Emergency chest wall reconstruction in open pneumo-thorax from gunshot chest: A case report.

Chest trauma, penetrating or blunt is common in this era of motor vehicle accidents, violence and terrorism in South Asia. Islamabad is the capital of Pakistan but there is no dedicated chest surgery unit in any government sector hospitals. Gunshot chest, is therefore managed by general surgery team in our tertiary care setting i.e. Federal Government Polyclinic Hospital and Post Graduate Medical Institute, Islamabad. We report a case of gunshot chest with lung contusion and open pneumothorax with a chest wall defect of 10 x 15 cm. in March 2015, this young man presented in emergency departmen

Exogenous testosterone affects early threat processing in socially anxious and healthy women.

Testosterone plays an important role in social threat processing. Recent evidence suggests that testosterone administration has socially anxiolytic effects, but it remains unknown whether this involves early vigilance or later, more sustained, processing-stages. We investigated the acute effects of testosterone administration on social threat processing in 19 female patients with Social Anxiety Disorder (SAD) and 19 healthy controls. Event-related potentials (ERPs) were recorded during an emotional Stroop task with subliminally presented faces. Testosterone induced qualitative changes in early

Learning to read without a left occipital lobe: right-hemispheric shift of visual word form area.

Using anatomical and functional magnetic resonance imaging, we studied the pattern of brain lateralization during spoken and written language tasks, in an 11-year-old girl who underwent a left occipitotemporal resection for a Sturge-Weber angioma at the age of 4 years, that is, after the development of speech but before the acquisition of reading. We observed a selective and successful shift to the right hemisphere of the visual component of reading, particularly the Visual Word Form Area, whereas the verbal components remained strongly left-lateralized. This emphasizes the potential utility o

Development and Validation of the Novel Exergame-Integrated Robotic Stepper Device for Seated Lower Limb Rehabilitation.

Seated rehabilitation is essential in early-stage recovery for patients who can sit but cannot stand or walk. Robotic-based lower limb rehabilitation provides precise, task-specific training for recovery, but its application in seated exercises remains limited, creating a significant gap in early-stage rehabilitation. This study presents a novel exergame-integrated robotic stepper for seated bilateral and multi-joint lower limb rehabilitation. Twenty healthy participants performed seated stepping exercises across eight modes: passive (G0), three-level assistive(G1-G3), active (G4), and three-l

Neurocognitive Correlates of Rumination Risk in Children: Comparing Competing Model Predictions in a Clinically Heterogeneous Sample.

The current study examined associations between rumination and executive function difficulties in preadolescent youth, using predictions outlined in the attentional scope and multiple systems models of rumination. This study aimed to (a) extend current conceptual models of rumination to youth, (b) clarify disparate model predictions regarding working memory updating ("updating"), inhibition, and shifting abilities, and (c) examine differential neurocognitive predictions between two forms of rumination, sadness and anger. One hundred and fifty-nine youths oversampled for ADHD and other forms of

Patterns of Longitudinal Neural Activity Linked to Different Cognitive Profiles in Parkinson's Disease.

Mild cognitive impairment in Parkinson's disease (PD) has been linked with functional brain changes. Previously, using functional magnetic resonance imaging (fMRI), we reported reduced cortico-striatal activity in patients with PD who also had mild cognitive impairment (MCI) vs. those who did not (non-MCI). We followed up these patients to investigate the longitudinal effect on the neural activity. Twenty-four non-demented patients with Parkinson's disease (non-MCI: 12, MCI: 12) were included in the study. Each participant underwent two fMRIs while performing the Wisconsin Card Sorting Task 20

AI-Driven Drug-Target Interaction Prediction: From Data Representation to Model Design.

