[Thromboangiitis obliterans associated with small-vessel vasculitis].

The existence of thromboangiitis obliterans has been questioned for some years. However, it is now accepted as a specific condition within the wide group of vasculitis. We report a patient with this condition, who, as usual, was a male smoker. The illness presented with thrombophlebitis migrans, which is an uncommon initial feature. The patient also had small vessel cutaneous vasculitis, which is an extremely uncommon association. We report the case and discuss some aspects of the etiology and pathogenesis of the disease.

Goodpasture's syndrome associated with pulmonary eosinophilic vasculitis.

Lung hemorrhage and antiglomerular basement membrane (anti-GBM) antibody mediated nephritis define Goodpasture's syndrome. We present the case of a 19-year-old Caucasian woman with unique clinical findings of Goodpasture's syndrome. Our patient initially presented with leukocytoclastic vasculitis of the skin followed by the development of nephritis and lung hemorrhage. An open lung biopsy done prior to diagnosing anti-GBM antibody disease demonstrated an intense eosinophilic vasculitis. Skin vasculitis has only been rarely reported, and to our knowledge this is the first reported case of pulmo…

Systemic symptoms associated with orbital venous vasculitis.

Orbital venous vasculitis has been suggested to cause characteristic periorbital pain in patients with pathologic changes in their orbital phlebograms. The orbital pain is characterized by being unilateral, not shifting side, boring and pressing, but not throbbing, increasing on eye strain, exposure to cold, or weather changes, and resistant to analgesics. It is ameliorated by steroids. Fifty patients with symptoms of orbital venous vasculitis were investigated for other symptoms that could be related to the vasculitis. When the 32 female patients were compared with a randomly selected age- an…

[Vesical neuropathy in systemic vasculitis: 3 cases].

Bladder neuropathy was diagnosed in 3 patients with systemic vasculitis (temporal arteritis: 1 case; periarteritis nodosa: 2 cases). Clinical characteristics were: dysuria, diminished or abolished bladder sensation leading to indolent bladder retention. Urodynamic investigation showed hypotonic and underactive detrusor, increased detrusor compliance, hyposensitive bladder, and/or overactive urethral closure. Needle electrode examination showed signs of denervation of periurethral muscles; sacral evoked latencies were increased, favouring pudenal nerve alterations. Symptoms and urodynamic abnor…

Rheumatoid vasculitis: survival and associated risk factors.

We describe the clinical and laboratory characteristics of 52 patients with rheumatoid vasculitis whose condition was diagnosed at a tertiary care center between 1974 and 1981, and we report their survival and the factors that were associated with decreased survival. The patients with rheumatoid vasculitis had decreased survival in comparison with an age-, sex-, and region-matched general population. Their survival was also decreased in comparison to that of an incidence cohort of community patients with rheumatoid arthritis. In the latter cohort, decreased survival was confined to those patie…

Strongyloides stercoralis hyperinfection masquerading as cerebral vasculitis.

Strongyloides stercoralis hyperinfection is a unique opportunistic infection in which the nematode disseminates widely to cause a multisystem illness. We treated a patient with systemic lupus erythematosus in whom ileus and fever developed and who later lapsed into coma. A xenon Xe 133 cerebral blood flow study showed a global reduction in flow, compatible with CNS vasculitis. The patient's condition failed to improve with high-dose steroid therapy, but he recovered rapidly after Strongyloides larvae were found in stool and sputum and treatment with thiabendazole was begun. We believe that hyp…

Cutaneous leucocytoclastic vasculitis associated with acute sarcoidosis.

A case of acute sarcoidosis associated with the simultaneous appearance of cutaneous leucocytoclastic vasculitis and erythema nodosum is reported. Circulating immune complexes were demonstrated and may have been important in the pathogenesis of both types of skin lesion.

Isolated benign cerebral vasculitis or migrainous vasospasm?

A 39-year-old woman experienced severe headache, epilepsy and rapidly progressive aphasia and hemianopia. Carotid angiograms displayed segmentary narrowing of intracranial arteries as previously described in benign cerebral vasculitis. Her superficial temporal artery was also involved, allowing a biopsy of the abnormal part of the vessel. Microscopical study of this artery was normal. A second carotid angiogram, 14 days later, showed normal intracranial arteries. These findings suggest arterial spasm rather than distal arteritis.

[Cryoglobulinemia, hemorrhagic vasculitis skin changes in hepatopathy].

Cryoglobulins in the blood of patients with liver diseases may be associated with immunologically induced cutaneous vasculitis and haemorrhage. A personal case of chronic cirrhogenic liver disease with progression to liver insufficiency is presented. The picture included IgG, IgA and IgM cryoglobulinaemia and palpable purpura. The main features of primary and secondary or symptomatic cryoglobulinaemia are described, and an account is given of the mostly recent literature on the association between cryoglobulinaemia and liver diseases. Its pathogenesis and possible forms of treatment are also d…

Intestinal vasculitis and lithium carbonate-associated diarrhoea.

A 57-year-old male manic depressive patient is described who presented as an ‘acute abdomen’ whilst receiving treatment with lithium carbonate. Small bowel resection was performed, the histopathology of which revealed local vasculitic changes with thrombosis and subsequent infarction of the ileal wall. The possible aetiological role of lithium in this vasculitis is discussed.

Central nervous system vasculitis in cytomegalovirus infection.

A 51-year-old man received cyclophosphamide, vincristine, procarbazine and prednisone in the treatment of a small-cell undifferentiated lymphoma. Two years later, he developed a rapidly progressive neurological syndrome characterized by a decline in alertness, deafness, blindness and paraplegia. Examination of his eyes revealed severe hemorrhagic chorioretinitis. Leg weakness was thought to be due to transverse myelopathy at a thoracic level. He had a grand mal convulsion and died from terminal bronchopneumonia. Autopsy examination of the eyes revealed sweeping destruction of the retina due to
