Prediction of recurrence based on the patellofemoral morphological profile and demographic factors in first-time and recurrent dislocators.

INTRODUCTION: Prediction of recurrence in first-time patellar dislocation is an unsolved mystery. The purpose of our study is to compare patellar instability patients and normal control groups with anatomical risk factors and validation of newer parameters patello-trochlear index (PTI) and tibial tuberosity-posterior cruciate ligament (TT-PCL) and also to find the significant risk factors that help to predict the recurrence of dislocation in first-time dislocators. METHODS: This is a comparison study between 50 normal individuals as a control population (group-1) and 94 patients with patellar

Closed subtalar dislocation with non-displaced fractures of talus and navicular: a case report and review of the literature.

Closed subtalar dislocations associated with talus and navicular fractures are rare injuries. We report on a case of a 43-year-old builder man with medial subtalar dislocation that was further complicated by minimally displaced talar and navicular fractures. Successful closed reduction under general anesthesia was followed by non-weight bearing and ankle immobilization with a below-knee cast for 6 ;weeks. At 3 years post-injury, the subtalar joint was stable, the foot and ankle mobility was in normal limits and the patient could still work as a builder. However, he complained for occasionally

Surgical management of hip dislocation in children with arthrogryposis multiplex congenita.

Arthrogryposis multiplex congenita (AMC) is a rare disease with multiple joint contractures. It is widely believed that bilaterally dislocated hips should not be reduced since movement is satisfactory and open reduction has had poor results. Since 1977 we have performed a new method of open reduction using an extensive anterolateral approach on ten hips in five children with AMC. The mean age at surgery was 31.5 months (17 to 64) and the mean follow-up was 11.8 years (3.8 to 19.5). At the final follow-up all children walked without crutches or canes. Two managed independently, one required a l

Short and mid term results of revision total knee arthroplasty with Global Modular Replacement System.

BACKGROUND: The original knee megaprostheses with fixed or rotating hinge articulation were custom made and only used for reconstruction of the knee following distal femoral or proximal tibial tumor resections. The aim of the study was to analyze the short- and mid-term results of revision total knee arthroplasty with Global Modular Replacement System (GMRS) used in difficult situations not amenable to reconstruction with standard total knee replacement implants. MATERIALS AND METHODS: Nine patients (9 knees) were treated with this comprehensive modular implant system, with a mean age of 73.7

[Emergencies in the knee joint surgery].

Anatomy of the knee joint is one of the most complicated when compared to other joints in the human body. Due to high demands and strong forces acting on the knee joint, it is also one of the most injured joints. Every structure in the joint can be damaged and some of the damages demand earlier treatment. Except intraarticular fractures, stress fractures of the proximal tibia, as a result of lower extremity malalignment, need earlier treatment to prevent the dislocation of the fragments. Damage to the articular cartilage needs earlier treatment due to its tendency to involve wider area of the

An original closed reduction technique for acute shoulder dislocation: the wrist-clamping and shoulder-lifting.

BACKGROUND: Acute anterior shoulder dislocation is one of the most common injuries in emergency medicine and orthopaedics. The aim of this study is to introduce a new closed reduction technique: the wrist-clamping and shoulder-lifting, for manual reduction of acute anterior shoulder dislocation. PATIENTS AND METHODS: The patient is instructed to a sitting position, the doctor hold the wrist of the arm with both hands, slowly rotated the arm to 90-degree of abduction and 60-degree of external rotation with gentle strength. After the shoulder muscles were relaxed by continuous traction, the wri

[Medial subtalar dislocation--case report].

INTRODUCTION: Subtalar dislocation (SI) is a term that refers to an injury in which there is dislocation of the talonavicular and talocalcanear joint, although the tibiotalar joint is intact. CASE OUTLINE: A case of medial subtalar dislocation as a result of basketball injury, so-called "basketball foot", is presented. Closed reposition in i.v. anaesthesia was performed with the patient in supine position and a knee flexed at 90 degrees. Longitudinal manual traction in line of deformity was carried out in plantar flexion. The reposition continued with abduction and eversion simultaneously inc

Patellar Tendon-Lateral Trochlear Ridge Distance: A Novel Measurement of Patellofemoral Instability.

BACKGROUND: Abnormalities in the trochlea-patella-tibia relationship have been shown to be risk factors for recurrent patellofemoral instability, although no current measurements quantify patellar containment in the trochlea. Standard measurements, such as tibial tubercle-trochlear groove (TT-TG) distance, do not account for the containment of the patella by the trochlea. Our goal was to develop a measurement to assess how well the trochlea contained the extensor mechanism. HYPOTHESIS: A novel measurement describing the amount of the patellar tendon lateral to the lateral trochlear ridge (PT-

The Reverse Biceps Femoris Muscle Flap: An Alternative Option for Soft-tissue Coverage Around the Popliteal Fossa.

