Dexmedetomidine constant-rate infusion with additional partial intravenous anesthesia for a dog undergoing partial pancreatectomy for an insulinoma.

Alpha-2 adrenergic agonist use in the anesthetic management of dogs undergoing partial pancreatectomy has been reported, but only as single-bolus administration of medetomidine in the premedication or low-dose constant-rate infusion (CRI) of dexmedetomidine, without other systemic analgesic drugs. A 10-year-old Boston terrier diagnosed with an insulinoma was presented to the Ontario Veterinary College Health Sciences Centre (Guelph, Ontario). The anesthetic management for partial pancreatectomy included a high-dose dexmedetomidine CRI (4 μg/kg per hour) as well as lidocaine and fentanyl CRIs f

Extracorporeal Membrane Oxygenation (VA-ECMO) in Management of Cardiogenic Shock.

Cardiogenic shock is a critical condition of low cardiac output resulting in insufficient systemic perfusion and end-organ dysfunction. Though significant advances have been achieved in reperfusion therapy and mechanical circulatory support, cardiogenic shock continues to be a life-threatening condition associated with a high rate of complications and excessively high patient mortality, reported to be between 35% and 50%. Extracorporeal membrane oxygenation can provide full cardiopulmonary support, has been increasingly used in the last two decades, and can be used to restore systemic end-orga

A computational and experimental study of veno-arterial extracorporeal membrane oxygenation in cardiogenic shock: defining the trade-off between perfusion and afterload.

Veno-Arterial Extracorporeal Membrane Oxygenation (VA-ECMO) is a type of mechanical circulatory support used, among others, in case of cardiogenic shock, consisting in percutaneous cannulation of the femoral artery. Despite the widespread use of this procedure in clinical practice, a deeper understanding of the complex interaction between native and ECMO output, as well as the fluid dynamics and perfusion of aorta and its branches is still required. Herein, a numerical and experimental approach is presented to model a VA-ECMO procedure on a patient-specific aortic geometry. For both approaches

Analysis of the cardiovascular effects of hyperbaric oxygen therapy in diabetic patients.

During hyperbaric oxygen (HBO2) therapy in humans, there are changes in cardiovascular physiology due to high pressure and hyperoxygenation. Peripheral vasoconstriction, bradycardia, and a decrease in cardiac output are observed during HBO2 therapy. These physiological effects of HBO2 therapy on the cardiovascular system are tolerated in healthy people. However, patients with underlying cardiac disease may experience severe problems during HBO2 therapy, such as pulmonary edema and death. In addition, cardiac complications may occur in patients with diabetes mellitus (DM). Therefore, HBO2 thera

Impact of heart rate on cross-bridge cycling kinetics in failing and nonfailing human myocardium.

The force-frequency relationship (FFR) is an important regulatory mechanism that increases the force-generating capacity as well as the contraction and relaxation kinetics in human cardiac muscle as the heart rate increases. In human heart failure, the normally positive FFR often becomes flat, or even negative. The rate of cross-bridge cycling, which has been reported to affect cardiac output, could be potentially dysregulated and contribute to blunted or negative FFR in heart failure. We recently developed and herein use a novel method for measuring the rate of tension redevelopment. This met

Neurohumoral mechanisms associated with orthostasis: reaffirmation of the significant contribution of the heart rate response.

The inability to compensate for acute central hypovolemia underlies the clinical development of orthostatic hypotension and instability (e.g., syncope). Although neuro-humoral control of both cardiac output and peripheral vascular resistance contributes to hemodynamic stability during orthostasis, a notion has been proposed that the failure of adequate peripheral vascular constriction rather than cardiac responses represents the primary mechanism underlying the development of orthostatic intolerance. This review article provides an opportunity to present compelling evidence captured over the p

The Effects of Vasoactive Medications on Mean Circulatory Filling Pressure, Venous Resistance, Systemic Vascular Resistance, Cardiac Index, and Oxygen Extraction After Pediatric Heart Transplant: Leveraging High-Fidelity Physiologic Data.

Background: The physiologic effects of vasoactive medications on the venous circulation remain incompletely understood. Contemporary bedside management often emphasizes the arterial circulation, whereas Guytonian physiology emphasizes the venous circulation and mean circulatory filling pressure in determining steady-state cardiac output. The primary aim of this study was to characterize the effect of vasoactive medications on mean circulatory filling pressure and venous resistance. Methods: Demographic data and vasoactive data were collected from the electronic health record and collated with

Maternal propranolol as a safe alternative to invasive fetal surgery in chorioangioma management.

