[Clinical research progress in non-invasive positive pressure ventilation in 2023].

Non-invasive positive pressure ventilation (NPPV) is a widely used method of providing respiratory support in a variety of clinical settings, including emergency departments, general wards, and intensive care units. The relevant research articles on NPPV published between 1st October 2022 and 30th September 2023 were retrieved from Medline and reviewed. In the management of acute respiratory failure (ARF) associated with COVID-19, studies have highlighted the significant influence of regional economic status on the choice of respiratory support strategies. It has been observed that NPPV is mor

Effects of highly overdosed indomethacin in a preterm infant with symptomatic patent ductus arteriosus.

We describe a preterm infant with severe idiopathic respiratory distress syndrome (iRDS, hyaline membrane disease) who needed artificial ventilation with high inspiratory pressure, high frequencies, 100% oxygen and developed a symptomatic patent ductus arteriosus (sPDA) in the course of the disease. The infant was given indomethacin to induce constriction of sPDA. Due to an error in drug dilution the patient received a 100-fold overdose of indomethacin. Compared to the normal study protocol side-effects such as renal failure were not observed probably due to sufficient fluid intake and good cl

Frequency of endotracheal suctioning for the prevention of respiratory morbidity in ventilated newborns.

BACKGROUND: Endotracheal suctioning consists of the mechanical aspiration of pulmonary secretions from the endotracheal tube (ETT) to prevent obstruction. The optimal frequency of ETT suctioning has not been defined. OBJECTIVES: To determine the effect of specific ordered frequency of ETT suctioning ('as scheduled') versus ETT suctioning only in case of indications ('as needed') and of more frequent ETT suctioning versus less frequent ETT suctioning on respiratory morbidity in ventilated newborns. SEARCH METHODS: We used the standard search strategy of the Cochrane Neonatal Review group to s

Congenital diaphragmatic hernia treated by perinatal stabilization.

The delay of surgery in conjunction with high-frequency oscillation ventilation combined with preoperative stabilization is a standard treatment for congenital diaphragmatic hernia neonates in the high-risk group. A new protocol of perinatal stabilization with this preoperative stabilization has been developed at the University of Tokyo Hospital. The most important components of this method of perinatal stabilization are cesarean section and the injection of morphine and pancuronium through the umbilical vein prior to ligation of the umbilical cord. In order to benefit from this treatment, con

Neurally adjusted ventilator assist in very low birth weight infants: Current status.

Continuous improvements in perinatal care have resulted in increased survival of premature infants. Their immature lungs are prone to injury with mechanical ventilation and this may develop into chronic lung disease (CLD) or bronchopulmonary dysplasia. Strategies to minimize the risk of lung injury have been developed and include improved antenatal management (education, regionalization, steroids, and antibiotics), exogenous surfactant administration and reduction of barotrauma by using exclusive or early noninvasive ventilatory support. The most frequently used mode of assisted ventilation is

Combination of PGE1 and Pulmonary Vasodilator Therapy in Managing a Challenging Case of Severe PAH Secondary to CDH.

Congenital diaphragmatic hernia (CDH) is a rare disease, which affects 1 in 2,500 newborns. Congenital diaphragmatic hernia can interfere with the normal development of the pulmonary parenchyma and vascular bed, and in severe cases, it can lead to the development of severe pulmonary arterial hypertension (PAH) and right ventricular failure. We present a neonate with CDH who developed severe PAH and right ventricular dysfunction and was managed with a unique strategy combining venoarterial extracorporeal membrane oxygenation, prostaglandin E1, and a variety of PAH therapies.

Aminophylline may stimulate respiration in rats by activation of dopaminergic receptors.

In intact young rats anesthetized with halothane, aminophylline produces an increase in respiratory minute ventilation due primarily to an increase in respiratory frequency. Although the simulation of respiration by inhalation of 10% CO2 is augmented only at high doses of aminophylline, the response to CO2 is increased at doses as low as 3 mg/kg of aminophylline. Division of the ninth cranial nerve together with removal of the carotid body did not alter the response, whereas vagotomy changed the response from one of increased frequency to an increase in tidal volume. Both awake and anesthetize

Activating effect of nasal and oral hyperventilation on epileptic electrographic phenomena: reflex mechanisms of nasal origin.

In experiments on animals, airflow through the nasal cavity elicits rhythmic synchronized activity that can trigger and/or elicit epileptic electrographic activities in the limbic structures of the brain. This could be demonstrated in studies of lower vertebrates (frogs and turtles). In the turtle the elicited paroxysmal activity often had the shape of regular high-voltage activity in the theta-frequency range (average frequency, 4.1 Hz). It was further proven in clinical experiments that nasal deep breathing with a closed mouth effectively activates epileptic electrographic phenomena of a tem

Intratracheally administered corticosteroids improve lung function in meconium-instilled rabbits.

Local administration of corticosteroids may diminish acute lung injury associated with meconium aspiration. Budesonide was given intratracheally in 2 doses of 0.25 mg/kg each by means of inpulsion effect of high-frequency jet ventilation 0.5 and 2.5 hours after meconium instillation to oxygen-ventilated adult rabbits. Within 5 hours after the first dose, budesonide significantly improved gas exchange and decreased right-to-left pulmonary shunts, central venous pressure, and ventilatory pressures. In addition, budesonide reduced the meconium-induced lung edema formation, airway hyperreactivity

A Quality Improvement Initiative to Reduce Unnecessary Screening Chest Radiographs in a Pediatric ICU.

