Construction of patient trajectories to model clinical trial outcomes: Application to Myasthenia Gravis.

Accurate prediction of patient outcomes in clinical trials is crucial for timely assessment of treatment efficacy. This study introduces a novel approach to predict patient response by constructing temporal trajectories from longitudinal clinical data. We aim to extrapolate these trajectories to forecast individual outcomes and identify when new patients align with established response patterns. Utilizing data from the MGTX trial of myasthenia gravis patients, we evaluate the predictability of these trajectories and discuss potential confounding factors. Furthermore, our analysis yields an aut…

Bridging the Heterogeneity of Myasthenia Gravis Severity Scores for Digital Twin Development.

Myasthenia gravis (MG) is a rare autoimmune neuromuscular disease. Clinical trials with rigorously collected data, especially for rare diseases, provide opportunities for mathematical modeling of patient outcomes over time; however, building a larger data set from multiple trials faces the challenge of harmonization of outcome measures. To accurately model MG and predict individual patient trajectories, one requires integrating three primary data types: (i) Laboratory and medication data, (ii) Electronic Health Record (EHR) data (e.g., age, sex, years since diagnosis, BMI), (iii) Disease sever…

Atypical Ocular Myasthenia-Like Presentation in a Patient With a Heterozygous SQSTM1 Variant.

We report the case of a patient presenting with diplopia, ptosis, and blurred vision who was found to have a heterozygous SQSTM1 gene variant (c.1175C>T) following electrophysiological evidence of a postsynaptic neuromuscular junction disorder. The patient had negative antibody testing, mildly elevated creatine phosphokinase (CPK), and abnormal organic acid analysis. Repetitive nerve stimulation demonstrated a decremental response consistent with impaired postsynaptic transmission. Given the atypical presentation and systemic findings, genetic testing was pursued, revealing a variant of uncert

Blink and You'll Miss It: Diagnostic Pitfalls in a Case of Missed Ocular Myasthenia Gravis in the Emergency Department.

Ocular myasthenia gravis (MG), a subtype of MG limited to the eyelids and extraocular muscles, is diagnostically challenging in the emergency department (ED). In the absence of generalized weakness, subtle symptoms such as ptosis and diplopia are more easily overlooked or misattributed to stroke or other neurologic pathologies. We present a case of a 69-year-old male whose diagnosis of ocular MG was delayed due to limited neurologic examination, underuse of simple bedside tools, and incidental imaging findings that initially suggested alternative endocrine pathology. Despite classic signs, inc…

The burden of disease in seronegative myasthenia gravis: a patient-centered perspective.

OBJECTIVE: Myasthenia gravis (MG) is an autoimmune disorder primarily caused by autoantibodies against the acetylcholine receptor (AChR). Approximately 15% of MG patients, categorized as seronegative (snMG), lack detectable antibodies. Due to the snMG status, there may be a diagnostic delay. Moreover, there are limited data on treatment response in comparison to AChR-Ab+ patients. This study examines the burden of disease, treatment response, and quality of life of snMG patients in comparison to AChR-ab+ MG patients and healthy controls. METHODS: A questionnaire-based survey was conducted col…

Rozanolixizumab for Myasthenia Gravis: a breakthrough treatment and future prospects.

Myasthenia gravis is a rare chronic autoimmune disorder affecting the post-synaptic neuromuscular junction, primarily mediated by pathogenic immunoglobulin G (IgG) targeting specific proteins like acetylcholine receptor (AChR), muscle-specific tyrosine kinase (MuSK), and low-density lipoprotein receptor-related protein 4 (LRP4). Modulating pathogenic IgG is a promising approach for disease management. Rozanolixizumab, a human IgG4 neonatal Fc receptor (FcRn) inhibitor, enhances the degradation of pathogenic IgG by 78%, marking a significant advancement in treating generalized myasthenia gravis…

Causal Variants in CHRNA1 and CHRNB1 Genes for Anti-acetylcholine Receptor Antibody Positive Myasthenia Gravis: Evidence from Bayesian Fine-Mapping and Genetic Association Study.

Autoantibodies target the acetylcholine receptor (AChR) in 85% of myasthenia gravis (MG) patients. Genomic studies highlighted the association of genes encoding AChR subunits (CHRNA1 and CHRNB1) and MG in European populations. Additionally, Mendelian randomization revealed rs4151121 at the CHRNB1 locus as a potential causal variant. Here, we performed Bayesian fine-mapping of the CHRNA1 locus using GWAS summary statistics, a linkage disequilibrium matrix and functional annotations. The GWAS lead hit rs35274388 was identified as a causal variant overlapping with the promoter region (p < 0.01). …
