A retrospective, multicenter, Scandinavian study of bone grafting of alveolar processes of severely atrophic jaws in combination with implant insertion was conducted with 150 patients. Five different grafting techniques were assessed: local or full onlay; inlay; combination of onlay/inlay grafts; and LeFort I osteotomies. The majority of the patients were treated using a one-stage approach (n = 125) and all had autogenous bone grafts. A total of 781 Brånemark implants were inserted, of which 624 were placed in bone grafts and alveolar bone. Twenty-five patients (17%) dropped out during the fol

Because of the low bone quality in the posterior maxilla, edentulism in this area often results in a resorbed osseous structure and a pneumatized maxillary sinus, which makes dental implant surgery in the posterior maxilla a challenge. Two main surgical approaches are available for the sinus lift procedure: lateral and crestal. Improvement of the maxillary sinus floor elevation technique and increase in predictability are desirable. This article describes an innovative approach to maxillary sinus floor elevation with piezoelectric surgery and hydraulic pressure for xenograft and simultaneous i

BACKGROUND: Horizontal ridge deficiencies in the anterior maxilla frequently compromise ideal implant positioning and esthetic outcomes. This randomized controlled clinical trial evaluated the clinical performance of customized CAD/CAM zirconia membranes compared with conventional collagen membranes for guided bone regeneration (GBR) performed simultaneously with early implant placement.
METHODS: Twenty systemically healthy patients requiring single-tooth replacement in the anterior maxilla underwent early implant placement (4-8 weeks after tooth extraction) combined with simultaneous contour

The autogenous shell technique offers strong biological performance by combining the mechanical stability of cortical bone with the rapid revascularization of particulate autograft. However, the method is highly technique-dependent, particularly when performed concurrently with implant placement, as both the bone shell and implant require spatially accurate positioning. A technique involving a fully digital protocol delivering controlled, predictable, and prosthetically guided shell grafting with simultaneous implant placement is described.

The sinus elevation procedure is a predictable technique to allow for placement of dental implants in the posterior maxilla when the height of the alveolar ridge is limited. The sinus elevation can be performed by various techniques. In the crestal approach, bone graft is utilized to hydraulically elevate the sinus membrane through an osteotomy prepared in the alveolar crest. The implant can be placed either immediately or at a later surgery. This is a case report of an oroantral communication that developed as a complication to a sinus elevation surgery performed with the crestal approach. A

This report details a case of multiple implant failure (so-called 'cluster phenomenon') in a patient with ankylosing spondylitis and considers some factors for implant failure including reduced bone density. It highlights the relationship between ankylosing spondylitis and osteoporosis although no direct link is made between these conditions and the implant failures in this case.

Reverse piezoelectricity is currently used in Dentistry for ultrasonic scaling devices. Using this technology, increasingly more powerful ultrasonic surgical lancets were developed within a few years, and these new tools have provided many practical solutions in oral and maxillofacial surgery. This article reviews the basic principles of piezosurgery, and its numerous clinical applications: in oral surgery (atraumatic extractions, wisdom teeth exposure, periapical cyst debridement, pre-prosthetic surgery), more specifically in periodontal (root planning and bone remodeling, crown lengthening)

The short and long term adverse effects after ridge reconstruction using hydroxylapatite (HA) are presented in this study. The HA was inserted using a modified tunnelling technique, followed by a lowering of the floor of the mouth and a vestibuloplasty using split thickness skin graft, 4-6 weeks later. The study comprised 637 patients followed for a period of 1 to 10 years (mean 6.0+/-2.6 years). Major loss of HA was seen in 17 patients (2.7%). Donor site visibility (skin graft) appeared to improve greatly over the years from 29.2% to 8.8% at the latest follow-up. Neurosurgery deficits also im

A technique is described that is designed to augment the severely atrophied edentulous mandible by the use of a bone or bone-hydroxylapatite filled mesh tray attached to the inferior border. This method permits the intraoral environment to remain unchanged, causing neither a decrease of intermaxillary distance nor placement of graft material on the ridge crest, which would offer a less stable and predictable base for acceptance of a subperiosteal implant. Lower facial length becomes augmented, inferior labial posture is improved, redundant cervical tissue is eliminated, and masticatory functio

Immediate implant placement in the esthetic area is a challenging surgical procedure. In the anterior maxilla, the esthetic outcome and the tissue stability over time are highly emphasized. A thorough preoperative analysis of the local site anatomy is essential to choosing the most suitable surgical and prosthetic protocol. Due to the heterogenous data from the literature, a diagnostic algorithm was developed to assist the clinician in the decision-making process for single-tooth implant rehabilitation in the anterior maxilla. The treatment options were based on the predictability of implant p