Soft-tissue reconstruction around the popliteal fossa is challenging, particularly when traditional local flap options are unavailable due to trauma or anatomic constraints. We report the case of an 81-year-old woman who sustained Gustilo-Anderson type IIIB distal femoral and tibial plateau fractures with knee dislocation and soft-tissue degloving. Following initial management with a knee-spanning external fixation and a lateral gastrocnemius flap, she developed a pressure wound over the lateral femoral condyle. She underwent knee arthrodesis with a joint-spanning intramedullary nail and soft-…

The clinical and radiological results of individualized surgical treatment depending on pathologic abnormalities in recurrent patellar dislocation: low recurrence rate, but unintended patella baja.

PURPOSE: To evaluate the clinical and radiological outcomes and chondral lesion change using individualized surgery for recurrent patellar dislocation. METHODS: A total of 31 knees with recurrent patellar dislocation underwent surgery depending on individual pathologic abnormalities. Pathologic abnormalities including medial laxity, lateral tightness, increased tibial tuberosity (TT)-to-trochlear groove distance (>20 mm), and patella alta (Caton-Deschamps ratio >1.2) were evaluated in each patient. The abnormalities were corrected through medial patellofemoral ligament reconstruction, TT dist

[The direct, dorsal approach to the treatment of unstable tibial posteromedial fracture-dislocations].

The surgical treatment of highly unstable tibial posteromedial fracture-dislocations (medial split fracture,Moore type I) is demanding and may cause significant problems. The anterior and the posteromedial approach require detachment of the medial capsuloligamental structures or arthrotomy of the knee joint. Neither of these operative techniques allow the optimal posterior positioning of the buttress plate in order to prevent caudal dislocation of the fragment during knee flexion. The published posterior approaches to the proximal tibia allow appropriate positioning of the anti-glide plate. Ho

Acute Arthroscopic Removal of a Bullet Embedded in the Glenoid: A Case Report.

INTRODUCTION: Congenital dislocation of the patella (CDP) is a rare condition and its treatment is not well defined. In CDP, patella is dislocated on the lateral aspect of the distal femur, laterally from the trochlear groove, it cannot be reduced manually and it is almost always associated with genu valgum, lateral torsion of the proximal tibia, and flexion contracture of the knee. This condition is present at birth, but the clinical findings can be subtle at birth due to the size and character of the structures being examined; early diagnosis is essential, but it may be delayed to late child

Roentgenographic and clinical findings of patellofemoral osteoarthrosis. With special reference to its relationship to femorotibial osteoarthrosis and etiologic factors.

In order to determine definitive clinical and roentgenographic findings of patellofemoral osteoarthrosis (PF-OA) and the etiologic factors of this disease as well as its relationship to femorotibial osteoarthrosis (FT-OA), 108 knees having moderate to severe PF-OA with or without FT-OA were investigated. Pain and disabilities of activities of daily living (ADL) due to PF-OA most likely appeared during movements requiring deep flexion of the knee. Roentgenographic findings revealed a great majority of the disease occurred on the lateral side of the PF joint, both in cases of PF-OA alone and in

Medial patellofemoral ligament reconstruction with a synthetic polyester suture tape graft and knotless anchors: Five-year clinical and functional outcomes.

INTRODUCTION: Medial patellofemoral ligament (MPFL) reconstruction is a well-established treatment for recurrent lateral patellar dislocations, yielding satisfactory clinical outcomes. Although synthetic materials are not widely used due to limited long-term data, they offer the potential to eliminate donor-site complications and may provide promising results. This study evaluated the five-year clinical and functional outcomes of MPFL reconstruction using suture tape, hypothesising that it is a safe alternative to traditional grafts. METHODS: Thirty patients aged 20 - 45 years with recurrent

Reconstruction of the medical patellofemoral ligament through a double-bundle and suspension fixation of the adductor muscle tendon combined with semi-patellar ligament displacement for treating recurrent patellar dislocation in adolescents with unclosed epiphysis: a retrospective study.

BACKGROUND: Reconstruction of the medial patellofemoral ligament is the core of the treatment of recurrent patellar dislocation (RPD). For adolescents with unclosed epiphysis, the surgical method is the key to the postoperative efficacy. And our study aimed to evaluate the clinical efficacy and imaging results of double-bundle and suspension fixation of the adductor muscle tendon combined with semi-patellar ligament displacement in the treatment of RPD in adolescents with tuberosity-trochlear groove (TT-TG) distance > 15 mm and unclosed epiphysis. METHODS: A total of 27 patients (13 males, 14

Proximal tibiofibular joint dislocation in young children: Is this the nursemaid's elbow of the lower extremity?

Anterolateral dislocation of the Proximal Tibiofibular Joint (PTFJ) is a rare injury of the knee commonly resulting from violent athletic injuries in adults. Reported here are examples of this injury in a 19 month old and a 4 year old following trivial mechanisms of injury. These cases raise the question of whether this injury may be an unrecognized cause of refusal to bear weight in children in this age group.
