Extracorporeal Membrane Oxygenation for Cardiac Indications in Adults: A Health Technology Assessment.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is a rescue therapy used to stabilize patients with hemodynamic compromise such as refractory cardiogenic shock or cardiac arrest. When used for cardiac arrest, ECMO is also known as extracorporeal cardiopulmonary resuscitation (ECPR). We conducted a health technology assessment of venoarterial ECMO for adults (aged ≥ 18 years) with cardiac arrest refractory to conventional cardiopulmonary resuscitation (CPR) or with cardiogenic shock refractory to conventional medical management (i.e., drugs, mechanical support such as intra-aortic balloo

Acute control of pulmonary regurgitation with a balloon "valve". An experimental investigation.

Operations for certain congenital cardiac lesions can produce pulmonary regurgitation. Pulmonary regurgitation contributes to right ventricular dysfunction, which may cause early postoperative morbidity and mortality. To ameliorate the problems of pulmonary regurgitation during the early postoperative period, we evaluated a method for its acute control. Complete pulmonary valvectomy was performed utilizing inflow occlusion in eight sheep. A catheter with a 15 ml spherical balloon was positioned in the pulmonary arterial trunk; its inflation and deflation were regulated by an intra-aortic ballo

The effect of the single aortic cross-clamp technique on cardiac and cerebral complications during coronary bypass surgery.

Cardiac and cerebral events during coronary artery bypass graft (CABG) surgery remain a major cause of morbidity and mortality. Efforts made to reduce these events will have a significant impact on CABG results. The objective of this study was to examine our results in 394 patients undergoing primary CABG using the single clamp technique that probably has better myocardial and cerebral protective properties than the conventional technique of partial aortic occlusion. Age range was 35 to 88, mean of 66 years, and 168 (43%) were > or = 70 years of age; 121 (31%) were females, 118 (30%) were diab

Diagnostic challenge of mitral regurgitation caused by concomitant coronary obstruction and dynamic left ventricular outflow tract obstruction after transcatheter aortic valve replacement: a case report.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is an established therapy for severe aortic stenosis (AS), but rare complications can rapidly become life-threatening, particularly in anatomically high-risk patients. CASE SUMMARY: A 94-year-old woman with very severe AS underwent transfemoral TAVR using a self-expanding valve. After transcatheter heart valve implantation, she developed haemodynamic collapse with right coronary artery obstruction and severe mitral regurgitation (MR). At that time, the aetiology of the severe MR could not be clearly identified. Although intra-aortic ba

Predictors of Mortality in Venoarterial Extracorporeal Membrane Oxygenation Regardless of Early Left Ventricular Unloading: A National Experience.

OBJECTIVE: The use of an intra-aortic balloon pump (IABP) has been suggested to unload the left ventricle while on venoarterial extracorporeal membrane oxygenation (VA-ECMO) for cardiogenic shock (CS), leading to possibly improved in-hospital mortality. However, the predictors of mortality on dual mechanical circulatory support have not yet been evaluated, especially in real-world clinical settings. Therefore, a case-control study was conducted to determine the rate of all-cause mortality associated with VA-ECMO use regardless of left ventricular (LV) unloading, and with early LV unloading in
