PDE4 Inhibitor-Responsive Dermatoses: An Emerging Concept in Dermatology.

Many inflammatory skin conditions share mechanistic pathways involving multiple receptors and cytokines. Perhaps there is no better endorsement of this idea than the foundational concept of "steroid-responsive dermatoses." Similarly, targeting the phosphodiesterase 4 (PDE4) enzyme, which exists at the crux of several pathways that contribute to inflammatory skin diseases, offers an avenue by which the pathogenesis of multiple conditions may be interrupted. This suggests that PDE4 inhibitors may assume a similar complementary framework for treating "PDE4 inhibitor-responsive dermatoses." Certai

Discovery of small molecule ACC inhibitors: Potential treatment for excessive sebum secretion.

Overproduction of sebum can lead to various skin disorders, including acne and seborrheic dermatitis. Acetyl-CoA carboxylase is a key enzyme in the de novo synthesis of sebum. Consequently, inhibiting acetyl-CoA carboxylase is a feasible strategy to reduce sebum production, thereby providing therapeutic benefits for associated skin conditions. This review described the de novo synthesis of sebum and discusseed the various isoforms, functions and catalytic mechanisms of acetyl-CoA carboxylase. Additionally, it offered a comprehensive overview of the research advancements and structural-activity

Psoriasis in infancy: therapy with calcipotriene ointment.

Psoriasis in infancy is often more therapeutically challenging than atopic and seborrheic dermatitis. The generalized nature of psoriasis and the intensity of inflammation often reduce the efficacy of topical corticosteroids. Furthermore, involvement of intertriginous skin and the presence of scalp disease limit the potency of the topical steroids that can be prescribed. We report on an infant treated with topical calcipotriene for infantile psoriasis who experienced greater benefit than he had with standard corticosteroid medications. Laboratory testing for calcium metabolism was normal durin

Keratosis lichenoides chronica: marked response to calcipotriol ointment.

Keratosis lichenoides chronica (KCL) is a rare dermatosis characterized by a distinctive seborrheic dermatitis-like facial eruption, together with violaceous, papular, and nodular lesions on the extremities and trunk, typically arranged in a linear and reticulate pattern. KLC is resistant to therapy, although spontaneous remission has been reported. We describe a 35-year-old woman with KLC who had the typical features of widespread violaceous, reticulate, and striae-like eruptions with a prominent keratotic component over a nine-year period and who responded well to treatment with calcipotriol

Meibomian gland dysfunction.

Blepharitis is probably the most common disease entity seen in the general ophthalmologist's office. A significant proportion of these cases are secondary to meibomian gland disease. This review outlines our knowledge of the histopathology, lipid abnormalities and role of microorganisms in meibomian gland dysfunction. We will also review the physiology of meibomian gland secretion and present models of meibomian gland dysfunction which have enhanced our knowledge of this condition. The importance of diagnosing associated conditions such as aqueous tear deficiency, contact lens intolerance, ros

A Novel IFIH1 Mutation Causing Hyper-IgE Syndrome Presenting as Childhood Erythroderma: Case Report.

Erythroderma in the pediatric population is an uncommon yet challenging condition. The etiologies encompass a wide variety of illnesses, including seborrheic dermatitis, infections, inborn errors of immunity, ichthyosiform disorders and metabolic abnormalities. Hyper-immunoglobulin E syndrome (HIES) occurs due to various known genetic variants, e.g., STAT3 (signal transducer and activator of transcription-3), IL6R (Interleukin-6 receptor) deficiency and others, with an extensive list of features including erythroderma in childhood. Our case describes association of a possible novel mutation in
