Leverage application on Gartland type IV supracondylar humeral fracture in children.

PURPOSE: Although the most complex management of Gartland type IV supracondylar humeral fracture (SCHF) due to instability, the gold standard of initial treatment remains closed reduction and percutaneous pinning. However, open reduction was inevitable in most published studies. This study reports the outcome of treatment by leverage-assisted closed reduction. METHOD: Twenty-seven patients were diagnosed as Gartland type IV SCHF during surgery in 214 preoperative Gartland type III fractures. Leverage-assisted reduction with percutaneous lateral pinning was done in these patients after failure

Axial loading injuries to the middle cervical spine segment. An analysis and classification of twenty-five cases.

Injuries to the cervical spine at the C3-C4 level involving the bony elements, intervertebral disks, and ligamentous structures are rare. We present 25 cases of traumatic C3-C4 injuries sustained by young athletes and documented by the National Football Head and Neck Injury Registry. Review of the cases reveals that the response of energy inputs at the C3-C4 level differ from those involving the upper (C1-C2) and lower (C4-C5-C6-C7) cervical segments. Specifically, the C3-C4 lesions appear to be unique with regard to the infrequency of bony fracture, difficulty in effecting and maintaining red

Nonarticular Base and Shaft Fractures of Children's Fingers: Are Follow-up X-rays Needed? Retrospective Study of Conservatively Treated Proximal and Middle Phalangeal Fractures.

BACKGROUND: Phalangeal fractures of the hand are common in children, and most extra-articular fractures can be treated with nonoperative management. Minimally or nondisplaced fractures may simply be immobilized, whereas displaced fractures need closed reduction before immobilization. Although few of these fractures displace secondarily, most schemes currently recommend follow-up x-rays after initial diagnosis. Our primary objective was to identify subgroups of finger fractures that are stable, thus requiring no radiographic monitoring. METHODS: This study was designed as a retrospective, sing

Bilateral avascular necrosis of the femoral head following asynchronous postictal femoral neck fractures: a case report and review of the literature.

Bilateral avascular necrosis (AVN) following postictal bilateral fracture neck of the femur is a rare occurrence. Here, we report a case of bilateral AVN of the femoral head following an asynchronous bilateral postictal fracture neck of the femur. A 16-year-old autistic boy presented with left hip pain following an episode of seizures and radiographs showed Delbet type II fracture neck of the left femur. This was treated by closed reduction and cancellous screw fixation and skeletal traction for 6 weeks. At 3 months, follow-up radiograph showed union of the fracture, but he had developed segme

A historical review of calcaneal fractures: from the crucifixion of Jesus Christ and Don Juan injuries to the current plate osteosynthesis.

PURPOSE: Calcaneal fractures are one of the most challenging injuries to treat and one of the most divisive. The purpose of this historical review is to highlight the evidence of calcaneal fracture and its treatment through history. METHODS: Archaeological, religious, artistic, literary and historical accounts were searched for descriptions of calcaneal fracture to give a thorough overview of the subject. The scientific literature was searched to highlight the evolution of treatment techniques. RESULTS: For over 2500 years, the only available option was conservative treatment due to the high

Posterior interosseous nerve entrapment after Monteggia fracture-dislocation in children.

OBJECTIVE: Although most of nerve injuries associated with Monteggia fracture-dislocation in children are neurapraxias and will recover spontaneously after conservative treatment, surgical exploration of the involved nerve is always required in the cases with the entrapment of posterior interosseous nerve (PIN). However, the necessity and time frame for surgical intervention for specific patterns of nerve dysfunction remains controversial. The aim of the report is to observe and understand the pathology of PIN injury associated with Monteggia fracture-dislocation in children, and to propose th

Posterior atlantoaxial dislocation without fracture and neurologic deficit: a case report and the review of literature.

Traumatic posterior atlantoaxial dislocation without related fracture of the odontoid process is very rare, and only ten cases have been previously reported. The objective of this paper was to describe a case of traumatic posterior atlantoaxial dislocation without related fracture of the odontoid process, and its management with atlantoaxial transarticular screw fixation and bony fusion through an anterior retropharyngeal approach, and to review the relevant literature. The patient's medical and radiographic history is reviewed as well as the relevant medical literature. Posterior atlantoaxial

Pisiform dislocation.

A pisiform dislocation is an uncommon injury which can lead to significant morbidity if missed. The literature regarding pisiform dislocation is limited and largely from case reports. In this case, we present a 51-year-old right-hand dominant male who sustained the injury after a fall. He attended the emergency department on the same day and a closed reduction was able to be performed under a haematoma block. On review in follow-up clinic the patient's symptoms had completely resolved.

Irreducible fracture of the calcaneus due to flexor hallucis longus tendon interposition.

An unusual fracture-dislocation of the calcaneus is presented in which tendinous interposition in the subtalar joint prevented the possibility of closed reduction. This situation has been reported in the literature in only five previous patients. The position of the flexor hallucis tendon in the joint could be visualized on the preoperative computed tomographic scan. The patient was treated operatively with reduction through a lateral approach without a supplemental medial incision.

The comparative study of mandibular ramus growth with different treatment methods for intracapsular condylar fracture.

