Perimenopause and metabolic vulnerability: hormones, body composition and lifestyle changes.

Perimenopause is a transitional phase characterized by substantial hormonal fluctuations, particularly declining estradiol levels, accompanied by age-related metabolic changes. Increasing evidence suggests that this period constitutes a critical window of metabolic vulnerability, marked by adverse alterations in body composition, energy metabolism and cardiometabolic health. Notably, these changes may develop even without significant increases in body weight, potentially delaying their recognition in clinical settings. A narrative review was conducted using the PubMed/MEDLINE, Scopus, Web of S

Hot flashes: a potential marker of deterioration of health-related quality of life.

OBJECTIVE: Hot flashes are among the most common symptoms of the menopausal transition and have traditionally been considered benign and self-limiting. However, increasing evidence suggests that they may indicate broader neurovascular and inflammatory dysregulation linked to reproductive aging. The possible effect of hot flush severity on health-related quality of life (HRQoL) remains inadequately studied, particularly in Latin American populations. This study aimed to examine the association between hot flash severity and HRQoL in middle-aged women using validated tools and a large, multicent

Global burden of gastroesophageal reflux disease in women of perimenopausal age: trends and forecasts.

OBJECTIVE: Gastroesophageal reflux disease (GERD) is a highly prevalent disorder that diminishes quality of life. Hormonal fluctuations during the perimenopausal transition (age 45-54 years) may heighten susceptibility to GERD, yet the magnitude of the burden in this group remains poorly characterized. This study aims to quantify GERD trends in women of perimenopausal age and forecast disease trajectories to 2045. METHOD: Data from the 2021 Global Burden of Disease (GBD) database were utilized. Age-standardized rates were calculated, annual percentage changes were estimated (EAPC) and Bayesia

What is in the bottle? An analysis of the ingredients in menopause supplements.

OBJECTIVE: Menopause supplements are a global market projected to reach $25 billion by 2030, despite a lack of evidence on their efficacy. This study analyzed the ingredients, doses and costs of menopause supplements in the UK. METHOD: A systematic search of nine UK retailers and Google Shopping was conducted using the terms 'menopause' and 'menopause supplements'. Products with the term 'menopause' in their name or description were included; duplicates, bundles and discontinued items were excluded. Ingredients were categorized as vitamins and minerals, botanicals, and other active and non-ac

The enigma of associations between reproductive health and development of osteoarthritis in midlife: a life-course approach.

Osteoarthritis (OA) is the most common form of arthritis. OA becomes more prevalent in women around the age of menopause, and it has been suggested that the decrease in estrogen at menopause may be important in the pathophysiology of OA development. The association between estrogen exposure and OA is complex; a recent meta-analysis suggested that menopause hormone therapy use may be associated with an increased risk of joint replacement, bringing this research area back into focus. This narrative review used a systematic process, adopting a life-course approach to consider relationships betwee

Female reproductive factors, genetic susceptibility and incident osteoporosis risk.

OBJECTIVE: Osteoporosis risk is shaped by both reproductive factors and genetic predisposition, yet their combined effects remain insufficiently studied. METHOD: A total of 49,885 women from the UK Biobank were analyzed, excluding those with baseline osteoporosis or missing reproductive data. Reproductive factors included age at menarche, menopause time/status, history of stillbirth, hysterectomy, oophorectomy and oral contraceptive (OC) use. Genetic susceptibility was assessed using the polygenic risk score (PRS). Cox models estimated associations, with restricted cubic splines testing non-l

Menopausal hormone therapy and pulmonary diseases.

Menopause is accompanied by loss of ovarian estrogen and progesterone production and by age-related changes in circulating androgens, with implications for respiratory physiology and chronic pulmonary disease. Sex steroids influence lung development, immune responses, airway tone, vascular remodeling and pathways involved in lung carcinogenesis. Epidemiological evidence linking menopausal hormone therapy (MHT) to asthma is heterogeneous; some cohorts show increased incident asthma or exacerbations, particularly with estrogen therapy and in specific subgroups, whereas others suggest preserved l

Mortality trends in US patients with gynecologic cancer and cardiovascular disease.

OBJECTIVES: Female genital organ cancers and cardiovascular disease (CVD) are major contributors to women's morbidity and mortality. Improved cancer survival and increasing cardiometabolic risk may amplify their coexistence, yet population-level mortality patterns remain unclear. METHODS: This study analyzed US national death certificate data (1999-2023) for women aged ≥25 years with both gynecologic cancer (ICD‑10 codes C51-C58) and CVD (ICD‑10 codes I00-I99). Age-adjusted mortality rates (AAMRs) were calculated using the 2000 US standard population. Joinpoint regression estimated the annual

Do menopausal symptoms signal early biological aging? Mitochondrial, endocrine and clinical insights.

The menopausal transition represents a pivotal period in female aging, marked by profound endocrine, metabolic and cellular shifts. Increasing evidence indicates that menopausal symptoms - vasomotor instability, sleep disturbances, fatigue and cognitive complaints - are more than consequences of estrogen withdrawal, and may serve as a potential clinical indicator of biological aging. Experimental and clinical data suggest that declining estrogen signaling contributes to mitochondrial dysfunction, inflammation and telomere attrition, processes that are closely linked to cellular senescence and

Perimenopausal migraine: a narrative review.

Migraine is a highly prevalent primary headache disorder that disproportionately affects women owing to the effects of sex hormones, particularly estrogen. Hyperestrogenism during early perimenopause, coupled with anovulatory cycles, is associated with an increased frequency and severity of migraine attacks and an increased prevalence of perimenstrual migraine. The natural decline in estrogen levels after menopause is generally associated with migraine improvement; however, this is variable and often complicated by chronic migraine, vasomotor symptoms, cardiovascular risk and use of menopause

The role of hormones in female androgenetic alopecia: focus on menopause.

Female androgenetic alopecia (FAGA) is the most common cause of hair loss in women. Even though its prevalence differs between populations, studies show consistent evidence that the incidence increases with age. This can be attributed to hormonal changes during the menopausal transition - a decrease of estrogen levels leading to the state of relative hyperandrogenism. Nonetheless, the pathogenesis of the disease remains unclear, as up-to-date studies have failed to prove the correlation between total and free testosterone concentrations and the prevalence of FAGA. This article provides a liter

Chronic pain at midlife.

Chronic pain disproportionately affects women and reproductive milestones influence its clinical manifestation. Mechanistic pathways underlying pain are determined by numerous biopsychosocial factors, the most important being age, hormonal changes and mental health. Menopause-related changes in estradiol interact bidirectionally with musculoskeletal degeneration, neuro-immune sensitization, neuroendocrine responses and other intrapersonal and socio-cultural vulnerabilities, collectively amplifying pain perception and maintenance. Approximately half of midlife women experience chronic pain ofte
