Effect-Site Target-Controlled Infusion in the Obese: Model Derivation and Performance Assessment.

BACKGROUND: The aim of this study is to derive a propofol pharmacokinetic (PK) pharmacodynamic (PD) model to perform effect-site target-controlled infusion (TCI) in obese patients, and to analyze its performance along with that of other available PK models. METHODS: In the first step of the study, a 3-compartment PK model linked to a sigmoidal inhibitory Emax PD model by a first-order rate constant (keo) was used to fit propofol concentration-bispectral index (BIS) data. Population modeling analysis was performed by nonlinear mixed effects regression in NONMEM (ICON, Dublin, Ireland). PK data

Semantic memory space becomes denser with age.

Semantic memory, a repository for concepts and factual information, plays a vital role in acquiring and retrieving knowledge. This study explores the impact of age-related knowledge accumulation on semantic cognition, investigating whether a denser representational space affects retrieval processes. Using a semantic feature verification task, we employ both behavioral (reaction time; RT) and neurophysiological (event-related potential; ERP) measures to explore these dynamics across young and older adults. Findings revealed an age-related RT difference in retrieval of semantically incongruent f

Magnetoencephalography Reveals Neuroprotective Effects of COVID-19 Vaccination in Non-Human Primates.

COVID-19, caused by the SARS-CoV-2 virus, can lead to widespread neurological complications, including cognitive deficits and neurodegenerative symptoms, even in the absence of significant structural brain abnormalities. The potential neuroprotective effects of SARS-CoV-2 vaccination remain underexplored. Here, we demonstrate the neuroprotective effects of a psoralen-inactivated SARS-CoV-2 vaccine in a non-human primate model using resting-state magnetoencephalography (MEG), a non-invasive neurophysiological recording technique with sub-millisecond temporal and submillimeter spatial resolution

Microsurgical resection of a giant thoracolumbosacral Schwannoma in an adolescent with intraoperative neuromonitoring and anatomical laminoplasty reconstruction.

Spinal schwannomas represent benign neoplasms arising from spinal nerve root sheaths, predominantly occurring in the cauda equina region. By … he coexistence of a schwannoma added complexity to the preoperative diagnosis. Under intraoperative neurophysiological monitoring (IONM), multi-level laminectomy with anatomical reconstruction and tumor resection from T10 to S4 was performed. The pathological result confirmed it to be a schwannoma. The coexistence of two types tumors within the spinal canal is rare, in additio …

Far Lateral Craniotomy and Occlusion In Situ of a Lateral Medullary Arteriovenous Malformation: 2-Dimensional Operative Video.

Lateral medullary arteriovenous malformations (AVMs) are located in the pia on the lateral medullary surface.1 They are supplied by arterial … of the arterial blood supply and occlusion of the draining vein to occlude the AVM. Intraoperative neurophysiological monitoring of motor and somatosensory evoked potentials was used, and the elimination of arteriovenous shunt flow was confirmed using indocyanine green videoangiography. Occlusion in situ preserves the flow to the delicate brainstem perforators and is safer th …

Trigeminal nerve schwannoma with ancient change. Case report and review of the literature.

Ancient change in a schwannoma is a histological variant typically found in longstanding tumors. Histologically, the tumor has biphasic feat … ydrocephalus. Using a lateral suboccipital approach together with image guidance and intraoperative neurophysiological monitoring, a gross-total macroscopic excision was performed. At surgery, the tumor was found to arise from the inferior division of the trigeminal nerve. The final histological diagnosis was schwannoma with ancient change. Note that ancient change in schwanno …

[Intra- and perioperative acoustic evoked brain stem potentials in cerebellopontile angle operations].

From a series of 35 cases with intraoperative monitoring of acoustic evoked brainstem responses 4 cases with cerebellopontine angle surgery are presented in detail. The aim of neuromonitoring is to provide the surgeon with an additional instrument to assess the functional state of the hearing pathway with objective neurophysiological criteria during dissection near the eighth nerve and brainstem. Thus, a near total loss of potential during a neurovascular decompression of the trigeminal nerve could be reversed by modifying the surgical procedure. In 2 cases of acoustic neurinoma with preserved

Synthesis, Radiochemistry, and Preclinical Assessment of the First GPR39 PET Imaging Agent.

GPR39 is a zinc-sensing G protein-coupled receptor with critical roles in neurophysiological and metabolic processes across brain, pancreas, gut, liver, and fat tissues. Activated by extracellular zinc ions, GPR39 is involved in neurodegenerative diseases including altered levels in Alzheimer's disease (AD). Quantifying GPR39 levels in vivo could significantly advance understanding of its role in various metabolic and disease processes, enabling drug development and treatment monitoring. Our study reports the synthesis, radiolabeling, and comprehensive preclinical evaluation of the first radio

Anesthetic Challenges in Debulking Surgery of Massive Plexiform Neurofibroma With Neurofibromatosis Type 1: A Case Report of Intraoperative Generalized Involuntary Movements and Severe Blood Loss.

