Successful implantation of Berlin Heart INCOR system after Bentall/De Bono operation.

Berlin Heart INCOR system is a novel left ventricle assist device (LVAD), which generates a laminar blood flow. Its cannulas are implanted in the heart apex and the ascending aorta. The present paper reports a case of successful implantation of the Berlin Heart INCOR system after a Bentall/De Bono operation. In this case the outflow cannula of the device was implanted in the Sorin Carbonart 27/30 conduit used in the previous operation. After the implantation no unusual events like thrombosis, anastomosis malfunction, and so forth, were observed. On the 100th day after the implantation the pati

Valve replacement in children: a challenge for a whole life.

Valvular pathology in infants and children poses numerous challenges to the paediatric cardiac surgeon. Without question, valvular repair is the goal of intervention because restoration of valvular anatomy and physiology using native tissue allows for growth and a potentially better long-term outcome. When reconstruction fails or is not feasible, valve replacement becomes inevitable. Which valve for which position is controversial. Homograft and bioprosthetic valves achieve superior haemodynamic results initially but at the cost of accelerated degeneration. Small patient size and the risk of t

Minimally invasive surgical aortic valve replacement through a right anterolateral thoracotomy.

In the field of minimally invasive surgical aortic valve replacement, the right anterolateral minithoracotomy is an approach that offers very limited procedural trauma. Our method of right anterolateral minithoracotomy uses femoral percutaneous cannulation, a high-definition thoracoscope and a transthoracic cross-clamp. It allows a minimized skin incision and full preservation of the sternum, sternocostal cartilages, and ribs. The patient we present in this video tutorial is a low-risk male with severe bicuspid aortic valve stenosis. We provide detailed insight into our preoperative planning a

Heart surgery with cardiopulmonary bypass in pregnant women.

OBJECTIVE: Definite data in heart surgery with extracorporeal circulation during pregnancy is limited. This report analyzes our experience in this area. METHODS: Fifteen women underwent open heart surgery under cardiopulmonary bypass during pregnancy at our institution between 1972 and 1998. Surgical procedures included valve replacement in 13 patients (12 mitral, 1 aortic), declotting of a tilting disk mitral prosthesis in one and closure, of an atrial septal defect in the remaining patient. RESULTS: Thirteen patients were in New York Heart Association functional class III to IV and were op

[Konno's operation for residual aortic stenosis following Nick's operation--a case report].

A 43-year-old woman successfully underwent the Konno's operation as a reoperative procedure for residual aortic stenosis following the Nicks' operation. The patient had had congenital valvular and subvalvular aortic stenosis and a patent ductus arteriosus previously divided. At age 33, she had undergone the Nicks' operation (posterior ring enlargement) and aortic valve replacement with a 19 mm Bicer valve prosthesis 9 years 6 months prior to the reoperation. The preoperative left ventricular-aortic pressure gradient was 120 mmHg which decreased but remained about 60 mmHg after the Nicks' proce

In-Graft Endovascular Stenting Repair for Supravalvular Stenosis From Aortic Rupture After Balloon-Expanding Transcatheter Aortic Valve Implantation.

An 81-year-old man with high-grade aortic valve stenosis and status post-coronary artery bypass grafting and supracoronary replacement of the ascending aorta was referred for transcatheter aortic valve implantation. He was in New York Heart Association class III and had dyspnea. After appropriate screening, we implanted a 29-mm SAPIEN XT valve (Edwards Lifesciences, Irvine, CA USA) through a transapical approach because of severe peripheral arterial occlusive disease. Postinterventional aortography revealed correct positioning and function of the valve and free coronary ostia but contrast extr

Reoperative cardiac valve surgery: a multivariable analysis of risk factors.

From December 1983 to March 1992, of 1650 patients who underwent a cardiac valve procedure 41 (2.5%) underwent reoperation on prosthetic heart valves. The indications for reoperation were: structural degeneration (13 patients, 32%), non-structural dysfunction (nine patients, 22%), prosthetic valve endocarditis (seven patients, 17%), valve thrombosis (four patients, 10%), native valve pathology (three patients, 7%), aortic ascending pathology (five patients, 12%). The hospital mortality rate for mitral valve reoperation was 12% (two of 17) and for reoperation on the aortic prosthesis 26% (five

Early and long-term outcome of aortic valve replacement with homograft versus mechanical prosthesis--8-year follow-up study.