Chorioangiomas are associated with a significant risk of fetal demise due to high-output cardiac failure. Standard invasive treatment, including fetoscopic laser photocoagulation and intratumoral embolization, shows limited success and has high fetal mortality rates. The biological parallels between chorioangiomas and infantile hemangiomas suggest that these tumors may share common therapeutic targets. Propranolol is a proven therapy for infantile hemangiomas, therefore this established efficacy provides a strong rationale for its consideration in the management of symptomatic or rapidly growi

A Generative AI Framework to Predict Cardiomyocyte Contraction Function from Single Static Images.

Understanding how cardiomyocyte structure governs contractile function is fundamental to cardiac biology and disease modeling, yet current approaches rely on time-resolved imaging and computationally intensive analysis. Here, we present a generative artificial intelligence (AI) framework that directly predicts contractile behavior of human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) from single static images. Our approach integrates a U-Net-based generator with a patch-based generative adversarial network (GAN) discriminator to translate morphological and sarcomere structu

Apple Supplementation Improves Hemodynamic Parameter and Attenuates Atherosclerosis in High-Fat Diet-Fed Apolipoprotein E-Knockout Mice.

Epidemiological studies describe the association between apple consumption and improved cardiovascular and metabolic dysfunction. Our recent multiparametric screening on cellular model studies has shown that apples exhibit vascular tropism including Granny Smith (GS) variety independently of the storage condition. The present study aimed to evaluate the cardiovascular and metabolic protection of supplementation of GS variety after storage in classic cold (GSCC) and extreme ultra-low oxygen conditions (GSXO) in the apolipoprotein E-deficient 8-week-old mice fed with high fat diet for 14 weeks.

Protein-Losing Enteropathy Following Fontan Palliation.

Protein-losing enteropathy (PLE) is a chronic condition involving multiple organ systems that may develop any time following Fontan completion. The pathogenesis of PLE is complex and multifactorial. Chronic venous hypertension, low cardiac output, and abnormal lymphatics may all play a role in the pathogenesis of PLE. Common signs and symptoms include chronic diarrhea, abdominal pain, and ascites. Diagnosis is based on the presence of signs and symptoms in addition to hypoalbuminemia and elevated stool alpha 1 antitrypsin. Early identification and a comprehensive approach to evaluation and tre

Obesity and the heart.

Obesity can result in alterations in cardiac structure and function even in the absence of systemic hypertension and underlying organic heart disease. Increased total blood volume creates a high cardiac output state that may cause ventricular dilatation and ultimately eccentric hypertrophy of the left (and possibly the right) ventricle. Eccentric left ventricular (LV) hypertrophy produces diastolic dysfunction. Systolic dysfunction may ensue due to excessive wall stress if wall thickening fails to keep pace with dilatation. This disorder is referred to as obesity cardiomyopathy. The presence o

Tissue valves in the mitral position. Five years' experience.

Between April 1969 and November 1973 103 patients underwent isolated mitral replacement with three-cusp stented tissue valves. Autologous fascia lata was used in 50 patients, homologous fascia lata in 21, and heterologous pericardium in 32. The early mortality rate (14.6%) was influenced by age, the extent of preoperative cardiac disability, and low cardiac output. The survivors were followed up for periods varying from 8 to 60 months (average 37 months). In general, a factor in late death (13.6%) was high preoperative pulmonary artery pressure. In the autologous fascial series valve failure a

The role of hemoglobin oxygen affinity in oxygen transport at high altitude.

Hemoglobin is involved in the regulation of O(2) transport in two ways: a long-term adjustment in red cell mass is mediated by erythropoietin (EPO), a response to renal oxgyenation. Short-term, rapid-response adjustments are mediated by ventilation, cardiac output, hemoglobin oxygen affinity (P50), barriers to O(2) diffusion, and the control of local microvascular tissue perfusion. The distribution of O(2) between dissolved (PO2) and hemoglobin-bound (saturation) is the familiar oxygen equilibrium curve, whose position is noted as P50. Human hemoglobin is not genetically adapted for function a

Effects of acute expansion of plasma volume on cardiovascular and thermal function during prolonged exercise.

To investigate the hypothesis that an increase in plasma volume (PV) is obligatory in reducing the cardiovascular drift that is associated with prolonged exercise following training, a plasma expander (Macrodex) was used to acutely elevate PV. Eight untrained volunteers [maximal oxygen consumption; VO2max 45.2 (2.2) ml x kg(-1) x min(-1), mean (SE)] cycled for 2 h [at 46 (4)% VO2max] in ambient conditions either with no PV expansion (CON) or following PV expansions of either 14% (LOW) or 21% (HIGH). During CON, heart rate (HR) increased (P < 0.05) from 147 (2.4) beats x min(-1) to 173 (3.6) be
