The management of a unicameral bone cyst varies from percutaneous needle biopsy, aspiration, and local injection of steroid, autogenous bone marrow, or demineralized bone matrix to the more invasive surgical procedures of conventional curettage and grafting (with autogenous or allogenous bone) or subtotal resection with bone grafting. The best treatment for a unicameral bone cyst is yet to be identified. Better understanding of the pathology will change the concept of management. The aim of treatment is to prevent pathologic fracture, to promote cyst healing, and to avoid cyst recurrence and r

A one-year-old, 48 kg, male neutered, Great Dane presented for bilateral thoracic limb lameness ascribed to an apparent developmental valgus deformity of both manus. Corrective radial-shaped osteotomies of the four weight-bearing metacarpal bones (II-V) were virtually planned based on CT scan data, and bone models of each corrected manus were three-dimensionally (3D)-printed, sterilized, and used intra-operatively to facilitate the procedure. Corrective radial-shaped osteotomies of metacarpals II-V were performed on the right and then left manus, nine weeks apart. In the right manus, metacarpa

Osteoporotic fractures in the elderly are increasingly numerous, but diaphyseal locations on native bone are quite rare. Pathological and periprosthetic fractures are not included in this review, as they are specific in terms of context and treatment. Cortical thinning and widening of the medullary canal alter local mechanical properties, necessitating adaptation of internal fixation. Thus, for nailing, the diameter of the implant has to be greater, and fixed-angle or multidirectional locking screws are used; for plate fixation, locking screws are required. To avoid secondary periprosthetic fr

Wrist prostheses have never achieved the sort of clinical outcomes found with those of hips and knees. We have developed a novel uncemented modular wrist prosthesis with screw fixation, metal-on-metal coupling, and ball-and-socket articulation. Eight patients admitted for wrist arthrodesis to treat primary or secondary osteoarthritis (not rheumatoid) were operated on. The prosthesis reduced the amount of bone removed and spared the distal radioulnar joint. After 7 to 9 years we found that the fixed centre of the ball-and-socket articulation provided good stability and mobility, and relief of p

PURPOSE: The advantages of rigid fixation over wire osteosynthesis are well established for the management of facial trauma. Miniplates in the frontozygomatic area are traditionally applied to the lateral face of the orbital rim, but with some undesirable effects, such as palpability, visibility, and risk of penetration into the anterior cranial fossa. The aim of this study was to perform an anatomical study to validate the use of miniplates on the temporal face of the frontozygomatic region.
METHODS: Osseous thickness measurements were performed in 30 skulls, on four points above and four be

OBJECT: Rigid fixation of the upper cervical spine has become an established method of durable stabilization for a variety of craniocervical pathological entities in children. In children, specifically, the use of C1-2 transarticular screws has been proposed in recent literature to be the gold standard configuration for pathology involving these levels. The authors reviewed the use of rigid fixation techniques alternative to C1-2 transarticular screws in children. Factors evaluated included ease of placement, complications, and postoperative stability.
METHODS: Seventeen patients, ranging in

PURPOSE: We performed a biomechanical analysis using the finite element method to assess the effects of plate length and the number of screws on construct stiffness, stress distribution, and fracture displacement in the fixation of type A2 distal humerus fractures.
METHODS: A 3-dimensional humerus model was constructed using computed tomography of a healthy man. After creating a 2-mm extra-articular fracture gap, orthogonal double-plate fixation was performed with an incremental increase in plate length and the number of screws, creating 17 fixation models. Four screws were placed in each pla

STUDY DESIGN: Experimental study.
OBJECTIVE: To evaluate the biomechanical stability of screws placed in expanded pedicles, and to compare those with the screws placed in the nonexpanded pedicles in an in vivo model.
SUMMARY OF BACKGROUND DATA: Pullout strength of a pedicle screw increases as the diameter of the screw gets larger. Small pedicles of pediatric patients prevent the use of large size pedicle screws in this age group. As shown in an in vitro study one option to overcome this problem is to dilate and use larger screw in immature spine. But dilation reduces the pullout strength of