Drug-target interaction (DTI) prediction is central to drug discovery, target identification, and drug repurposing. With the rapid growth of biomedical data and advances in artificial intelligence (AI), DTI prediction has shifted from docking, similarity-based inference, and hand-crafted features toward data-driven representation learning and interaction modeling. This review examines AI-driven DTI prediction methodologies, covering binding theories, task formulations, data representation, model design, translational applications, and unresolved challenges. First, we introduce classical molecu

Attentional processing of concern-related stimuli in dissociative identity disorder: No evidence for inter-identity differences.

Clinically derived theoretical models of dissociative identity disorder (DID) suggest that these individuals experience shifts in cognitive functioning between identity states. While prior research has found no objective inter-identity differences in memory, the present study investigated whether such differences are evident in a related cognitive domain: attentional processing. It was hypothesized that individuals with DID would exhibit differences in their attentional bias (AB) toward concern-related stimuli. They were compared to individuals with PTSD who experienced interpersonal childhood

Firefighter exposure to polycyclic aromatic hydrocarbons and black carbon during prescribed burns and wildfires.

Prescribed burns are valuable tools for landscape management, which reduce wildfire risk in fire-prone ecosystems. However, they generate smoke emissions containing hazardous pollutants such as polycyclic aromatic hydrocarbons (PAHs) and black carbon (BC). Here, PAHs were analyzed in atmospheric particulate matter (PM2.5) filters in firefighter's personal real-time BC monitors during prescribed burns and wildfires between 2022 and 2024 in Catalonia (NE Spain). The innovative analytical method using gas-chromatography coupled to Q-exactive Orbitrap mass spectrometry (GC-Orbitrap-MS) allows the

Training less threatening interpretations over the Internet: Does the number of missing letters matter?

BACKGROUND AND OBJECTIVES: Cognitive Bias Modification to reduce threat interpretations (CBM-I) trains individuals to resolve ambiguous scenarios via completion of word fragments that assign benign meanings to scenarios. The current study tested: 1) whether Internet-based CBM-I can shift interpretations to be more positive/less negative, and 2) whether varying the number of letters missing in the word fragments (assumed to increase task difficulty) moderates CBM-I's effects. METHODS: Participants (N = 350) completed a brief online version of CBM-I, followed by assessments of interpretation bi

Anosognosia in parietal lobe syndrome.

Patients with right parietal lesions often deny their paralysis (anosognosia), but do they have "tacit" knowledge of their paralysis? I devised three novel tests to explore this. First, the patients were given a choice between a bimanual task (e.g., tying shoe laces) vs a unimanual one (e.g., threading a bolt). They chose the former on 17 of 18 trials and, surprisingly, showed no frustration or learning despite repeated failed attempts. I conclude that they have no tacit knowledge of paralysis (or, if such knowledge exists, it is not available for this particular task). Second, I used a "virtu

Gaze Allocation and Performance Across Task-Demand Conditions During Squat-Based Exergaming: Pilot Study Using Eye Tracking.

BACKGROUND: Exergames integrate motor, cognitive, and postural-control demands; however, how specific task-demand manipulations influence gaze allocation during exergame performance remains insufficiently characterized. OBJECTIVE: This exploratory pilot study used a within-participant design to examine whether gaze allocation and task performance differed across baseline, concurrent cognitive-task, and unilateral-squat conditions during squat-based exergaming in healthy young adults. METHODS: Eight healthy adults (mean age 20, SD 1 years; 7 men and 1 woman) used a squat-based exergame (Ring

Chronic Spasticity Care.

Chronic spasticity is a dynamic, multifactorial condition that demands continuous, personalized management. This article reviews the evolving pathophysiology of spasticity, emphasizing the interplay of neural hyperexcitability and structural muscle changes. Botulinum toxin type A remains a key intervention but achieves optimal outcomes only when integrated into a multidisciplinary rehabilitation framework. Evidence supports a multimodal approach and the use of adjunct therapies such as stretching, strengthening, orthoses, and task-specific training. Technological advances and goal-oriented too

Transport and industrial safety, how are they affected by sleepiness and sleep restriction?

