Pregnancy complicated by Ebstein's anomaly: oxygen administration to mother for chronic fetal hypoxemia. A therapeutic case report.

For chronic fetal hypoxia due to maternal Ebstein's anomaly, oxygen was administered daily to the mother by mask for 105 days. At 20 weeks of gestation, umbilical venous blood gases in room air showed pH 7.42, PO2 25.7 mm Hg, PCO2 33.7 mm Hg and O2 saturation 48.7%, and changed to 7.45, 39.1 mm Hg, 25.9 mm Hg and 77.4% on 3 liters/min of oxygen inhalation by mask, respectively. The PO2 of the maternal arterial blood gases increased to 30 mm Hg on oxygen administration at 15 weeks of gestation, but at 25 weeks of gestation the PO2 increased by only about 10 mm Hg. At 30 weeks, intrauterine grow

[A successfully treated case of Wolff-Parkinson-White syndrome with Ebstein's anomaly complicated with protein-losing gastroenteropathy].

A 17-year-old male underwent the division of the double accessory pathways under the diagnosis of Wolff-Parkinson-White (WPW) syndrome. At this time, we perceived that Ebstein's anomaly was combined with WPW syndrome, but we didn't treat Ebstein's anomaly, because it was slight. After this operation he relieved from the tachycardia attack completely. Ten months later, he was re-admitted to our hospital because of cardiomegaly and systemic edema. Hypoproteinemia was pointed out. On the basis of the intracardiac catheterization and 131I-RISA test, it was made to be sure that hypoproteinemia was

A comparison of the clinical, haemodynamic and angiographic features in right ventricular endomyocardial fibrosis and Ebstein's anomaly of the tricuspid valve.

The clinical, haemodynamic and angiographic features of 18 patients with right ventricular endomyocardial fibrosis (RVEMF) and 8 patients with Ebstein's anomaly of the tricuspid valve (EATV) have been compared. Diagnosis was confirmed by selective angiography. The position of the tricuspid annulus was identified from selective right ventricular angiograms and confirmed by selective right coronary angiography. In 83% of RVEMF patients the tricuspid annulus was displaced to the left of the spine. A false impression of displacement of the tricuspid leaflet can thus be created. However, a tricuspi

[Study on tricuspid regurgitation in Ebstein's anomaly using pulsed Doppler technique combined with two-dimensional echocardiography].

To investigate the localization of tricuspid regurgitant flow in Ebstein's anomaly, pulsed Doppler echocardiography (PDE) was performed on nine patients with or without associated cardiac lesions. The equipment used was characterized by the real time frequency analysis of Doppler signals obtained from the sampling sites confirmed on the cross-sectional images. We employed the apical four-chamber view or right ventricular inflow view in order to facilitate the sampling of Doppler signals from the three-sided heart chambers of this lesion. Doppler signals were recorded from several points in the

Early closure of the tricuspid valve in a case of Ebstein's anomaly with type B Wolff-Parkinson-White Syndrome.

Echocardiographic studies of a 19-year-old female with Ebstein's anomaly and type B Wolff-Parkinson-White syndrome showed an earlier closure of the anterior tricuspid leaflet (ATL) than of the anterior mitral leaflet (AML), contrary to the previous reports. The interval between the closure of the ATL and the AML was 20 msec and 30 msec, respectively, before and after administration of edrophonium chloride. However, closure of the AML preceded that of the ATL after administration of atropine sulfate and during supraventricular tachycardia by 10 msec and 60 msec, respectively, concomitant with t

Bovine Jugular Vein Conduit for Tricuspid Valve Replacement in a Young Infant With Ebstein Anomaly: A Case Report With 16 Years of Follow-up.

Ebstein anomaly is a rare congenital heart defect that is characterized by failure of the tricuspid valve leaflets to delaminate from the right ventricular endocardium, resulting in apical displacement of the tricuspid valve annulus and atrialization of the right ventricle. An irreparable tricuspid valve generally precludes at least initial attempts at a biventricular repair in a young infant. Although there are commercially available prosthetic tricuspid valves small enough to be implanted in these patients, they are generally not appropriate for young infants due to their bulk, which makes t…

Assessment of Left Ventricular Myocardial Fibrosis in Adult Patients With Ebstein Anomaly: A Retrospective Cohort Study Based on Cardiac Magnetic Resonance and Histopathological Samples.