INTRODUCTION: Cement-augmented pedicle screw instrumentation procedures are performed for stabilization in patients with surgical spinal conditions with poor bone quality at the target levels. Although this is done to combat the risk of failure due to loosening, cutting out or pulling out of the screws, failure may still occur. The purpose of this study was to identify the modalities and rates of failure of CAPSI constructs to better predict occurrence of failure.
METHODS: We retrospectively reviewed clinical data of patients who underwent CAPSI fusion procedures between 2010 and 2022. We col

Implant choice is a matter of concern in athletes and active patients who sustain a Jones fracture because they are prone to failure including non-union, screw failure, and refracture. The aim of this study was to compare the biomechanical behavior of a Jones fracture-specific screw (JFXS) with a cannulated headless compression screw (HCS) in a simulated partial weight-bearing and ultimate load Jones fracture fixation model. Ten matched pairs of human anatomical specimens underwent Jones fracture creation and consecutive intramedullary stabilization with a solid JFXS or a cannulated HCS. The b

There is an ongoing debate on which fixation technique should be preferred for the prophylactic fixation of the asymptomatic contralateral hip in slipped capital femoral epiphysis (SCFE). In the case of Kirschner-wire (K-wire) fixation, there is a possibility of secondary loss of fixation because of longitudinal growth of the physis, whereas in screw fixation, physeal growth of the femoral neck might be impaired. The aim of this matched-pair study was to compare the longitudinal growth of the femoral neck in screw fixation versus K-wire fixation of the asymptomatic contralateral hip in SCFE. A

OBJECT: Excellent fixation of an artificial ligament in bone is mandatory for initial stability. ACL reconstruction with the LARS artificial ligament may fail if anchorage to bone is inadequate. The weak metaphyseal bone of the proximal tibia is prone to inadequate fixation. This study evaluates the initial mechanical stability of two techniques with an interference screw on the tibial side of an ACL reconstruction with the LARS ligament.
METHODS: Six left tibias were obtained from 1 to 3 year old mongrel dog weighing 20 to 26 kg. ACL straight line reconstruction according to the technique de

BACKGROUND: Fixation of mandibular subcondylar fractures presents challenges due to limited surgical access and the small size of the proximal fragment; therefore, single-piece and easily applicable three-dimensional (3D) plates may be considered as an alternative. Although titanium is frequently used as a plate material, it has been reported to have certain clinical and imaging-related limitations. In comparison, Carbon Fiber-Reinforced Polyetheretherketone (CFR-PEEK) has emerged as a favorable alternative for mandibular fracture fixation due to its bone-like elastic properties, radiolucency,

Upgradation is still in need for the clinically applied interference screws in anterior cruciate ligament reconstruction for more reliable fixation. Silk fibroin bulk materials offer a promising opportunity for this application except lacking osteoinductivity to some extent. Here we report a novel silk-based bulk material with high content of hydroxyapatite-silk fibroin (HA-SF) hybrid particles, which is prepared via a dual-network hydrogel. This composite bulk material possesses a compression modulus of 3.2 GPa, comparable to that of the natural compact bone, and presents satisfactory cytocom

A Huckstep intramedullary nail was used to treat 23 fractures in which the femur was mechanically deficient due to pathologic fracture, nonunion, or complex fracture in osteoporotic bone. All nailings were performed by open surgery, and in some cases this was augmented by corticocancellous bone grafting. The Huckstep nail allows the use of multiple cross screws at 15-mm intervals, providing immediate and adequate stability with successful early postoperative weightbearing. The results at followup were good in functional and radiologic terms, with no cases of fixation failure or infection. The

Rigid fixation of osteotomy segments is frequently used to reduce relapse and allow for early mobilization of the mandible following the sagittal ramus split osteotomy. This study evaluated cortical bone thickness in the retromolar area of 49 human mandibles to determine if there is an advantage (in terms of cortical thickness) to placement of screws for rigid fixation at the external oblique ridge versus placement at the inferior border. The mandibles were sectioned vertically at three sites in the retromolar area, corresponding to the bone available for rigid fixation of the sagittal osteoto