BACKGROUND: The Critical Care Societies Collaborative included not ordering diagnostic tests at regular intervals as one of their Choosing Wisely initiatives. A reduction in unnecessary chest radiographs (CXRs) can help reduce exposure to radiation and eliminate health care waste. We aimed to reduce daily screening CXRs in a pediatric ICU (PICU) by 20% from baseline within 4 months of implementation of CXR criteria. METHODS: All intubated patients in the PICU were included in this quality improvement project. Patients with tracheostomies were excluded. We developed criteria delineating which

Alternative treatment may lower the need for use of extracorporeal membrane oxygenation.

Access to artificial surfactant and high frequency oscillatory ventilation (HFO) in Japan seems to affect the actual indications for extracorporeal membrane oxygenation (ECMO). The relation between the methods of treatment and survival and/or neurological sequelae of 27 neonates with severe respiratory failure who would have met the US ECMO entry criteria in the Neonatal Intensive Care Unit of National Children's Hospital, Tokyo between January 1988 and May 1992 were retrospectively analyzed. Out of 27 neonates, conventional treatment including artificial surfactant was successful in 6 cases (

[The use of bronchofibroscopy in intensive care].

The bronchofibroscope has enabled the exploration of new avenues of diagnosis and treatment in intensive-care medicine. Its proper use depends on correct indication and conscientious and careful maintenance. This paper presents, first of all, the most essential technical and maintenance measures required in handling the bronchofibroscope. This is followed by a detailed description of the therapeutic use of the instrument in bronchial lavage with instillation of salbutamol, and of the more recent methods of broncho-alveolar lavage and high-frequency ventilation. Finally, indications for broncho…

Giant Gastroschisis with Complete Liver Herniation: A Case Report of Two Patients.

INTRODUCTION: There are no reported survivors of gastroschisis with complete liver herniation. We describe a case report of two patients, one of whom survived. CASE #1: The patient was born with gastroschisis and herniation of the entire liver. Along with silo placement, the abdominal fascia was attached to an external traction system for growth. Complete closure was achieved at 5 months. Due to pulmonary hypoplasia, high-frequency ventilation was required. The patient is doing well, on a home ventilator wean, at 20 months. CASE #2: The patient was born prematurely with gastroschisis, total …

Congenital diaphragmatic hernia.

Congenital diaphragmatic hernia occurs in approximately 1 in every 2500 live births and is associated with a reported mortality of almost 35% in live-born patients and a higher mortality when in utero deaths are counted. Ventilator-induced lung injury, pulmonary hypoplasia, and other associated anomalies account for the high death rate. Numerous adjunctive measures have been used to treat these patients. Inhaled vasodilators (nitric oxide), intravenous vasodilators, and fetal therapy have no proven benefit. While animal models of congenital diaphragmatic hernia are surfactant deficient, contro

Extensive posterior-lateral tracheal laceration complicating percutaneous dilational tracheostomy.

BACKGROUND: An extensive posterior-lateral longitudinal tracheal laceration is an uncommon but serious complication of percutaneous dilational tracheostomy (PDT). We report the successful management of three ventilator-dependent patients whose percutaneous tracheostomy was complicated by an extensive longitudinal posterior-lateral tracheal laceration requiring operative repair. METHODS: A retrospective review of 134 cases of PDT with concurrent bronchoscopy was performed between April 1997 and July 1999 and compared with a review of 124 cases of open tracheostomy. Tracheal lacerations were pr

Effect of respiratory apparatus on timing and depth of breathing in man.

The effects on breathing of (1) nose-clips, with and without mouthpieces, (2) tube breathing with incremental dead spaces up to 600 ml, and (3) a Venturi mask with a high air inflow reducing the effective dead space to zero were measured with respiratory inductance plethysmography. The effective dead spaces of the lengths of tubing were 25% smaller than their water-displacement volumes. The principal response to the increase in dead space was an increase in VT without any change in frequency or timing. The increase in VE was 16% greater than the ventilation of the effective dead space. The fac

Ventilatory response to hyperoxia in newborn rats born in hypoxia--possible relationship to carotid body dopamine.

1. The influence of postnatal hypoxia on regulation of breathing and turnover rate of carotid body dopamine was examined in newborn rats. The percentage change in frequency, tidal volume and ventilation elicited by transient hyperoxia was assessed by flow plethysmography in unanaesthetized pups. The alteration in ventilation was taken as an index of peripheral chemoreceptor activity. 2. The rats were born and reared in hypoxia. The inspired oxygen fraction (FI,O2) was 0.12-0.14 until 2 days after delivery when the rats were placed into room air and the ventilatory chemoreflex was tested. At 4

Accuracy and Interpretation of Transcutaneous Carbon Dioxide Monitoring in Critically Ill Children.

OBJECTIVES: Transcutaneous carbon dioxide (Tc co2 ) monitoring can noninvasively assess ventilation by estimating carbon dioxide ( CO2 ) levels in the blood. We aimed to evaluate the accuracy of Tc co2 monitoring in critically ill children by comparing it to the partial pressure of arterial carbon dioxide (Pa co2 ). In addition, we sought to determine the variation between Tc co2 and Pa co2 acceptable to clinicians to modify patient care and to determine which patient-level factors may affect the accuracy of Tc co2 measurements. DESIGN: Retrospective observational cohort study. SETTING: Sing