BACKGROUND: The association between Ebstein anomaly and myocardial fibrosis, particularly in the left ventricle, has been controversial. We aimed to assess the prevalence of replacement fibrosis with a focus on the left ventricle (LV) using cardiac magnetic resonance (CMR), make a histopathological association between LV fibrosis and CMR findings, and explore whether LV fibrosis is an independent risk factor for cardiovascular disease mortality using a derived risk score. METHODS: We performed a 12-year (2009-2021) retrospective cohort of adult patients with Ebstein anomaly who underwent CMR.…

Left Bundle Branch Pacing Across a Bioprosthetic Valve in a Post-Glenn Ebstein's Anomaly With Complete AV Block.

We report a 14-year-old female with a history of surgically corrected Ebstein anomaly, including a bidirectional Glenn shunt and atrial septostomy. Following a bioprosthetic tricuspid valve replacement, the patient developed complete atrioventricular block and became pacemaker-dependent. A unique transvenous approach was utilized: the ventricular lead was advanced from the left axillary vein through the left subclavian vein, superior vena cava, right pulmonary artery, and main pulmonary artery into the right ventricle, resulting in successful left bundle branch pacing. This case illustrates an…

Ebstein's anomaly with coarctation of the aorta and a bicuspid aortic valve: a case report of a rare association with unique prenatal findings.

Ebstein's anomaly is rarely accompanied by coarctation of the aorta, although patients with Ebstein's anomaly have a relatively small left ventricle. Here, we report a rare case of Ebstein's anomaly with coarctation of the aorta and a bicuspid aortic valve. We compared the foetal echocardiographic parameters of five previous cases with Ebstein's anomaly without left heart obstruction to explore the association between left ventricular volume, Ebstein's anomaly severity, and left heart obstruction.

Cardiovascular magnetic resonance evidence of myocardial fibrosis and its clinical significance in adolescent and adult patients with Ebstein's anomaly.

BACKGROUND: Myocardial fibrosis is a common pathophysiological process that is related to ventricular remodeling in congenital heart disease. However, the presence, characteristics, and clinical significance of myocardial fibrosis in Ebstein's anomaly have not been fully investigated. This study aimed to evaluate myocardial fibrosis using cardiovascular magnetic resonance (CMR) late gadolinium enhancement (LGE) and T1 mapping techniques, and to explore the significance of myocardial fibrosis in adolescent and adult patients with Ebstein's anomaly. METHODS: Forty-four consecutive patients with

Late-Onset Cyanosis and Finger Clubbing in a 37-Year-Old Female with Ebstein's Anomaly and Atrial Septal Defect.

UNLABELLED: It is common to suspect Eisenmenger syndrome when patients with atrial septal defect (ASD) show cyanosis and clubbing. However, ASD accompanying Ebstein's anomaly frequently shows cyanosis without severe pulmonary hypertension, especially in the early period after birth. So the medical decision to decline surgical treatment in those patients should not be determined merely by clinical features such as cyanosis and clubbing. We report a case of a 37-year-old female with Ebstein's anomaly and secundum ASD who showed lip cyanosis and finger clubbing, but maintained normal pulmonary ar

Peak oxygen uptake correlates with disease severity and predicts outcome in adult patients with Ebstein's anomaly of the tricuspid valve.

BACKGROUND: Ebstein's anomaly of the tricuspid valve often results in biventricular dysfunction and functional deterioration. Little is known about the relation between exercise capacity, disease severity and outcome in adults with Ebstein's anomaly. METHODS: Data on all patients with Ebstein's anomaly of the tricuspid valve who underwent cardiopulmonary exercise testing in our tertiary center were collected. The relation between exercise parameters, anatomic severity (Glasgow outcome score) and the combined end-point of death, non-elective hospitalization and surgical repair was studied usin
