years. There were nineteen males and one female. The mean age was 25.2 years. Factors evaluated were fracture displacement, delay in treatment, and carpal instability. Fracture displacement and carpal instability were documented in patients by abnormal x-rays showing fragment displacement, abnormal scapholunate and radiolunate angles, etc., or by fluoroscopically controlled arthrography. We concluded that intercarpal ligamentous instability is consistently present and, therefore, t

[Diagnostically Approach to Pediatric Carpal Fractures: a Retrospective Analysis].

INTRODUCTION: Carpal fractures in children are rare, but can be missed, as their clinical symptoms are unspecific and discrete. Even X-ray diagnosis is difficult. Timely diagnosis and consistent therapy are especially important for scaphoid fractures, as they can help to avoid complications such as non-union or avascular necrosis. A diagnostic approach to paediatric carpal fractures will be discussed on the basis of the following group of patients. METHODS: Retrospective analysis of children under 14 years treated in our institution between 09/2010 and 02/2012 for clinically suspected carpal