BACKGROUND: Aortic valve disease is an important and frequent clinical problem with a mortality rate as high as 50-80% in a 5-year natural history of patients with severe aortic valve disease. Biological or mechanical prosthesis implantation is the only way to improve prognosis. HYPOTHESIS: The aim of our study was to assess the clinical outcome of aortic valve replacement according to the underlying valve pathology and the type of replacement device, that is, aortic homografts versus mechanical prostheses. METHODS: The study group consisted of 143 patients with a mean follow-up period of 4.

Long-term results of surgery on aortic valve diseases.

At the Heart Institute of Japan, Tokyo Women's Medical College, 474 aortic valve replacements (AVR) are performed using various kinds of prosthetic devices during the period of 1965, 5-1978, 12. Most popular valve prosthesis was Björk-Shiley valve totalling 215. 56 cases were subjected to study of more than 5 years follow up. They carried operative mortality of 16.1% and late mortality of 7.1%. Thromboembolism was 5.4%. 3 patients delivered 4 normal full term babies. There were 4 reoperations for aortic orifice with successful outcome. Cardiothoracic ratio, left ventricular strain pattern on E

Utility of 18F-FDG PET/CT Imaging in Diagnosing Pulmonary Prosthetic Valve Endocarditis in a Pediatric Patient.

A 15-year-old girl with a history of complex congenital heart disease and prior pulmonary valve replacement presented with suspected endocarditis. PET/CT imaging with 18F-FDG was performed to evaluate the potential presence of intracardiac vegetations after previously inconclusive findings from CT angiography, transthoracic echocardiography, and transesophageal echocardiography. PET/CT detected heterogeneous, asymmetric, increased 18F-FDG uptake in the region of the pulmonary valve prosthesis, typical for infection, and confirmed diagnosis of bacterial infective endocarditis. [Aortic valve replacement in a patient with chronic idiopathic thrombocytopenic purpura].

We reported a case of aortic valve replacement in a patient with chronic idiopathic thrombocytopenic purpura (ITP). We used high-dose intravenous gammaglobulin therapy (400 mg/kg/day) for five days to increase the platelet prior to the operation. The number of platelet was 8-10 x 10(4)/microliters at admission and it was increased to 23 x 10(4)/microliters before surgery. Aortic valve was replaced to SJM#21A prosthesis using cardiopulmonary bypass. The platelet was transfused after the bypass. Perioperative hemorrhage was moderate and postoperative course was uneventful. We believe that high-d

Bioprosthetic aortic valve diameter and thickness are directly related to leaflet fluttering: Results from a combined experimental and computational modeling study.

OBJECTIVE: Bioprosthetic heart valves (BHVs) are commonly used in surgical and percutaneous valve replacement. The durability of percutaneous valve replacement is unknown, but surgical valves have been shown to require reintervention after 10 to 15 years. Further, smaller-diameter surgical BHVs generally experience higher rates of prosthesis-patient mismatch, which leads to higher rates of failure. Bioprosthetic aortic valves can flutter in systole, and fluttering is associated with fatigue and failure in flexible structures. The determinants of flutter in BHVs have not been well characterized

A serious complication late after mitral valve replacement: left ventricular rupture with pseudoaneurysm.

We report a left ventricular pseudoaneurysm, an unusual complication of mitral valve replacement (MVR) in a 65-year-old man who had undergone coronary artery bypass grafting and MVR surgery two years ago. The patient was referred to our clinics because of progressive exertional dyspnea and palpitations. Transthoracic and transesophageal echocardiography showed a large pseudoaneurysm of the posterolateral left ventricular wall and computerized tomographic angiography (CTA) demonstrated the location and the size of the pseudoaneurysm and its neck. Urgent surgical repair was accomplished without
