Disease-oriented approach to botulinum toxin use.

Botulinum toxin (BoNT) has been used for over a quarter of century for the treatment of well over 100 different indications. Many of the symptoms for which BoNT has been found to be effective occur in a variety of neurological disorders. One neurodegenerative disorder in which BoNT has been used extensively to treat various symptoms is Parkinson's disease (PD). This review will highlight the following therapeutic applications of BoNT in conditions associated with PD: limb dystonia, blepharospasm and lid apraxia, bruxism, cervical dystonia (anterocollis), camptocormia, hand and jaw tremor, rigi…

Botulinum toxin in the treatment of tremors, dystonias, sialorrhea and other symptoms associated with Parkinson's disease.

Botulinum toxins are an effective treatment modality for a growing number of neurologic conditions. Although there has been varied interest and success in their use, they have been studied for a variety of conditions associated with Parkinson's disease. Conditions reviewed in this paper include hand and jaw tremor, dystonia, blepharospasm and apraxia of eyelid opening, bruxism, camptocormia, freezing of gait, sialorrhea and constipation. We will make comments when applicable on our unique experience with botulinum toxin in these conditions. Other conditions associated with Parkinson's disease,…

Anesthesia with EMLA cream for botulinum A toxin injection into eyelids.

We investigated the efficacy of an eutectic mixture of local anesthetics (EMLA) in reducing the pain of a botulinum toxin injection into eyelids. 17 patients with facial dyskinesia (9 blepharospasm and 8 hemifacial spasm) who had been treated regularly with botulinum toxin injections received EMLA cream and placebo. The pain was assessed by the ophthalmologist and the patient. Injection with EMLA cream showed lower pain scores (mean 1.82 +/- 2.13, median 1) than injection with placebo (8.76 +/- 2.17; p = 0.0001). Percutaneous anesthesia induced by EMLA cream is an effective and safe method whi…

[New applications in maxillofacial surgery: Local injections with botulinum toxin type A].

Intramuscular injections of botulinum neurotoxin type A cause reversible chemodenervation and subsequent paralysis by blocking the presynaptic release of acetylcholine. Botulinum toxin type A has emerged as the most effective form of symptomatic treatment for abnormabilities in muscle movement (blepharospasm, hemifacial spasm, torticollis) and has been approved for use in these conditions. First results in the treatment of patients suffering from oromandibular dystonia, myogenic craniomandibular dysfunction and recurrent dislocation of the temporomandibular joint are presented. In most cases, …

Botulinum toxin as a therapeutic agent.

Botulinum toxin is a presynaptic neuromuscular blocking agent that, when injected intramuscularly in minute quantities, can produce selective muscle weakness. This property is employed therapeutically to provide symptomatic relief in conditions related to excessive muscle activities in strabismus, blepharospasm, hemifacial spasm, cervical dystonia, spasmodic dysphonia (adductor type), and jaw closing dystonia. It is investigational for a long list of medical conditions. It is a marketed drug in a number of countries in the world, but its use has only been approved by different regulatory agenc…

[Rapid eye fatigue--causes and therapy].

Tired eyes even after a short period of strain are well known as asthenopia. This term derives from a Greek expression meaning 'weak eye'. The following reasons are to be taken into account: uncorrected refractional errors such as hyperopia and astigmatism, accommodative and fusional defects and heterophoria, or combinations there of. With many cases of asthenopia the clinical symptomatology may resemble an organic eye disease or an affection of the brain [dry eye, Graves' disease, myasthenia, special form of essential blepharospasm, neurogenic latent eye muscle palsies, narcolepsy]. It is the…

[Spitting cobra ophthalmia (Naja nigricollis)].

The spitting cobra (Naja nigricollis) can eject its venom into its adversary's eyes from a distance of several meters. This causes an immediate and painful conjunctival infection, followed by blepharospasm, corneal and conjunctival edema, and erosion. Corneal ulcer, in some cases with perforation, hypopyon and endophthalmitis are the results. The present paper reports on a 35-year-old patient from Hemkoa (South Upper Volta) whose eyes had been affected by the venom of a spitting cobra 5 years previously. The patient had meanwhile lost his sight as a result of bilateral sclerosing keratitis. Th…

Acute laryngeal and bronchial obstruction after CS (o-chlorobenzylidenemalononitrile) gas inhalation.