This review shows that impaired/shortened sleep is a major cause of accidents in industry and transport. The reason is either sleep pathology, voluntary (non-pathological) sleep reduction, or activity during the circadian low. Night or morning work is a prominent factor with regard to the latter two. However, the link between sleep restriction (or shift work) and safety is much better established in the transport industry than in other industrial areas. The reason is that driving a vehicle is a task with continuous demand for attention and immediate punishment for lapses, whereas industrial wo

Adjunctive utility of gaze-based indices obtained with Gazefinder for ADHD assessment in children.

Diagnosis of attention-deficit/hyperactivity disorder (ADHD) relies primarily on subjective clinical assessments, necessitating the establishment of objective, low-burden supplementary indicators. While eye-tracking has emerged as a noninvasive method for capturing temporal fluctuations in attentional allocation, its discriminative performance in ADHD remains under-explored. This study examined differences in gaze indices between children with ADHD and typically developing (TD) children using the Gazefinder system and evaluated the utility of these indices in distinguishing diagnostic groups.

Machine learning-based stratification of chagas heart failure severity using ECG power spectral biomarkers.

PURPOSE: This study presents a machine learning methodology to automatically classify heart failure severity in Chagas disease (CD) patients using non-invasive 24-hour ECG-Holter signals. METHODS: Following American Heart Association (AHA) guidelines, the cohort was stratified into three Left Ventricular Ejection Fraction (LVEF)-based severity groups: Normal (LVEF ≥ 0.50, n=197), Moderate (0.40 ≤ LVEF < 0.50, n=106), and Severe (LVEF < 0.40, n=77), totaling N=380 patients. From short 10-second ECG segments, we extracted eleven spectral features derived from the power spectral density (PSD). Cl

Using brain potentials to understand prism adaptation: the error-related negativity and the P300.

Prism adaptation (PA) is both a perceptual-motor learning task as well as a promising rehabilitation tool for visuo-spatial neglect (VSN)-a spatial attention disorder often experienced after stroke resulting in slowed and/or inaccurate motor responses to contralesional targets. During PA, individuals are exposed to prism-induced shifts of the visual-field while performing a visuo-guided reaching task. After adaptation, with goggles removed, visuomotor responding is shifted to the opposite direction of that initially induced by the prisms. This visuomotor aftereffect has been used to study visu

Automatic rib fracture detection on postmortem CT data using deep learning.

OBJECTIVES: To assess the performance of automatic rib fracture detection of an existing deep learning (DL) model, nnDetection, on a postmortem (PM) CT scan dataset, and to identify the main factors of domain shift between clinical and PM CT imaging. BACKGROUND: Rib fracture detection and classification in forensic investigations is a time-consuming yet crucial task that can contribute to determine the cause of death. DL models are a promising tool, as recent research shows that radiologists using DL models can detect rib fractures in clinical CT scans at higher sensitivity and in shorter tim

Patient Outcomes and Hospital Nurses' Workload: A Cross-Sectional Observational Study in Slovenian Hospitals Using the RN4CAST Survey.

INTRODUCTION: Higher nursing workload increases the odds of patient deaths, as the work environment has a significant effect on patient outcomes. The aim of the study was to explore the relation between patient outcomes and nurses' working conditions in hospitals. METHODS: Administrative data on discharges of surgical patients for the year 2019 in eight general hospitals and two university medical centres in Slovenia were collected to determine in-hospital mortality within 30 days of admission. The RN4CAST survey questionnaire was used to gather data from nurses in these hospitals, with 1,010

The Development of Size Sequencing Skills: An Empirical and Computational Analysis.

We explore a long-observed phenomenon in children's cognitive development known as size seriation. It is not until children are around 7 years of age that they spontaneously use a strict ascending or descending order of magnitude to organize sets of objects differing in size. Incomplete and inaccurate ordering shown by younger children has been thought to be related to their incomplete grasp of the mathematical concept of a unit. Piaget first brought attention to children's difficulties in solving ordering and size-matching tests, but his tasks and explanations have been progressively neglecte
