Extensively drug-resistant tuberculosis.

Extensively drug-resistant tuberculosis (XDR-TB) is defined as Mycobacterium tuberculosis infection that is resistant to isoniazid, rifampin, any fluoroquinolone, and any injectable drug (amynoglicosides or polypetides). Although initially described in South Africa, it has emerged as a global threat, and cases have been reported from several countries, including the United States. XDR-TB has emerged mainly as a consequence of previous inadequate or poorly administered treatment, from failure of the public health infrastructure. As the diagnosis of this condition requires antibiotic susceptibil…

[Evaluation of the Widal test for diagnosing typhoid fever in Lebanon].

Typhoid fever is a major public health problem in developing countries. In Lebanon, where the sanitary infrastructure was severely damaged during the civil war, the high prevalence of typhoid fever creates a pressing need for a rapid and reliable diagnostic tool. To evaluate the diagnostic usefulness of the Widal test, patients with a positive blood culture for Salmonella typhi (n = 78) were compared to controls with fever due to another infection (n = 65) and with blood donors (n = 96). Discrimination between the patient group and the two control groups was best when an agglutinin O titer of

[Impact of climate change and pollution on the global health of older adults].

Climate change and pollution are major public health challenges, with older adults particularly at risk due to frailty, multimorbidity, social isolation, and economic vulnerability. From a global health perspective, in which human health is closely linked to surrounding fauna, flora, and the broader environment, environmental exposures are key determinants of healthy ageing. We conducted a narrative review (2000-2025) based on PubMed, Google Scholar, and institutional reports (World Health Organization, IPCC, Inserm). The review focused on adults aged ≥ 60 years and the health impacts associat

Chinese Medicine as First-Line Treatment for Subthreshold Mental Disorders in Primary Care: Opportunities and Challenges.

Subthreshold mental disorders (SMDs), characterized by clusters of psychiatric symptoms that do not meet the criteria for a formal diagnosis yet are sufficiently severe to impair daily functioning. SMDs exhibit a high prevalence and an elevated risk of progression to diagnosed disorders and impose a substantial socioeconomic burden. Despite their significant impact, SMDs often go overlooked and untreated due to a global shortage of mental health professionals and stigmatization associated with conventional psychological and psychiatric treatments. This perspective advocates the integration of

Malaria control in Malawi: current status and directions for the future.

The last decade has seen an increase in investment and concerted efforts by the Malawi Ministry of Health and partners to control malaria disease. This report summarizes what is known about the burden of malaria and the strategies being implemented to control it in Malawi. Over the past 5 years, roll out of treatment and prevention efforts have been successful in the country, as demonstrated by increased use of insecticide treated nets, improved access to prompt and effective treatment and the initiation of pilot studies of indoor residual spraying. However, unlike other countries in the regio

Medical exposure to micro(nano)plastics: An exposure pathway with potentially significant harm to human health that should not be overlooked.

Micro(nano)plastics (MNPs) are an emerging type of contaminants that are widely present in the environments that people live in. MNPs can enter the human body in a variety of pathways, but the three main ones are through dietary intake, air inhalation, and skin contact. However, it has been discovered that medical plastics used in medical activities also pose potential risks to MNPs exposure as exposure pathways are continuously refined and clarified. Unfortunately, there is currently insufficient study on the exposure of medical plastics and MNPs, and exposure risks and potential health probl

Feasibility of mainstreaming schistosomiasis and soil transmitted helminthiasis mass drug administration into primary healthcare: a baseline survey for a community case study in Western Kenya.

BACKGROUND: Mass drug administration campaigns have enabled great progress in the prevention and control of schistosomiasis and soil transmitted helminthiasis. However, the persistence of these infections continues to challenge public health systems despite decades of control efforts. This study was commissioned to evaluate the feasibility of a transition from periodic mass drug administration campaigns to a sustainable, facility- and community-based delivery model in Western Kenya. METHODS: The study is part of a non-randomized, controlled before-and-after study using a mixed-methods approac

Transforming long-term post-acute care for the aging population through home infusion therapy in China: an assessment of need and demand (Part 1).

BACKGROUND: China's healthcare system is confronting a rising burden of antimicrobial resistance, chronic complex diseases, and an aging population requiring long-term, post-acute care. As a value-based care model in the USA for over 50 years, home infusion therapy (HIT), combined with home health services (HHS), presents a sustainable alternative to prolonged hospitalization for intravenous medication administration. In this two-part narrative review, Part 1 provides a needs assessment describing the demographic, clinical, and public health factors driving the demand for HIT and HHS in China.

State-level analysis of mental health disparities between White, Black, and Hispanic populations before and after COVID-19.

The COVID-19 pandemic had disparate impacts on Black and Hispanic Americans, as observed in mortality and infection rates. The mental health impact of the pandemic is less clear, with little research exploring mental health disparities and differences by state and region. Using the Behavioral Risk Factor Surveillance System (BRFSS), state-level differences in unhealthy mental health days were examined between 2019 and 2021 for White, Black, and Hispanic respondents. Nonparametric tests assessed group differences in unhealthy mental health days, while state-level within- and between-group diffe

Human monkeypox.

Human monkeypox is a zoonotic Orthopoxvirus with a presentation similar to smallpox. Clinical differentiation of the disease from smallpox and varicella is difficult. Laboratory diagnostics are principal components to identification and surveillance of disease, and new tests are needed for a more precise and rapid diagnosis. The majority of human infections occur in Central Africa, where surveillance in rural areas with poor infrastructure is difficult but can be accomplished with evidence-guided tools and educational materials to inform public health workers of important principles. Considerations regarding mass vaccination against typhoid fever as an adjunct to sanitation and public health measures: potential use in an epidemic in Tajikistan.

We report on the ongoing epidemic of typhoid fever in Tajikistan that started in 1996. It has involved more than 24,000 cases to date, and is characterized by multiple point sources, overflow of sewage, contaminated municipal water, and person-to-person spread. Of the Salmonella typhi isolates available for testing in western laboratories, more than 90% are multidrug-resistant (MDR). Most recently, 28 (82%) of 34 isolates are resistant to ciprofloxacin, representing the first reported epidemic of quinolone-resistant typhoid fever. In the past, mass immunization during typhoid fever epidemics h

HIV/AIDS, parasites and co-infections: publication patterns in China.

BACKGROUND: Since its discovery, HIV/AIDS has arguably captured more attention among the Chinese biomedical research community than most other infectious diseases. Traditional parasitic diseases, on the other hand, are perceived as being increasingly neglected. However, it has long been recognized that interactions between HIV and other infective agents, including parasites, influence the health status of people living with HIV/AIDS. This study aimed at systematically reviewing the Chinese scientific literature on HIV/AIDS and parasites between 1986 and 2006 in order to substantiate or refute

Retinotopy drives the variation in scene responses across visual field map divisions of the occipital place area.

IMPORTANCE: The COVID-19 pandemic has caused millions of infections and deaths and resulted in unprecedented international public health social and economic crises. As SARS-CoV-2 spread across the globe and its impact became evident, the development of safe and effective vaccines became a priority. Outlining the processes used to establish and support the conduct of the phase 3 randomized clinical trials that led to the rapid emergency use authorization and approval of several COVID-19 vaccines is of major significance for current and future pandemic response efforts. OBSERVATIONS: To support