Neurofibromatosis type 1 is a multisystem genetic disorder commonly associated with plexiform neurofibromas, benign but highly vascular tumors that present significant perioperative challenges. Surgical debulking of plexiform neurofibromas is often associated with difficult anesthetic management due to tumor vascularity, hemorrhage risk, airway considerations, and the need for intraoperative neurophysiological monitoring. We report the case of a 48-year-old man with neurofibromatosis type 1 who underwent debulking of a large right thigh plexiform neurofibroma under general anesthesia without n…

The role of surgery for optic pathway gliomas in the era of precision medicine.

Optic pathway gliomas (OPGs) represent a unique subset of brain tumours that primarily affect the paediatric population. Traditionally, thes … vances in surgical techniques, including neuronavigation, endoscopic approaches, and intraoperative neurophysiological monitoring, have enhanced the safety and efficacy of operative interventions. Despite these developments, the complexity of OPGs necessitates a multidisciplinary approach, focusing on long-term outcomes and quality of life. Future research is needed to further …

Case report: Case analysis of multiple sclerosis with preclinical systemic lupus erythematosus presenting as rare bilateral horizontal gaze palsy.

We present an analysis of a case initially manifesting as bilateral horizontal gaze palsy, eventually diagnosed as multiple sclerosis (MS) with preclinical systemic lupus erythematosus (p-SLE). The patient, a 25-year-old male, exhibited restricted movement in both eyes. Cranial MRI revealed multiple demyelinating lesions; serum analyses indicated elevated levels of antinuclear antibodies (ANA), anti-Sm antibodies, and anti-nRNP antibodies. Oligoclonal bands were identified in the cerebrospinal fluid. Neurophysiological assessments demonstrated damage to the optic, auditory, and facial nerves.

ECOLOGICALLY VALID LONG-TERM MOOD MONITORING OF INDIVIDUALS WITH BIPOLAR DISORDER USING SPEECH.

Speech patterns are modulated by the emotional and neurophysiological state of the speaker. There exists a growing body of work that computationally examines this modulation in patients suffering from depression, autism, and post-traumatic stress disorder. However, the majority of the work in this area focuses on the analysis of structured speech collected in controlled environments. Here we expand on the existing literature by examining bipolar disorder (BP). BP is characterized by mood transitions, varying from a healthy euthymic state to states characterized by mania or depression. The spee

Event-related potentials in a Go/Nogo task of abnormal response inhibition in heroin addicts.

Inhibitory control dysfunction is regarded as a core feature in addicts. The major objective of this study was to explore the time course of response inhibition in chronic heroin addicts and provide the neurophysiological evidence of their inhibitory control dysfunction. The amplitudes and latencies of ERP components were studied in fourteen heroin addicts (mean duration of heroin use being (13.54+/-5.71) years (Mean+/-SD), average abstinence being ((4.67+/-6.44) months)) and fourteen matched healthy controls with a visual Go/Nogo task. Our results showed that heroin addicts demonstrated signi

Clinical impact of transcranial unilateral motor evoked potentials monitoring in cerebral aneurysm endovascular treatment.

OBJECTIVE: Neurosurgery commonly employs motor evoked potential (MEP) monitoring to assess intraoperative motor function. However, its utility in neuroendovascular procedures has been sporadically reported, widespread adoption remains limited, largely due to challenges associated with stimulus-induced patient movement and subsequent image misregistration. This study aimed to address these limitations by evaluating the feasibility of an optimized transcranial electrical stimulation MEP (TES-MEP) protocol designed to minimize body movement while preserving monitoring reliability. METHODS: We per

Intraoperative Electroencephalogram Suppression Predicts Postoperative Delirium.

BACKGROUND: Postoperative delirium is a common complication associated with increased morbidity and mortality, longer hospital stays, and greater health care expenditures. Intraoperative electroencephalogram (EEG) slowing has been associated previously with postoperative delirium, but the relationship between intraoperative EEG suppression and postoperative delirium has not been investigated. METHODS: In this observational cohort study, 727 adult patients who received general anesthesia with planned intensive care unit admission were included. Duration of intraoperative EEG suppression was re

Comparative analysis of acoustic therapies for tinnitus treatment based on auditory event-related potentials.

INTRODUCTION: So far, Auditory Event-Related Potential (AERP) features have been used to characterize neural activity of patients with tinnitus. However, these EEG patterns could be used to evaluate tinnitus evolution as well. The aim of the present study is to propose a methodology based on AERPs to evaluate the effectiveness of four acoustic therapies for tinnitus treatment. METHODS: The acoustic therapies were: (1) Tinnitus Retraining Therapy (TRT), (2) Auditory Discrimination Therapy (ADT), (3) Therapy for Enriched Acoustic Environment (TEAE), and (4) Binaural Beats Therapy (BBT).
