Effect of the Ten-valent Pneumococcal Conjugate Vaccine on Invasive Pneumococcal Disease and Pneumonia in Infants Younger Than Ten Weeks of Age in Southern Mozambique: A Population-based Prospective Surveillance Study.

BACKGROUND: Mozambique introduced 10-valent-pneumococcal conjugate vaccine (PCV10) in 2013. We aimed to evaluate the indirect effect of PCV10 on invasive pneumococcal disease (IPD) and clinical severe pneumonia (CSP) in infants aged less than 10 weeks. METHODS: Routine surveillance data for IPD and CSP among admitted infants less than 10 weeks of age were analyzed to compare IPD and CSP incidence using demographic surveillance data from pre- and post-PCV10 periods. IPD was defined as isolation of pneumococcus from blood or cerebrospinal fluid, and CSP according to World Health Organization cr

Pneumococcal serotype distribution and outcomes in cancer patients with community-acquired pneumonia: A prospective study using the 15-valent pneumococcal conjugate vaccine serotype specific urinary antigen detection assay.

OBJECTIVES: Cancer patients with pneumococcal community-acquired pneumonia (CAP) have higher mortality than the general population. Data on … CAP, and 31.7% among patients with pneumococcal pneumonia. CONCLUSION: Serotype 7F, covered by all pneumococcal vaccines, was the most common serotype. The high 30-day all-cause mortality highlights the substantial burden of CAP in cancer patients. Enhanced serotype surveillance and vaccine-effectiveness studies are warranted.

Phase 2 trial to evaluate the safety and immunogenicity of a 25-valent pneumococcal conjugate vaccine (IVT-PCV25) in infants.

INTRODUCTION: IVT-PCV25 is a 25-valent pneumococcal conjugate vaccine designed to broaden serotype coverage beyond current vaccines. We evaluated the safety and immunogenicity in infants of three dose-ranging formulations of IVT-PCV25 in comparison to licensed Prevnar-20. METHODS: In this double-blind phase 2 trial in the Philippines, 421 healthy infants were randomly assigned (1:1:1:1) to three IVT-PCV25-formulation arms or a Prevnar-20 arm. Three doses were administered 4-weeks apart from 6 weeks of age, alongside routine vaccinations. Adverse events (AEs), and IgG and functional antibody (

Development and analytical characterization of a 24-plex microsphere-based assay for the quantitation of the antibodies against pneumococcal vaccine serotypes.

BACKGROUND: Quantitative assessment of pneumococcal vaccine-induced immunity is essential for monitoring vaccine effectiveness. The World Health Organization (WHO) pneumococcal polysaccharide enzyme-linked immunosorbent assay (Pn PS ELISA) remains the reference standard for measuring serotype-specific IgG concentrations; however, it is limited by its low throughput and high resource requirements. Our study aim is to develop and analytically characterize a 24-plex microsphere-based immunoassay for simultaneous quantitation of serotype-specific IgG antibodies against 24 pneumococcal vaccine sero

The impact of a pneumococcal vaccination on disease activity in children and adolescents with inflammatory bowel disease: a 2-year prospective study.

BACKGROUND: Infectious diseases are known triggers for inflammatory bowel disease (IBD) exacerbations. Although vaccines can prevent many such infections, hesitancy persists among pediatric IBD patients and their caregivers due to concerns about vaccine-induced disease flare-ups. The aim of the study was to evaluate the impact of the 13-valent pneumococcal conjugate vaccine (PCV13) on disease activity in children and adolescents with IBD over a 24-month period post-vaccination. RESEARCH DESIGN AND METHODS: This prospective, multicenter cohort study included IBD patients aged 4-18 years. Parti

Emergence of Non-vaccine Serotypes and Antibiotic Resistance in Pneumococcal Carriage Among Korean Children: A Cross-sectional Study.

BACKGROUND: Streptococcus pneumoniae colonization in children sustains transmission and often precedes invasive disease. As Korea transitions from the 13-valent pneumococcal conjugate vaccine (PCV13) to a 15-valent vaccine (PCV15) that offers broader serotype coverage, up-to-date data on circulating serotypes and antibiotic susceptibility are needed to guide immunization policy. METHODS: We conducted a cross-sectional pneumococcal carriage survey from 2024 to 2025 in 37 daycare centers across Seoul and Gyeonggi-do and at a pediatric outpatient clinic, enrolling 601 children (primarily 7-59 mo

Tracking spatio-temporal dynamics of early immune responses to an intranasal OMV-based pneumococcal vaccine candidate in mice.

Outer membrane vesicle (OMV)-based vaccines elicit strong immune responses and have emerged as a versatile platform for targeting multiple pathogens, yet the mechanisms underlying their efficacy remain incompletely understood. Here, we investigated the early immune response following intranasal administration of an OMV-based pneumococcal vaccine candidate in mice. Using in vivo bioluminescence imaging, spectral flow cytometry, and high-resolution microscopy, we tracked OMV biodistribution and immune activation throughout the respiratory tract from 1 to 72 h post-immunization. OMVs persisted in

Persistent vaccine-type invasive pneumococcal disease in children despite high PCV13 coverage: the role of serotype 3.

BACKGROUND: Despite widespread implementation of pneumococcal conjugate vaccines (PCVs), invasive pneumococcal disease (IPD) caused by vaccine serotypes continues to occur. Evidence on vaccine failure and breakthrough infections in settings with sustained high PCV13 coverage remains limited. METHODS: We conducted a retrospective observational study including children aged <18 years with IPD admitted to three paediatric referral hospitals in Catalonia, Spain, from 2018 to 2023, a period of high PCV13 coverage (>96%). Cases meeting the epidemiological definitions of vaccine failure and breakthr

Temporal patterns and potential missed opportunities in pneumococcal vaccination among adults with diabetes: A population-based cohort study.

Pneumococcal vaccination is recommended for adults with diabetes because of their increased risk of pneumococcal disease, yet uptake remains suboptimal. This population-based retrospective cohort study assessed temporal patterns and factors associated with first recorded pneumococcal vaccination among adults with diabetes in the Romagna Local Health Authority, Italy, using linked administrative healthcare databases. We included 77,138 individuals (57,113 prevalent and 20,025 incident cases) followed through June 30, 2024. Monthly coverage was estimated using a person-time approach; time to vac

Humoral immune response to 10-valent pneumococcal conjugate vaccine (PCV10) in individuals with type 2 diabetes mellitus.

BACKGROUND: Pneumococcal infections pose a significant health problem in individuals with comorbid conditions such as Type 2 diabetes mellitus. Although pneumococcal vaccines are recommended in individuals with type 2 diabetes, there is a lack of data on the immunogenicity of pneumococcal vaccines in the type 2 diabetes population. This pilot study was therefore developed to determine if the humoral immune response to the 10-valent pneumococcal conjugate vaccine (PCV10) in those with and without type 2 diabetes is comparable. METHODS: A total of 40 (24 with type 2 diabetes and 16 without type…

Intranasal vaccination with probiotic Escherichia coli membrane vesicles displaying pneumococcal capsular polysaccharides elicits mucosal immunity.

A novel vaccine modality is needed to generate both systemic and mucosal immunity in the respiratory tract, where pathogens are most likely to colonize and to initiate infection. Here, we demonstrated that intranasal immunization with probiotic Escherichia coli-derived membrane vesicles displaying serotype-14 pneumococcal capsule (CPS14+MVs) elicited potent IgG responses without adjuvant in mice, which were comparable and significantly superior to those of the injected CPS14+MVs and two licensed pneumococcal vaccines (conjugate and polysaccharide types), respectively. Notably, IgA class-switch…
