INTRODUCTION: Studies suggest that women with ischemic heart disease are less likely to experience appropriate ICD therapies for ventricular arrhythmias (VT/VF). We evaluated the influence of sex on arrhythmic events or death in subjects enrolled in MADIT-CRT.
METHODS AND RESULTS: Arrhythmic event rates, defined as VT/VF treated with defibrillator therapy or all-cause death, were determined among 1,790 subjects enrolled in MADIT-CRT with documented 3-year follow-up. Predictors of VT/VF/death were identified using multivariate analysis. Ninety-one (21%) women and 466 (35%) men experienced VT/V

BACKGROUND: Inspiratory muscle weakness is common among patients with chronic heart failure undergoing cardiac resynchronization therapy (CRT). Home-based inspiratory muscle training (IMT) could be a vital therapeutic strategy, particularly for those with limited access to cardiac rehabilitation programs.
OBJECTIVES: This study aimed to investigate the effects of a 12-week home-based IMT program on inspiratory muscle strength, cardiac function, NT-proBNP levels, exercise capacity, quality of life, and daily activities. It examined the relationships between changes in NT-proBNP levels, walking…

BACKGROUND: Patients with adult congenital heart disease (ACHD) often develop heart failure (HF). Cardiac resynchronization therapy (CRT) may provide benefit, but evidence is limited to observational studies and guidelines are extrapolated from acquired HF.
OBJECTIVE: This study aimed to systematically evaluate the effects of CRT in ACHD, including both biventricular pacing (BiV) and conduction system pacing (CSP) strategies, on electrocardiographic, functional, and clinical outcomes.
METHODS: We conducted a systematic review and meta-analysis following Preferred Reporting Items for Systemat…

Cardiac resynchronization therapy (CRT) via left bundle branch area pacing (LBBAP) has emerged as effective and safe as conventional CRT. Left-sided CRT implantation in patients with persistent left superior vena cava (PLSVC) is challenging and impossible in some patients. Right-sided CRT implantation, either conventional or LBBAP, is also tricky, as the delivery sheaths are feasible for left-sided implantations. Here, we present a patient with PLSVC who underwent successful right-sided CRT implantation via LBBAP.

BACKGROUND: Data comparing outcomes between cardiac resynchronization therapy (CRT) upgrade in patients with left bundle branch block (LBBB) or right ventricular (RV) pacing and de novo CRT in LBBB are limited.
OBJECTIVE: To evaluate long-term outcomes of CRT upgrade in patients with LBBB or RV pacing compared with de novo CRT in LBBB.
METHODS: We analyzed 395 consecutive patients who received CRT, classified into 3 groups: CRT upgrade for LBBB or RV pacing (Upgrade group, n = 111), de novo CRT for LBBB (LBBB group, n = 84), and CRT for non-LBBB (non-LBBB group, n = 200). The primary end poi…

Conventional biventricular (BiV) pacing cardiac resynchronization therapy (CRT) is an established treatment for heart failure patients. Recently, multiple novel CRT delivering technologies such as His-Bundle pacing have been investigated as alternative pacing strategies for optimal treatment benefit. Electromechanical Wave Imaging (EWI), a high frame-rate echocardiography-based modality, is capable of visualizing the change from dyssynchronous activation to resynchronized BiV-paced ventricles in 3D. This proof-of-concept study introduces a new EWI-based dispersion metric to further characteriz…

BACKGROUND: The use of electrocardiography (ECG) is a practical method to evaluate the response to cardiac resynchronization therapy (CRT) implantation, as it is easily performed and saves time.
AIM: This study aimed to assess the predictive value of the T-wave duration and Tpeak-Tend (Tp-e) interval following the CRT implantation administered to heart failure patients.
METHODS: Sixty-seven patients with left ventricular ejection fraction ≤35, New York Heart Association (NYHA) class II-III, ambulatory class IV, normal sinus rhythm, who have complete left bundle branch block on ECG and treate…

BACKGROUND: Cardiac implanted electronic devices (CIED) have significantly improved the survival and quality of life in heart failure patients. Although implantable cardioverter-defibrillators (ICD) and cardiac resynchronization therapy (CRT) have a major role in patients with moderate to severe heart failure symptoms, the role of these devices in patients with a left ventricular assist device (LVAD) is not yet well defined. The burden of CIED-related procedures in patients with an LVAD is high. The price of lead malfunctions and pocket complications requires creative approaches to tackle CIED…