PURPOSE: The purpose of this study was to evaluate mandibular ramus growth after different treatment methods for intracapsular condylar fracture (ICF) of growing goats. METHOD: Twelve 6-month-old goats were randomly divided into 3 groups (n = 4). The right ICF was created and respectively treated with open reduction and internal fixation (ORIF) and closed treatment. No operation was taken in the control group. The maximum mouth opening and height of mandibular ramus were measured immediately and 3 months after operation by 3-dimensional computed tomographic analysis. Results of the 3 groups w

Temporary transarticular stabilization of a glenohumeral fracture dislocation: a technique revisited.

A 93-year-old patient with an unstable fracture-dislocation of the glenohumeral joint and medical problems that precluded open surgery was first managed by an unsuccessful closed reduction. Temporary transarticular pin fixation then was used successfully, allowing the patient to return to early activities with minimal morbidity.

Combined Supracondylar Humerus Fracture, Monteggia Injury, and Radial/Ulnar Shaft Fractures in a 3-Year-Old Child.

Ipsilateral pediatric elbow and forearm injuries are uncommon, particularly those comprising a supracondylar humerus fracture, radiocapitellar joint dislocation, proximal ulna fracture, and distal forearm fracture. We present the case of a boy who was 3 years, 6 months old and sustained this constellation of injuries. He underwent urgent treatment with closed reduction and percutaneous fixation of the humerus and flexible intramedullary nail fixation of the ulna. He presented with an ulnar sensory and motor neuropraxia that resolved, and he had full radiographic union and upper extremity range

Late-Onset Radial Nerve Palsy in the Setting of Humeral Shaft Hypertrophic Nonunion: A Case Report.

CASE: A 67-year-old woman presented 9 months after a closed midshaft humerus fracture with a new onset radial nerve palsy. Radiographs demonstrated a hypertrophic nonunion. Upon exploration, the radial nerve was in continuity and entrapped in fracture callus. It was extricated from the callus, and an open reduction and plate fixation was performed. Full radial nerve function returned by 3 months. CONCLUSION: We recommend that delayed onset radial nerve palsies be treated on a semiurgent basis with radial nerve exploration and decompression followed by internal fixation to achieve primary bone

Ulnar neuropathy following distal humerus fracture fixation.

Ulnar nerve dysfunction is a well-recognized phenomenon following distal humerus fractures. Its fixed anatomic position predisposes the nerve to injury. Injury can occur at the time of injury, during closed-fracture manipulation, intraoperatively during fracture fixation (when it is routinely identified), or during fracture healing. Intraoperative management varies widely and can include in situ decompression or anterior transposition. This article reviews the literature and presents 24 patient cases. A 38% incidence of late ulnar neuropathy following open reduction and internal fixation is id

Talar neck fractures.

Talus fractures are rare injuries that account for approximately 3% of all foot fractures; talar neck fractures account for almost 50% of all talus fractures. The appropriate diagnosis and treatment of these fractures play an important role in the patient's outcome. Treatment has evolved slowly throughout the years, from closed treatment to open reduction and internal fixation (ORIF). The treatment of type I and type II talar neck fractures is not uniform in the orthopedic community. The option of closed reduction versus ORIF is dependent upon the degree of injury, surgeon experience, and pref

Open knee dislocation, triple intra-articular fractures and patellar tendon rupture: case report of a knee disaster treated with aggressive irrigation/debridement, early anatomic reduction and internal fixation.

BACKGROUND: Open knee fracture-dislocation is a rare orthopedic injury. However, the importance of its correct management could not be overstated. To the best of our knowledge, this is the fifth study reporting a case with simultaneous Hoffa fracture and knee dislocation and the 1st study describing a patient with open plateau fracture-dislocation accompanied with Hoffa fracture, patella fracture, and patellar tendon tear. In addition, this report is noticeable as our case had no gross ligament injury unlike frequent association of knee dislocation with knee collateral ligament damage. CASE P

Consensus for management of sacral fractures: from the diagnosis to the treatment, with a focus on the role of decompression in sacral fractures.

BACKGROUND: There is no evidence in the current literature about the best treatment option in sacral fracture with or without neurological impairment. MATERIALS AND METHODS: The Italian Pelvic Trauma Association (A.I.P.) decided to organize a consensus to define the best treatment for traumatic and insufficiency fractures according to neurological impairment. RESULTS: Consensus has been reached for the following statements: When complete neurological examination cannot be performed, pelvic X-rays, CT scan, hip and pelvis MRI, lumbosacral MRI, and lower extremities evoked potentials are usefu

Facet fracture-dislocation injuries of the cervical spine.

Sixty-eight patients with acute traumatic cervical facet fracture-dislocation injuries are presented. These patients represented 6.7% of all cervical spine fractures identified over a 12-year period. Thirty-one patients had unilateral facet injuries and 37 had bilateral facet injuries. Neurological morbidity was 90% and was most severe among bilateral facet injury patients (84% complete injuries). Spinal shock was identified in 13 patients and was a poor prognostic indicator for the subsequent recovery of associated neurological deficits. Closed reduction was attempted in 66 of 68 patients and