CS gas (o-chlorobenzylidenemalononitrile) is one of the most commonly used riot agents. It can create excessive tearing, conjunctivitis, uncontrolled blinking (blepharospasm) and a sensation of burning and pain at initial exposure. Pulmonary edema (ARDS) and/or diffuse airway lesions on human would be lethal after CS inhalation. We report a case with acute laryngeal and bronchial obstruction due to vocal cord edema and extensive crusting at glottic level, trachea and bronchi. The CS gas was sprayed in a 6 x 6 m(2) closed room, and she was exposed to increased concentration of the gas for 10 s.…

Brueghel Syndrome or Meige Syndrome? Two Sides of a Same Disease.

Different eponyms such as "Wood syndrome," Meige syndrome, "Brueghel syndrome," "Blepharospasm plus syndrome" have been used to describe segmental craniocervical dystonias. These facial and/or oromandibular movement disorders are characterized by muscle contractions and spasms involving eyes, facial region, and sometimes pharynx, jaw, floor of the mouth, and tongue. The pathophysiology of craniocervical dystonia is poorly understood, but abnormal plasticity and impaired inhibition are suspected. Injection of botulinum toxin appears to be the best therapeutic option for treating segmental crani…

Non-motor symptoms in patients with adult-onset focal dystonia: Sensory and psychiatric disturbances.

Dystonia is characterized by the presence of involuntary muscle contractions that cause abnormal movements and posture. Adult onset focal dystonia include cervical dystonia, blepharospasm, arm dystonia and laryngeal dystonia. Besides motor manifestations, patients with focal dystonia frequently also display non-motor signs and symptoms. In this paper, we review the evidence of sensory and psychiatric disturbances in adult patients with focal dystonia. Clinical studies and neurophysiological investigations consistently show that the sensory system is involved in dystonia. Several studies have a…

[Catamnesis of patients sustaining the acute period of epidemic encephalitis in Primor'e].

The authors studied 20 patients (12 males and 8 females) from 5--40 years of age who had acute forms of epidemic encephalitis. Progressive development was marked only in 2 patients (in one case typical parkinsonism, in the other--a hallucinatory-paranoid schizophrenoform syndrome). In the majority of the cases neurological symptoms of the acute period gradually subsided up till a full disappearance (in 8 out of 20 cases). Among the residual of symptoms of contemporary epidemic encephalitis special attention should be drawn to blepharoclonus and blepharospasm, converging both monocular nistagmu…

Acupuncture for Meige syndrome: A case report.

INTRODUCTION: Meige syndrome (MS) is a cranial dystonia disorder characterized by blepharospasm and oromandibular dystonia. Current treatments include botulinum toxin therapy, oral medications, and surgical interventions. Here, we present a case that the patient's symptoms improved significantly after treatment with acupuncture. CASE DESCRIPTION: A 72-year-old woman came to our acupuncture clinic with eyelid twitching and jaw muscle tension. She described being diagnosed with MS two years ago due to the same symptoms, and her symptoms gradually worsened over the past six months. After 4 weeks…

Facial pain in a case of cranial dystonia: a case report.

A 76-year-old man had shown sustained excruciating facial pain in the maxillary region for more than 30 years. Since he was suffering from blepharospasm, facial electromyography was performed and revealed a perioral dystonia. This possible cause of facial pain might have been overlooked had dystonia not been considered and electromyographical studies performed. Repeated intramuscular perioral injections of botulinum toxin brought about complete pain relief. This case shows that involuntary activity of facial muscles can cause a severe chronic pain syndrome. Possible mechanisms include irritati…

The effect of nocturnal physiological sleep on various movement disorders.

Thirty-one subjects affected by different movement disorders underwent polygraphic and videotape monitoring during nocturnal sleep, to assess movement patterns during the night. It was possible to distinguish two categories of disorders according to their pattern of movements. In the largest group (Meige's syndrome, blepharospasm, amyotrophic choreoacanthocytosis, Tourette syndrome, tonic foot, hemiballism) abnormal movements were still present during sleep, but decreased in frequency and amplitude in all stages. The second group presented three syndromes (nocturnal paroxysmal dystonia, noctur…

Ultraviolet Keratitis: From the Pathophysiological Basis to Prevention and Clinical Management.

Ultraviolet keratitis is caused by the toxic effects of acute high-dose ultraviolet radiation (UVR) reflecting the sensitivity of the ocular surface to photochemical injury. The clinical syndrome presents with ocular pain, tearing, conjunctival chemosis, blepharospasm, and deterioration of vision typically several hours after exposure, lasting up to 3 days. Mountaineers, skiers, and beach recreationalists are particularly at risk to suffer from ultraviolet (UV) keratitis as the reflectivity of UVR in these environments is extremely high. The aim of this review is to raise awareness about the p…