BACKGROUND: Cardiac resynchronization therapy (CRT) uses left ventricular (LV) pacing to restore rapid synchronized LV activation when it is delayed in patients with myocardial disease.
OBJECTIVE: Although intrinsic LV activation delays are understood, little is known about reactions to LV stimulation and whether they are affected by QRS duration (QRSd), morphology, LV substrate, or choice of electrode pair. The purpose of this study was to test these interactions.
METHODS: In 120 heart failure patients with left bundle branch block (LBBB) and QRS >120 ms receiving CRT with quadripolar LV le…

CONTEXT: Clinicians lack reliable methods to predict which patients with congestive heart failure (CHF) will benefit from cardiac resynchronization therapy (CRT). Symptom burden may help to predict response, but this information is buried in free-text clinical notes. Natural language processing (NLP) may identify symptoms recorded in the electronic health record and thereby enable this information to inform clinical decisions about the appropriateness of CRT.
OBJECTIVES: To develop, train, and test a deep NLP model that identifies documented symptoms in patients with CHF receiving CRT.

It is now well recognized that heart failure (HF) patients with left bundle branch block (LBBB) derive substantial clinical benefits from cardiac resynchronization therapy (CRT), and LBBB has become one of the important predictors for CRT response. The conventional tachypacing-induced HF model has several major limitations, including absence of stable LBBB and rapid reversal of left ventricular (LV) dysfunction after cessation of pacing. Hence, it is essential to establish an optimal model of chronic HF with isolated LBBB for studying CRT benefits. In the present study, a canine model of async…

OBJECTIVES: In this study, the authors sought to assess the impact of body and heart size on sex-specific cardiac resynchronization therapy (CRT) response rate, according to QRS duration (QRSd) as a continuum.
BACKGROUND: Effects of CRT differ between sexes for any given QRSd.
METHODS: New York Heart Association functional class III/IV patients with nonischemic cardiomyopathy and "true" left bundle branch block (LBBB) were evaluated. Left ventricular mass (LVM) and end-diastolic volume were measured echocardiographically. Positive response was defined by left ventricular ejection fraction (L…

BACKGROUND: Although association of metabolic syndrome (MS) and ischemic heart disease is strongly established, it is not known whether presence of MS may differently influence clinical responses to cardiac resynchronization therapy (CRT). The aim of this study was to evaluate the associations between obesity and metabolic features and the clinical outcome after cardiac resynchronization with defibrillator therapy (CRT-D), compared to an implantable cardioverter defibrillator (ICD).
METHODS: The risk of heart failure (HF) or death and death alone was evaluated in 829 non-obese patients, 156 o…

Cardiac resynchronization therapy (CRT) is a well-known treatment modality for patients with a reduced left ventricular ejection fraction accompanied by a ventricular conduction delay. However, a large proportion of patients does not benefit from this therapy. Better patient selection may importantly reduce the number of non-responders. Here, we review the strengths and weaknesses of the electrocardiogram (ECG) markers currently being used in guidelines for patient selection, e.g., QRS duration and morphology. We shed light on the current knowledge on the underlying electrical substrate and th…

Heart failure (HF) is one of the biggest epidemics of modern cardiovascular medicine. Cardiac resynchronization therapy (CRT) with biventricular (BiV) pacing has proven to have an integral role in the management of patients with reduced left ventricular (LV) function and left bundle branch blocks (LBBB). However, CRT with BiV pacing is not always feasible and even when it is, the percentage of non-responders remains high. Limitations in LV lead implantation due to anatomical or other constraints; non response to BiV pacing due to lead position or patient related factors and lack of benefit in …

This article describes a 45-year-old lady with longstanding dilated cardiomyopathy, who has progressed to New York Heart Association class III and has failed to respond to optimal pharmacologic therapy. On the basis of gender and the presence of an left bundle branch block, she has good prospects of responding to cardiac resynchronization therapy. In the background of nonischemic cardiomyopathy and the absence of myocardial scarring on cardiovascular magnetic resonance, her annualized risk of sudden cardiac death is low, estimated at 2.6%.

AIMS: It has been suggested that cardiac resynchronization therapy (CRT) is less utilized, dyssynchrony occurs at narrower QRS, and CRT is more beneficial in women compared with men. We tested the hypotheses that (i) CRT is more underutilized and (ii) QRS prolongation and left bundle branch block (LBBB) are more harmful in women.
METHODS AND RESULTS: We studied 14 713 patients (28% women) with left ventricular ejection fraction (LVEF) <40% in the Swedish Heart Failure Registry. In women vs. men, CRT was present in 4 vs. 7% (P < 0.001) and was absent but with indication in 30 vs. 31% (P = 0.82…
